PUS7-dependent Ψ reshapes specific synaptic gene exons to facilitate fear extinction memory formation.

RNA modifications serve as dynamic regulators of neural plasticity through their ability to fine-tune transcript stability and splicing. Pseudouridine (Ψ), an evolutionarily conserved RNA modification catalyzed by pseudouridine synthases, plays established roles in neurodevelopment, yet its functional significance in activity-dependent behavioral adaptation remains poorly defined. Here, we investigate Ψ-mediated epitranscriptomic regulation within the infralimbic prefrontal cortex (ILPFC), a brain region requiring precise synaptic remodeling for the clinically relevant form of fear extinction memory. Combining transcriptome-wide pseudouridylation profiling with behavioral analysis in mice, we identified selective Ψ enrichment at exons of synaptic regulatory genes within ILPFC during fear extinction learning. Fear extinction in the ILPFC drives concomitant exonic Ψ deposition and upregulation of synaptogenic transcripts, processes that involve pseudouridine synthase PUS7. Crucially, PUS7 knockdown in the ILPFC selectively impaired fear extinction memory formation without altering baseline fear expression, establishing a causal link between Ψ-dependent RNA processing and activity-dependent synaptic structural remodeling in this microcircuit. Our findings demonstrate that PUS7-mediated Ψ modification spatiotemporally regulates activity-dependent RNA dynamics in the ILPFC, providing the evidence that epitranscriptomic mechanisms precisely coordinate synaptic gene expression within behaviorally defined brain sub-region. This work bridges molecular RNA biology with systems neuroscience, revealing a novel mechanism for activity-dependent regulation of fear extinction in ILPFC.

[Auditory capability evaluation for children after cochlear implantation using meaningful auditory integration scale].

OBJECTIVE: To analyze the auditory capability of preschool children before and after cochlear implantation using meaningful auditory integration scale (MAIS) questionnaire. METHODS: Eighty-two prelingually deaf patients participated in this study. They received a cochlear implant at the age of 3 to 6 years and 11 months. The audiologists who were trained for the research used the MAIS questionnaire. Audiologists asked for the parents' answers and recorded all the information about the device using (Q1,2) and the patient's spontaneous auditory behavioural responses including spontaneous alerting to sound Q3 approximately 6 and deriving meaning from sound (Q7 approximately 10). The evaluation was performed before operation and 1 , 3, 6 months, 1, 1.5, 2 years after switch-on. RESULTS: The scores of question 1a and 1b were not significantly different among the different periods after switch-on. The scores of question 2 to 10 were significantly different among the different periods after switch-on. CONCLUSIONS: Considerable variability across subjects' auditory ability after cochlear implantation was noted. Most of the patients showed no consistent response to sound in everyday life before implantation. After cochlear implantation, a significant increase in auditory capability occurred. The children demonstrated faster development of device using relative to spontaneous alerting to sound and deriving meaning from sound.

[Analysis of the found age and way for children age 0 to 6 with hearing loss].

OBJECTIVE: To investigate the children with hearing loss from the age 0 to 6, and discuss the found age, found way and audiological characteristics. METHODS: General information of found age and found way of 265 children, were investigated with self-made questionnaire and routine audiological evaluations, and then made statistical analysis. RESULTS: The average (x +/- s) found age for the children with hearing loss was (23.21 +/- 10.02) months, and the first average coming age was (28.01 +/- 13.41) months. The found age of girls [(27.11 +/- 13.13) months] was 6.1 months later than the boys' [(21.03 +/- 12.32) months] and the countryside children [(28.27 +/- 11.09) months] later than the city's [(19.52 +/- 13.05) months] 8.65 months in the average found age. The found age of children who were found with speech disability was later than others. As the hearing loss degree of children went milder, the found age might later. CONCLUSIONS: The popularization of knowledge in preventing from hearing loss must be strengthened. It is also necessary to popularize newborn hearing screening and early intervention while to enhance the parents' consciousness.

[Studies on ex vivo expansion of megakaryocytic progenitor and its application--review].

