MDCT of small-bowel disease: value of 3D imaging.

OBJECTIVE: Our objective is to show the various clinical applications of MDCT enterography for evaluating small-bowel disease, with a focus on the added value of 3D imaging. CONCLUSION: MDCT and refined 3D imaging processes can offer a full examination of the small bowel as well as powerful information about the bowel and its surrounding structures.

Inferior phrenic arterial bleeding after adult liver transplantation: incidence, clinical manifestations, and predictive CT features.

OBJECTIVE: The purpose of this study was to determine the incidence of inferior phrenic arterial bleeding after liver transplantation in adult patients and to describe its clinical manifestations and predictive CT features. CONCLUSION: Inferior phrenic arterial bleeding occurs frequently and is one of the major postoperative bleeding sites in patients undergoing liver transplantation. Awareness of its common clinical manifestations and predictive CT features is essential for prompt diagnosis and treatment of this early vascular complication, and this knowledge can help to avoid unnecessary reoperation and catastrophic hypotension.

Biliary ductal evaluation of hilar cholangiocarcinoma: three-dimensional direct multi-detector row CT cholangiographic findings versus surgical and pathologic results--feasibility study.

The study was conducted, with institutional review board approval and informed patient consent, to assess the feasibility and diagnostic effectiveness of three-dimensional direct multi-detector row computed tomographic (CT) cholangiography for determining the extent of bile duct invasion by hilar cholangiocarcinoma. Eleven patients underwent contrast material-enhanced direct multi-detector row CT cholangiography of the primary and secondary biliary confluence levels and then surgical resection. In most patients, CT cholangiography was tolerable and yielded excellent or good opacification of the biliary tree. CT cholangiography enabled a correct diagnosis of the extent of ductal involvement at all 11 primary confluence levels and at 18 of the 19 secondary confluence levels. Three secondary confluences, which could not be analyzed owing to nonopacification or poor opacification, proved to be involved by hilar cholangiocarcinoma. The authors conclude that three-dimensional direct multi-detector row CT cholangiography is accurate and feasible for defining the extent of ductal invasion by hilar cholangiocarcinoma, especially in patients with preliminary biliary drainage.

Wash-in rate on the basis of dynamic contrast-enhanced MRI: usefulness for prostate cancer detection and localization.

PURPOSE: To evaluate the usefulness of the wash-in rate based on dynamic contrast-enhanced (DCE) MRI for the detection and localization of prostate cancer. MATERIALS AND METHODS: In 53 patients, the wash-in rate was measured in the cancer area and in three normal areas (the peripheral zone, inner portion of the transitional zone, and outer portion of the transitional zone). On the basis of these data, parametric imaging was generated and then its accuracy for cancer detection and location was evaluated compared to that of T2-weighted imaging without the use of an endorectal coil. For that purpose the entire prostate was divided into 18 segments. RESULTS: The wash-in rate value was greater in cancer tissue (9.2/second) than in three normal tissues (3.3/second, 6.7/second, and 3.2/second, respectively; P<0.001). The sensitivity and specificity were greater on parametric imaging of the wash-in rate compared to T2-weighted imaging in the entire prostate (96% and 82% vs. 65% and 60%, respectively) and the peripheral zone (96% and 97% vs. 75% and 53%; P<0.05). In the transitional zone, the sensitivity was greater on parametric imaging (96%) than on T2-weighted imaging (45%; P=0.016), but the specificity was similar (51% vs. 73%; P=0.102). CONCLUSION: The wash-in rate based on DCE-MRI is a useful parameter for prostate cancer detection and localization.

Extrahepatic spread of hepatocellular carcinoma: a pictorial review.

Although extrahepatic spread of hepatocellular carcinoma (HCC) is uncommon, it can be found anywhere in the body. Most extrahepatic metastases of HCC occur in patients with advanced-stage intrahepatic tumor, but incidental extrahepatic lesions have also occasionally been found in patients with early-stage intrahepatic HCC. The detection of extrahepatic metastatic disease is crucial when planning therapy for patients with HCC and should be used to avoid unnecessary surgical intervention. In this study we illustrate the radiologic findings of extrahepatic metastases of HCC involving various sites. The presumed mechanism of extrahepatic extension of HCC is also discussed.

Intravascular papillary endothelial hyperplasia of the lung.

Intravascular papillary endothelial hyperplasia (IPEH) is a benign vascular endothelial proliferation, which is usually confined to the lumen of preexisting vessels or vascular malformation. IPEH usually presents as a single lesion, and it rarely involves the lung. We describe a rare case of intrapulmonary IPEH presented by multiple variably sized, well-defined nodules occurring in a 52-year-old woman.

Current diagnostic role of CT in evaluating internal hernia.

Transmesenteric hernia is frequently encountered in practice, and because of the high risk of strangulation, even small internal hernias are dangerous. Knowledge of the characteristic imaging features and the mechanism of internal hernia can lead to early detection and correct diagnosis, both of which can save the patient's life. This article describes common CT findings of internal hernia with emphasis on various postoperative transmesenteric hernias as well as other common internal hernias.
