Foetal echocardiography.

A modern diagnostic and therapeutical approach to paediatric cardiology enables early application of foetal echocardiography in order of achieving diagnosis of congenital heart anomalies in utero. The aim of this study is to evaluate the percentage of prenatal diagnosis of congenital heart anomalies. This study has been conducted on 73 patients at Paediatric clinic of Clinical Centre of Sarajevo in a period from January 2000 until December 2004 with diagnosis of heart malformations. Among them 14 were preterm newborns, 40 boys. Diagnosis of cardiac anomalies with left to right shunt was done in 56.1%, obstructive 13.7%, cyanotic 1.36% and complex in 28.7% patients. The prenatal diagnosis was established in 4 patients (5.5%) by ultrasound examination which is very low in comparison to other European countries. There is a need for making prenatal diagnosis of congenital heart anomalies in foetus as early as it can be done in order to treat cardiac anomalies in utero, to decrease the number of congenital heart anomalies and to reduce the cost of cardiosurgical and postsurgical treatment.

Saturation with oxygen for ductal dependent congenital heart diseases before and after the prostaglandin therapy.

Ductal dependent congenital heart diseases represent 14-20% of all congenital heart diseases. A primary goal of the treatment of these diseases is to retain ductus open until the final cardiosurgical treatment. Prostaglandins are presently the only medicaments, which have a capability to keep ductus open. By means of a retrospective study in a period from January, 2000 until December, 2002 at the Paediatric clinic of the Clinical centre of the University in Sarajevo, 14 patients (treated with prostaglandins) diagnosed with ductal dependent congenital heart diseases were analyzed. In our sample, there are 9/14 male patients (64.3%), 11/14 (78.6%) were full-term newborns, while 10/14 (71.4%) were eutrophic at birth. An average saturation increase, after the prostaglandin therapy, measured in blood from the capillaries is 29, and measured transcutanlly is 32 units. Duration of prostaglandin therapy in our study was on average 17.2 days. The most common cause of death was insufficientia cardiorespiratoria (4 out of 11), but sepsis/infection (3 out of 11) and insufficientia renalis were also common. 78.6% (11 out of 14) patients died partly because of the complexity of these diseases, but also because a cardiosurgical treatment is delayed. A goal of this study is evaluation of saturation with oxygen before and after the prostaglandin therapy.

[Prognostic significance of juvenile chronic arthritis onset types].

Juvenile chronic arthritis (JCA) is a disease or a group of diseases with chronic nonsuportive connective tissue inflammation. Depending of the onset of the disease and its course in the first six months there are three major types: systemic, poliyarticular and pauciarticular disease. Indicators of bad prognosis were: systemic form with signs of inflammation in the first six months, poliarticular onset and course, RF positivity, positive ANA and eye involvement. In the period from 1996 to 2002 at the Cardiology-Rheumatology Department in Sarajevo Pediatric Clinic 63 patients were addmited. Out of this 49 (78%) were new cases JCA was more frequent among school age girls 35 (57%). Systemic--onset JCA 9 patients (14%), polyarticular onset 19 patients (30%), pauciarticular onset 35 (55%). 15 (23.8%) was RF (rheumatoid factor) negative, 4 (6.3%) RF positive. Pauciarticular forms type 1 were 20 (31.7%), type II were 15 (23.8%). In 11 (17.5%) patients laboratory tests and clinical signs indicated poor prognosis. All patients with indicators of poor prognosis needed more aggressive therapy. Although JCA is the most frequent rheumatic disease and the leading cause of invalidity, and one of the most frequent chronic disease in children, we still do not know enough. The optimal treatment is still a challenge especially onset of JCA with poor prognosis.

[Risk factors for the development of congenital heart defects in children born in the Tuzla Canton].