Application of ex vivo expanded megakaryocytic progenitor cells (MKPC) is a strategy for the treatment of thrombocytopenia after hematopoietic stem cell transplantation. Some growth factors including thrombopoietin (TPO), megakaryocyte growth and development factor (MGDF), interleukin (IL)-1, IL-3, IL-6, IL-11, platelet-derived growth factor (PDGF), and serotonin (5-HT) have been demonstrated to play an important role on the regulation of megakaryocyte/platelet development, the efficient conditions for the expansion of the megakaryocyte (MK) progenitors from hematopoietic stem/progenitor cells were discussed in this review article. TPO alone produced a high proportion of MK progenitors but a low total cell count. The addition of IL-1 beta, IL-3, IL-6 and Flt-3L improved the expansion outcome. The combination of three to five cytokines produced more efficient expansions of hematopoietic stem and MK progenitors. PDGF also enhanced the ex vivo expansion of CD61+ CD41+ cells and CD34+ cells in combination with TPO, IL-1 beta, IL-3, IL-6 and Flt-3L. PDGF is a suitable growth factor to improve the ex vivo expansion of MKPC without promoting their in vitro maturation. More importantly, PDGF also enhanced the engraftment of human stem and progenitor cells in NOD/SCID mice. It has been reported that MKPC can be safely administered to autologous peripheral blood progenitor cell transplant recipients. In short, MKPC can be expanded ex vivo and safely applied to autologous transplant.

[Influence of substance P on the release of histamine in the human hypertrophic scar tissue].

OBJECTIVE: To investigate the influence of substance P (SP) on the release of histamine in the human hypertrophic scar tissue, and to explore the prerequisite of their interaction. METHODS: Tissue specimens of normal skin and hypertrophic scar from eight hospitalized patients were excised and cut into 0.5 to 1.0 mm3 pieces, and the histamine release by mast cell (MC) under the stimulation of different concentration of SP and calcium, as well as the different affect time of SP, were determined with fluorescence spectrometer. Then the histamine release rate was calculated. RESULTS: There was obvious release of histamine when SP concentration was 1 x 10(-6) mol/L , and the release rate was (50.0 +/- 3.6) %, which was significantly higher than that by SP in the concentration of 0 mol/L [(44.0 +/- 3.2) %, P < 0.01]. Therefore it seemed to be dose-dependent. About 90% of histamine was released within 15 minutes of 5 x 10(-1) mol/L Substance P stimulation, and it was also time-dependent. The histamine release reached the peak when calcium concentration was 5 x 10(-3) mol/L, which seemed to be dose-dependent, but it decreased transiently when calcium concentration was 1 x 10(-3) mol/L. In all occasions, the influence of SP on the histamine release by MC in hypertrophic scar (HS) was markedly higher than that in normal skin (NS) (P < 0.01). Conclusion The influence of SP on the histamine release by MC in HS was markedly higher than that in NS, and it might be closely related to itching sensation and the formation of hypertrophic scar.

Biological characterization of a heterodimer-selective retinoid X receptor modulator: potential benefits for the treatment of type 2 diabetes.

Specific retinoid X receptor (RXR) agonists, such as LG100268 (LG268), and the thiazolidinedione (TZD) PPARgamma agonists, such as rosiglitazone, produce insulin sensitization in rodent models of insulin resistance and type 2 diabetes. In sharp contrast to the TZDs that produce significant increases in body weight gain, RXR agonists reduce body weight gain and food consumption. Unfortunately, RXR agonists also suppress the thyroid hormone axis and generally produce hypertriglyceridemia. Heterodimer-selective RXR modulators have been identified that, in rodents, retain the metabolic benefits of RXR agonists with reduced side effects. These modulators bind specifically to RXR with high affinity and are RXR homodimer partial agonists. Although RXR agonists activate many heterodimer partners, these modulators selectively activate RXR:PPARalpha and RXR:PPARgamma, but not RXR:RARalpha, RXR:LXRalpha, RXR:LXRbeta, or RXR:FXRalpha. We report the in vivo characterization of one RXR modulator, LG101506 (LG1506). In Zucker fatty (fa/fa) rats, LG1506 is a potent insulin sensitizer that also enhances the insulin-sensitizing activities of rosiglitazone. Administration of LG1506 reduces both body weight gain and food consumption and blocks the TZD-induced weight gain when coadministered with rosiglitazone. LG1506 does not significantly suppress the thyroid hormone axis in rats, nor does it elevate triglycerides in Sprague Dawley rats. However, LG1506 produces a unique pattern of triglycerides elevation in Zucker rats. LG1506 elevates high-density lipoprotein cholesterol in humanized apolipoprotein A-1-transgenic mice. Therefore, selective RXR modulators are a promising approach for developing improved therapies for type 2 diabetes, although additional studies are needed to understand the strain-specific effects on triglycerides.

