[Trigeminal radiofrequency rhizotomy for the treatment of trigeminal neuralgia: results and technical modification].

The purpose of this study was to evaluate the efficacy of radiofrequency trigeminal rhizotomy in treating 135 patients harboring trigeminal neuralgia, and to introduce a technical modification to guide the puncture of the foramen ovale. A hundred and one (74.8%) patients were treated with a single surgical procedure whereas the 34 (25.2%) remaining patients required two procedures. Follow-up ranges from 6 months to 15 years. Pain relief in the immediate postoperative was achieved in 131 (97.0%) patients. After the initial procedure, recurrence happened in 33 (24.5%) patients. The complications included decrease corneal reflex (4.4%), masseter paresis (2.2%), painful dysesthesia (1.5%) and anesthesia dolorosa (0.7%). The radiofrequency trigeminal rizhotomy is a low risk, highly effective and minimally invasive procedure. The use of the computerized tomography guided fluoroscopy turns foramen ovale's puncture easier, fast and precise.

[Neurovascular structures of the posterior surface of the petrous pyramid: correlation with the approaches of the cerebellopontine angle].

A topographic study of posterior surface of the petrous pyramid was performed in 20 human cadaveri heads. The distances between the neurovascular structures were measured in the points where they contact the posterior surface of the petrous pyramid. The study also points out the relationship between the bone landmarks and the transverse and the superior petrous sinuses. The result of this study was correlated with the approaches to the cerebellopontine angle.

[Suction-irrigator device for microsurgery: technical note].

A modification of the conventional suction device for microsurgery is described. It consists of a built-in tube in another tube, being the first connected to the suction device and the second to the irrigation. This suction-irrigator device allows to accomplish the suction and irrigation simultaneously and in a precise way.

[Landmarks to the cranial approaches].

The knowledge of the craniotopography allows the delimitation of the cranial approaches. In this study the landmarks, defined in relation to the craniometric points and used in the different cranial approaches, were systematized. Twenty two landmarks are described: the first twelve are in relation to the skull base and the remainder are in relation to the skull vertex.

Percutaneous trigeminal nerve radiofrequency rhizotomy guided by computerized tomography fluoroscopy. Technical note.

The authors describe a new procedure for percutaneous trigeminal radiofrequency rhizotomy. Computerized tomography fluoroscopy is used for guidance of the rhizotomy needle insertion through the foramen ovale. Ten patients were treated using this method, and in each case the target was reached with a single puncture. The potential benefits of this method are presented.

[Definition of the anterolateral occipital lobe limit in anatomical specimens and image examination].

The anterolateral limit of the occipital lobe was studied in anatomical specimens and with neuroimaging. Seven human cadaver heads, 103 normal CT-scan and 104 MRJ of the brain were studied. There was a fold of the dura mater on the transverse sinus (preoccipital tentorial plica) and a bony protuberance related directly to the preoccipital notch. It was also determined the mean distance between the parieto-occipital sulcus and the lambdoid suture. In the imaging studies, especially magnetic resonance, it was possible to identify the preoccipital notch and/or a protuberance in the cranial vault related to this notch, besides the parieto-occipital sulcus and lambdoid suture, making possible, therefore, the definition of the anterolateral limit of the occipital lobe.