Congenital heart diseases (CHD) are taking high position on the list of neonatal and infant mortality, and they are significantly involved in mortality of children and adults. During cardiogenesis various genetic and non-genetic ethiological factors are starting pathogenetic mechanism what results in developing of CHD. Purpose of research is evaluation of participation of some risk-factors in developing of CHD in children on Tuzla Canton area. This research included 352 children up to 15 years old, in which was discovered CHD during period from 1.1.1994 to 31.12.1999. Using method of anamnestic questionnaire we analyzed data related to occurence of CHD in relatives, reproductive age of mother, mother's illness and taking medications in first trimester of pregnancy, and professional exposure to harmful chemicals. In 24 or 6.81% of cases CHD was registered in one of first-degree relatives, and in only one case in second-degree of relatives. It was noticeable that CHD in highest number of cases (291 or 83.14%) were present in children whose mothers were 20-35 years old, while only 18 or 5.11% of mothers aged 35 and more what is considered as high risk age for development of anomalies in general. In 70 or 20% cases CHD was associated with other systems anomalies, including syndromes and chromosomopathies. Risk-factors related to earlier pregnancies of mothers were found in 40 (11.36%) of children. Data about acute and chronic diseases of mother during pregnancy were found in 36 children. analysis of exposure to harmful chemicals during first trimester of pregnancy is showing that 39 or 11.08% mothers were exposed to nicotine, while 17 or 4.83% were taking medications. Exposure to professional poisoning were found in 5 or 1.42% of cases. Results of research are pointing on need for continued observation genetic and wide range of other potential risk-factors from environment related to development of CHD in our area. This way we could explain some specific characteristic occurrences of CHD in some areas.

[Monitoring oxygen saturation in the pediatric population].

Pulsoxymetry is noninvasive technique which is in use for percentage estimation of hemoglobin saturation of oxygen in arterial blood. It represents a therapeutical screening of treatment in paediatric population. The aim of this retrospective study is to evaluate the validity of noninvasive technique for determination of oxygen concentration in cardiac and pulomological patients. The study included 221 patients, with a mean age of 39 months (1.5-192) who were hospitalised during the period of 1.6.2000. till 31.12.2002. at Pulmoalergology and Cardiorheumatology department of Paediatric clinic CCU Sarajevo. Three groups of patients were evaluated: first one, which included 194/221 patients with obstructive and inflammatory lung diseases, 17/221 patients formed II group of pts with congenital heart anomalies (CHA) and the third one which had 10/221 patients with CHA associated with lung diseases. In all patients the diagnostic-laboratory investigations were done including transcutaneous determination of oxygen saturation by pulsoxyimetry (PO) Nellcor and Johnson-Johnson, as well as capillary, with adequate treatment. The mean oxygen saturation (OS) in the first group of patients at admission by PO was 86%, capillary 73.6%, and after treatment OS PO was 91.6%. In the II group the mean OS at admission was 88.5% PO, capillary 83%, after treatment 94.9%. The mean OS at admission in third patient's group was 76%, and after treatment 91.2% which is statistically significant (F = 0.03) and is in concordance with modern therapeutical approach. Pulseoxymetry is noninvasive, painless, simple method which offers a valid continuous data needed for the adequate choice of treatment in paediatric patients.

[Morbidity trends in the cardio-rheumatology department of the Pediatric Clinic in Sarajevo during the pre-war, war and post-war periods].

In this work is presented the structure of the morbidity of the patients treated at the Cardiohematology department of the Pediatric Clinic in Sarajevo. It were followed four the most frequent groups of the diseases. The results show the trend of the increasing of the number of the hospitalized children with the congenital anomalies of the heart and juvenile chronic arthritis, with the decrease of the number of the ill become by streptococcal infections of the upper respiratory tract and the rheumatic illness. It is increased the number of the hospitalized children is the result of the beginning of the cardiosurgical way of the taking care of these patients. The decreased of the number of the ill become is the consequence of the improvement of the healthcare protection at the primary level with the continuity of the further trend of the decrease of the number of the patients ill become from acute rheumatic disease.