[Analysis of factors effecting satisfaction of cochlear implants in prelingually deafened adolescents].

OBJECTIVE: By means of investigating how the benefit and the influence factors are for the pre-lingual deaf adolescents who received cochlear implantation (CI) METHODS: A questionnaire survey was conducted among 30 prelingually deaf adolescents-cochlear implant recipients, aged 9 approximately 20, 15 of which began to use hearing aid before the age of 6, and 19 of which had received speech testing after the CI, and their families to understand the degree of satisfaction, anticipation of the outcome. Nine of the 15 patients who began to use hearing aid before the age of 6 communicated by the oral communication/labial reading model (oral group), and 10 of the other 15 patients who had not used hearing aid after the age of 6 mainly communicated by sign language (sign group). RESULTS: The satisfaction score of the group with a hearing aid fitting history was 3.93 +/- 0.88, significantly higher than that of the group without a hearing aid fitting history (3.00 +/- 1.13, P < 0.05). The scores of speech production ability in vowel, consonant, single syllable, disyllable, trisyllable, and short sentence in the groups with and without a hearing aid fitting history were 76%/52%, 64%/45%, 71%/26%, 82%/96%, 68%/12%, and 49%/13% respectively (all P < 0.05), and there was no significant difference in the speech production ability in tone (P > 0.05) between these 2 groups. There were no significant differences in the total satisfaction degree, satisfaction degree in speech sound, and satisfaction degree in environmental sound between the oral group and sign group (all P > 0.05). The scores in vowel, consonant, single syllable, disyllable, trisyllable, short sentence, and tone of the oral and sign group were 74%, 60%, 64%, 61%, 57%, 43%, 27% and 43%, 39%, 26%, 26%, 20%, 6%, 5% respectively (all P > 0.01). The anticipation of the patient and the parents about outcome before implantation was negatively correlated with the satisfaction degree after implantation, (gamma = -0.479, P < 0.05). CONCLUSION: Cochlear implantation benefits greatly the deaf adolescent; however, it is necessary to establish a specific evaluation system according to recipients' needs.

Comparisons between neural response imaging thresholds, electrically evoked auditory reflex thresholds and most comfortable loudness levels in CII bionic ear users with HiResolution sound processing strategies.

CONCLUSIONS: The data collected in this study indicated that first Neural Response Imaging (NRI) thresholds had a better correlation with HiResolution most comfortable loudness (M) levels than tNRI thresholds. Electrically evoked auditory reflex thresholds (EARTs) had a higher correlation with HiResolution M levels than tNRI thresholds and a lower correlation than first NRI thresholds. NRI is a very useful method for programming the cochlear implants of young children who cannot demonstrate a reliable judgment of loudness. OBJECTIVE: To investigate how HiResolution sound processing, designed to deliver high-rate stimuli, relates to EARTs and electrically evoked compound action potential measurements produced by low-rate stimuli. MATERIAL AND METHODS: Nine profoundly hearing-impaired children and adults aged 6-29 years participated in the study. NRI responses were elicited using pulse trains consisting of biphasic pulses at a pulse width per phase of 32 micros delivered at a frequency of 30 Hz using SoundWave programming software. Stimuli were delivered to the odd electrodes (1, 3, 5, 7, 9, 11, 13 and 15) along the array. tNRI (NRI threshold) and first NRI thresholds were recorded for each stimulating electrode. "Speech bursts" stimuli used in EARTs recording were delivered to four electrodes at a time and stapedial reflexes were recorded from the impedance bridge. The M levels used were those used by each patient in their everyday HiResolution programs. RESULTS: For 8 patients (53 stimulating electrodes) the correlation between tNRI threshold and M level was r=0.675 (p=0.000) and that between first NRI thresholds and M level was r=0.741 (p=0.000). On average the M-level value was 20 CU (Current Unit) lower than the first NRI threshold value and 12 CU higher than the tNRI threshold value. The M-level patterns across the electrode array overall were similar to the tNRI or first NRI threshold patterns. For 7 patients (112 stimulating electrodes) the correlation between EART and M levels was r=0.710 (p=0.000). On average the EART value was 14 CU higher than the M-level value.

