Transient postpartum diabetes insipidus associated with HELLP syndrome.

Diabetes insipidus in pregnancy has different causes. The association of diabetes insipidus with disturbances of liver function has been reported, however, diabetes insipidus has rarely been reported in HELLP syndrome. We present a 23-year-old primigravida with a singleton gestation complicated by HELLP syndrome who developed postpartum diabetes insipidus. Labor was induced promptly to terminate pregnancy because of intrauterine fetal death and liver dysfunction. 1-deamino-8-D-arginine-vasopressin was administered. Diabetes insipidus and liver dysfunction resolved within 2 weeks. Development of diabetes insipidus may result from increased vasopressinase activity mainly caused by deterioration of liver functions caused by HELLP syndrome. In pregnant women with liver disease as a result of any cause, the development of diabetes insipidus should be assessed with particular attention.

TSH influences serum leptin levels independent of thyroid hormones in hypothyroid and hyperthyroid patients.

Leptin is considered to play a role in maintenance of energy balance and body weight by neuroendocrine mechanisms. The physiological mechanisms for thyroid hormone-induced alteration in serum leptin are not well known. In the present study, the relationship between thyroid hormones and leptin levels was investigated in patients with overt hypothyroidism and hyperthyroidism before and after successful treatment. Leptin levels were determined by radioimmunoassay and body mass index (BMI) was calculated for each subject. Serum leptin levels of 26 hypothyroid and 22 hyperthyroid patients were compared with those of 20 healthy volunteers who comprised the controls. Serum leptin levels of hypothyroid patients (28.4 +/- 4.1 ng/ml) were found to be significantly higher than the controls (19.1 +/- 3.2 ng/ml) (p<0.01), whereas hyperthyroid patients had lower levels (10.7 +/- 1.2 ng/ml) (p<0.01). In hypothyroid patients, serum leptin levels were decreased significantly to 20.6 +/- 2.1 ng/ml with thyroxin treatment (p<0.05). However, in hyperthyroid group, serum leptin levels were increased to 12.4 +/- 2.2 ng/ml by treatment (p>0.05). BMI was not changed with the treatment in either group. The serum leptin levels were correlated with BMI and thyrotropin (TSH) in both hypothyroid and hyperthyroid patients. Serum leptin levels are affected in thyroid disorders and the correlation of leptin with TSH is independent of thyroid hormones.

Doppler sonographic evaluation of cerebral blood flow in anemia resulting from chronic renal failure.

OBJECTIVE: This study was undertaken to describe Doppler sonographic measurement of cerebral blood flow in anemia secondary to chronic renal failure and to compare the results with data gathered from healthy control subjects. We also aimed to compare vertebral and internal carotid artery blood flows to see whether any alteration of dominance in cerebral blood supply would occur. METHODS: We studied 27 predialytic patients with chronic anemia resulting from chronic renal failure and 20 healthy control subjects by means of extracranial Doppler sonography. In these patients, blood flows of bilateral internal carotid and vertebral arteries were measured, and net vertebral artery, net internal carotid artery, and total cerebral blood flows were estimated. Statistical significance was observed between groups, and the data were correlated with hemoglobin level. Variation of the difference of the Doppler measurements between case and control groups by side (left or right) or disease status (patient or control subject) was analyzed. RESULTS: From the assessed Doppler parameters, only cerebral blood flow and right and net vertebral artery blood flows had a significant difference between groups (P < .05) and showed a negative correlation with hemoglobin level. Vertebral artery blood flow was found to have significant interactions with disease status (P = .009) and side (P = .054). CONCLUSIONS: Right vertebral artery blood flow is most prone to increase in chronic anemia of chronic renal failure. This effect also appears as increasing net vertebral artery blood flow and cerebral blood flow.

Cutaneous lobular capillary hemangioma induced by pregnancy.

Lobular capillary hemangioma, which is also named as pyogenic granuloma, is a common vascular proliferation of skin and mucous membranes. Hormonal influence in the development of lobular capillary hemangioma on the mucosal surfaces has been demonstrated but for cutaneous lesions located on the skin, this association has not been shown yet. In this report, a 25-year-old female patient with cutaneous lobular capillary hemangioma of the cheek induced by pregnancy is presented. The lesion was excised and lobular capillary hemangioma was given as diagnosis by histopathological evaluation. Estrogen receptor was weakly positive by immunohistochemical analysis. We suggest that elevated levels of estrogens during pregnancy may have an important role also on the development of cutaneous lobular capillary hemangiomas by direct hormonal action.

Changes in serum markers of oxidative stress with varying periods of haemodialysis.

BACKGROUND AND AIMS: Oxidative stress possibly helps promote the progression and complications of chronic renal failure (CRF). Haemodialysis (HD) may aggravate oxidative stress. This controlled, cross-sectional clinical study with blind outcome assessment evaluated the effect of prolonged HD treatment on oxidative stress. METHODS AND RESULTS: Seventy patients (M/F = 33/37) with CRF and who were on HD were divided into six groups with differing treatment periods of HD; from 3 to 12 months to 85-120 months. Twelve healthy subjects acted as controls. The serum levels of thiobarbituric acid reactive substances (TBARS), total sulfhydryl (SH) groups and protein carbonyls (PCs) were determined. Compared with controls, PCs were increased in patients, and this positively correlated with the duration of HD. Treatment for more than 24 months caused the most striking increases in PCs, with relevant differences as compared to those on HD for a maximum of 12 months. Increasing periods of HD were associated with increases in TBARS and similar decreases in SH as compared with controls; the differences between the SH levels of those on HD for more than 84 months and those on HD for up to 24 months were significant. CONCLUSION: Our results indicate that HD continued for more than 2 years aggravates the latter. Decreased potential for oxygen-radical-scavenger activity becomes pronounced after 7 years of HD treatment.

Alkaptonuric ochronosis: a case with multiple joint replacement arthroplasties.

Alkaptonuria is an extremely rare hereditary disorder of metabolism which may affect various systems. Degeneration of the articular surfaces in major joints leads to complications, which may further necessitate joint replacements. In this report, a case of advanced alkaptonuria with multiple joint replacements is presented and musculoskeletal system involvement in this disorder is reviewed.

Usefulness of the Helicobacter pylori stool antigen test for detection Helicobacter pylori infection.

Several diagnostic tests are available for evaluating Helicobacter pylori (H. pylori) infection: histological examination, culture of gastric biopsy specimens, rapid urease test, urea breath test and serology. In this study, we assessed the reliability of a newly developed enzyme immunoassay HpSA (H. pylori Stool Antigen) kit for detecting H. pylori antigen in stool. Eighty-five patients (50 males, 35 females; mean age 41.6 +/- 9.8 years) with dyspeptic symptoms who were examined by upper gastrointestinal endoscopy. The patients with a history of previous treatment with proton pump inhibitors, bismuth compounds or antibiotics were excluded. During the endoscopic examination biopsies were taken from antrum and corpus for rapid urease test and histological examination. Stool specimens were submitted to the laboratory and HpSA test was performed. H. pylori was considered in condition with rapid urease test and histopathological examination for H. pylori positive. Forty-six of 85 patients were positive and remaining 39 patients were negative for H. pylori with the rapid urease test and pathologic evaluation. When 0.160 was adopted as the cut-off value, in accordance with the manufacturer's recommendations; stool antigen has been detected in 45 of the 46 H. pylori positive patients. The sensitivity and specificity of HpSA test were 97.8%, 94.9% respectively. These results indicate that HpSA is a highly reliable diagnostic method for H. pylori infection.