[Umbilical cord blood transplantation for patients with beta-thalassemia major].

OBJECTIVE: The beta-thalassemia major is a common hereditary hematology disease in southern China. The combination of blood transfusion and iron chelation is now the reference treatment. The allogeneic hematopoietic stem cell transplantation is the only curative therapy for beta-thalassemia major. In this study the investigators observed and evaluated the effects of umbilical cord blood transplantation (UCBT) for patients with beta-thalassemia major. METHODS: Twelve cases of beta-thalassemia major aged from 1.3 to 8.3 years (8 male and 4 female) received UCBT. Eleven of the twelve donors were siblings and one was unrelative. Eight patients received no antigen and four patients received two antigen disparate grafts. According to the Pesaro's classification for thalassemia, 10 patients were at grade I or II, and 2 were at grade III. The HLA-identical patients accepted the conditioning regimen consisting of busulfan, cyclophosphamide and antithymocyteglobulin. The HLA-mismatched patients accepted the conditioning regimen consisting of hypertransfusions, continuous iv desferrioxamine, hydroxyurea, fludarabine, busulfan, cyclophosphamide and antithymocyteglobulin. The harvest stem cells contained 3.63 - 16.0 x 10(7)/kg of nucleated cells, 0.11 - 1.03 x 10(6)/kg of CD(34)(+) cells and 0.17 - 1.18 x 10(5)/kg of colony-forming-unit-granulocyte macrophages. Cyclosporine alone or in combination with mycophenolate mofetil (MMF) was given for acute graft-versus-host disease (aGVHD) prophylaxis. RESULTS: Of the 12 patients, 10 were engrafted. Ten patients had neutrophil recovery (> 0.5 x 10(9)/L) and seven patients had platelet recovery (> 50 x 10(9)/L). The median time was 18.1 and 57.3 days, respectively. Seven patients had disease-free survival (DFS) at a median follow up of 23 months (range 4 - 63 months). Three patients had rejection and autologous hematopoitic reconstitution. Two patients were not engrafted. One patient acquired severe aplastic anemia, another patient died of severe infection. The incidences of grade I and grade II aGVHD were 60% (6/10) and 40% (4/10), respectively. There were no long-term complications in the disease free survivors. CONCLUSIONS: Grade I-II beta-thalassemia major patients receiving sibling UCBT had high DFS. UCBT is an effective way to treat beta-thalassemia major.

[Comparing the characteristics of psychophysical tests between cochlear implant children with large vestibular aqueduct syndrome and normal inner ear].

OBJECTIVE: The purpose of this study is to compare the characteristics of psychophysical tests between cochlear implant children with large vestibular aqueduct syndrome and normal inner ear and to analyse the characteristics of psychophysical and impedance tests for cochlear implant patients with large vestibular aqueduct syndrome. METHOD: One hundred and thirty-one cochlear implant pre-lingual deafness patients participated in this study. They received cochlear implant surgery at the age of 1.5 to 12 years old from Nov, 1998 to Aug, 2003. The patients were divided into two groups. Group A had 111 patients with normal inner ear. Group B had 20 patients with radiographically proven large vestibular aqueduct syndrome. All of the patients were inserted with Nucleus 24M. The time of the first programming session was approximately 4 weeks after surgery. The psychophysical tests included T and C-Level tests. The programming techniques used in the test were suitable for the age of the patients. The R116 and R126 software were used to test the impedance of the electrodes automatically. RESULT: Using the independent-samples t-test, the impedance and T/C-Level of each electrode did not differ significantly between group A and B ( P > 0.05). The impedance and T/C-Level were not significantly different between group A and B at the time of switch-on, 1 week, 2 weeks, 3 weeks, 2 months, 3 months, 6 months, 9 months and 1 year after switch-on (P > 0.05). CONCLUSION: The parameters of mapping were not significantly different between cochlear implant children with large vestibular aqueduct syndrome and normal inner ear. The management and procedure of mapping used in cochlear implant children with normal inner ear can be used in the children with large vestibular aqueduct syndrome.

[Follow-up study for newborns and infants who failed hearing screening].

OBJECTIVE: To study the audiological characteristics of newborns and infants who failed hearing screening. METHODS: One hundred and six infants failed hearing screening received follow-up study with routine audiological evaluations (auditory brainstem response, distortion product otoacoustic emission, tympanometry and visual reinforcement audiometry). RESULTS: Sixty-five infants (61.3%) of this group were normal hearing subjects and 39(36. 8% ) of the infants had hearing loss. Two cases (1.9%) received follow-up by phone. Fifteen cases (14.2%) with conductive hearing loss and 24 cases (22.6%) with sensorineural hearing loss. Thirteen (12.3%), 14 (13.2%), 6 (5.7%), and 6 (5.7%) cases were found to be mild, moderate, severe and profound hearing loss respectively. Diagnosis of hearing loss in the thirty-nine infants conducted a prevalence of 0.264% (39/14 785) of congenital hearing loss (both binaural and monaural). The hearing level of those cases with severe and profound hearing loss basically did not change, but that of cases with mild and moderate hearing loss changed. CONCLUSIONS: Early identification and intervention of infants with severe and profound hearing loss by 6 months of age were successful. Infants with mild and moderate hearing loss should be followed up to six or eight months and received routine audiologic evaluations.

[Off-pump coronary artery bypass grafting in patients over the age of seventy].

OBJECTIVE: To discuss the choice of surgical methods and the safety and efficacy of off-pump coronary artery bypass grafting (OPCAB) in elderly patients. METHODS: From Sept. 1997 to Feb. 2003, 63 cases over the age of seventy (including seventy) undertook OPCAB in our department. We compared the clinical data of those patients with that of 94 cases undertook conventional coronary artery bypass grafting (CABG) at the same age group and that of 58 cases younger than seventy received OPCAB. RESULTS: The clinical outcomes of OPCAB is better than that of CABG in elderly patients concerning inotropic drug, postoperative transfusion, re-operation, intubation time, complications incidence and in-hospital mortality. Furthermore, there is no significant difference of complication incidence and in-hospital mortality between the elderly OPCAB group and the younger OPCAB group. CONCLUSIONS: OPCAB is a safe and efficacious method of myocardial revascularization in the elderly.

Design and synthesis of alpha-aryloxy-alpha-methylhydrocinnamic acids: a novel class of dual peroxisome proliferator-activated receptor alpha/gamma agonists.

The design and synthesis of the dual peroxisome proliferator activated receptor (PPAR) alpha/gamma agonist (S)-2-methyl-3-[4-[2-(5-methyl-2-thiophen-2-yl-oxazol-4-yl)ethoxy]phenyl]-2-phenoxypropionic acid (2) for the treatment of type 2 diabetes and associated dyslipidemia are described. 2 possesses a potent dual hPPAR alpha/gamma agonist profile (IC(50) = 28 and 10 nM; EC(50) = 9 and 4 nM, respectively, for hPPARalpha and hPPARgamma). In preclinical models, 2 substantially improves insulin sensitivity and potently reverses diabetic hyperglycemia while significantly improving overall lipid homeostasis.

Morphology of auditory hair cells in guinea pig cochlea after transgene expression.

It is very important to determine if recombinant adenoviral vector (Ad) can damage the auditory hair cells in guinea pig cochlea after transgene expression. In this study, the scanning electron microscope was used to determine if there was loss of the auditory hair cells after Ad.LacZ (Ad5 containing Escherichia coli galactosidase) was inoculated into the cochlea through the round window membrane. Seven days later all inner and outer hair cells were found to express the LacZ gene. Except for the sparse loss of outer hair cells in the basal turn and the second turn, there was insignificant loss in the other turns. All inner hair cells were present. The damage to auditory hair cells resulting from intracochlear inoculation of Ad is limited, and this vector can be used as one of the ideal delivery tools in gene therapy of the cochlea.

[Multi-channel cochlear implants in patients with Mondini malformation].

OBJECTIVE: To describe clinical experiences with multi-channel cochlear implantation in patients with Mondini malformation. METHODS: Among 300 patients who received multi-channel cochlear implants from 1996 to 2002 in Beijing Tongren Hospital, 15 patients were diagnosed with Mondini malformation. A retrospective analysis was performed dealing with the surgical techniques, mapping and rehabilitations characteristics after surgery. 15 patients with normal cochlear structure are consider as control group. RESULTS: Gusher is found more common than the normal cochlear implantation, most of them are serious. The electrodes are inserted in the "cochleostomy" in full length of 13 Patients, 2 pairs of electrodes remains outside of "cochleostomy" in 2 patients. No serious complications occurred after implantation. All patients have auditory sensations. The impedance of the electrodes, the T level, C level and the hearing threshold are similar with the normal cochlear implantation group. The results have no significant difference in compare with normal cochlear group(P > 0.05). CONCLUSION: Multi-channel cochlear implantation could be performed safely in patients with Mondini malformation. The primary outcome for patients with Mondini malformation are similar to those with normal cochlear structure following the multi-channel cochlear implantation.

[The psychophysical tests in different age of patients with cochlear implants].

OBJECTIVE: The aim of this research is to analyse and compare the psychophysical tests in different age of patients with a cochlear implant. METHOD: One hundred and seventy patients with Nucleus CI24M cochlear implant system participated in this study. The patients were divided into four groups according to their age. They were group A (<3 year), B(3-<6 year) C(6-12 year) and D(>12 year). The psychophysical tests included T and C-Level evaluation. RESULT: C-Levels of each electrode (except electrode No. 20) were significantly different between group A and D at the time of one month after switch-on (P<0.01, P<0.05 for the No. 2, 18, 19, 21, 22 electrode). The normalized differences between used C-Levels and C-Levels for group D were significantly higher than the other three groups at the switch-on session (P<0.01, P<0.05 between group D and C). The normalized differences between used C-Levels and C-Levels were significantly different for the group A and D at one week after first programming session (P<0.05). CONCLUSION: T and C-Level for different age groups were different. The normalized differences between used C-Levels and C-Levels were significantly different among the four groups in the few weeks after switch-on.

[Immunogenicity analysis of HIV vaccine based on replicating vaccinia virus Tiantan vector].

OBJECTIVE: To investigate the immunogenicity of HIV vaccine vTKgpe based on Vaccinia Virus Tiantan vector in mice and rabbits. METHODS: Mice were inoculated with HIV vaccine vTKgpe by intramuscular (i.m.), intradermal (i.d.) or subcutaneous (s.c.) injections. Blood sample were collected every week, and then antibodies against HIV and vaccinia virus vector were detected. At week 4, some mice were killed and cellular immune responses were examined by flow cytometer. Additionally two rabbits were vaccinated subcutaneously, blood sample were tested as done with mice. RESULTS: In mice i.m. and s.c. groups, HIV specific antibodies emerged at week 2 and declined at week 4. Antibodies against vector elevated rapidly at week 4, and potentially affected HIV specific antibody detection. Cellular immune responses were only detected in s.c. group. Serum of rabbit showed that anti-HIV antibody appeared at week 2 and maintained for several weeks. CONCLUSION: Vaccine vTKgpe innoculated by i.m. and s.c routes inclined to induce humoral immune responses in mice, but in i.d. group, inclined to induce cellular immune responses. Response to the recombinant vaccinia virus was more sensitive in rabbit than in mice.

[Cochlear implantation in patients with inner ear malformations].

OBJECTIVE: To describe clinical experiences with multi-channel cochlear implantation in patients with bilateral inner ear malformations. METHODS: Among 410 patients who received multi-channel cochlear implantations from 1996 to 2004 in Beijing Tongren Hospital, 82 patients were diagnosed with inner ear malformations and implanted. A retrospective analysis was performed about the surgical characteristics and mapping characteristics after implantation. RESULTS: (1) All patients had auditory sensations. (2) Gusher was more common than the normal cochlear implantation. (3) The electrodes were inserted in the "cochleostomy" in full length of 80 Patients, but 2 pairs of electrodes remained outside of "cochleostomy" in 2 patients. (4) No serious complications occurred after implantation. (5) The impedance of the electrodes, the T level and C level were similar with the normal cochlear implantation. The results had no significant difference in compare with normal cochlear group (P > 0. 05). (6) The abilities of speech discrimination and spoken language were improved through rehabilitation. CONCLUSIONS: The cochlear implantation can be performed safely in inner ear malformations. The outcome of hearing rehabilitation for patients with inner ear malformations are similar to those children with normal cochlear structure followed the multi-channel cochlear implantation.
