Actinomycosis or tuberculosis? A diagnostic dilemma.

Actinomycosis of spine is a very rare disease. Very few cases have been studied and reported in the past. The dilemma of distinguishing the condition from other disorders relies on the competency of the treating physician and a proper knowledge of the subtle radiological differences between these disorders especially in underdeveloped and developing countries where tuberculosis still has a very strong foothold. A rare atypical case of actinomycosis of spine resembling tuberculosis is presented.

Limb length discrepancy following titanium elastic nailing in paediatric femoral shaft fractures.

The authors performed a prospective study to evaluate limb length discrepancy in children following titanium elastic nailing for femoral shaft fractures. Thirty-seven children (28 boys and 9 girls) were included in the study. The average time to radiological union in our study was 7.8 weeks (range: 5 to 14). Results after 3 years were evaluated for 29 children. Limb lengthening was noted in the first year in 15 children: at the time of nail removal, an average of 10.6 mm and at the end of one year 8.7 mm. After three years only nine were lengthened an average of 2.7 mm. An average of 12.6 mm shortening was seen in four patients at the time of nail removal, reduced to 12.1 mm at the end of one year. After three years three remained short, an average of 11.7 mm. No limb length discrepancy was seen in 10 patients. We conclude that limb length discrepancy is common following elastic nailing in paediatric femoral fractures, with lengthening being more frequent than shortening. Lengthening tends to decline with time at an average rate of around 1.5 mm per year.

Synthesis and antihepatotoxic activity of some new chalcones containing 1, 4 - dioxane ring system.

Silybum marianum is a medicinal plant used widely for treating liver diseases. The silymarin, a mixture of three flavolignan isomers namely silybin (1), silydianin (2), and silychristin (3) is an active constituent of the plant. However, silybin containing 1, 4-dioxane ring is the most potent antihepatotoxic agent. In contrast, other isomers do not possess 1, 4-dioxane ring, and thus do not exhibit a significant activity. We, therefore, thought that 1, 4-dioxane ring plays an important role in displaying antihepatotoxic activity, and have prepared some chalcones containing 1, 4-dioxane ring. The synthesized compounds were evaluated for antihepatotoxic activity against carbon tetrachloride induced hepatotoxicity in albino rats. The degree of protection was measured by using biochemical parameters like serum glutamic oxaloacetic transaminase (SGOT), serum glutamic pyruvate transaminase (SGPT), alkaline phosphatase (ALKP), total protein (TP) and total albumin (TA). The compounds namely 2-hydroxy-4-methoxy-3',4'-(2"- hydroxy methyl-1", 4"- dioxano) chalcone (9b) and 2-hydroxy- 4, 6-dimethoxy-3', 4'-(2"-hydroxy methyl-1", 4"- dioxano) chalcone (9c) exhibited a potent activity in comparison to standard drug silybon-70. The other compounds also showed moderate activity.

Cervical cord compression in an elderly patient with Hurler's syndrome: a case report.

STUDY DESIGN: A case of cervical cord compression in an elderly lady with Hurler's syndrome is reported. OBJECTIVE: To report the occurrence of cervical cord compression in a 56-year-old patient of Hurler's syndrome, making her the oldest patient with Hurler's. The case report also highlights the appropriate management that could be given to this group of difficult patients. SUMMARY OF BACKGROUND DATA: The literature on Hurler's syndrome is reviewed in terms of long-term survival and surgical management of spinal compression. No report in the literature describes cervical spine decompression in Hurler's syndrome in a patient this old. METHOD: Clinical and radiologic features of Hurler's induced cervical cord compression are described. Pathologic changes of Hurler's myleopathy are also reported. RESULTS: The patient underwent cervical spine decompression and tolerated the procedure well. The patient was able to mobilize free of the wheel chair and showed significant neurologic recovery. CONCLUSION: The case report highlights the success of surgery in Hurler's induced cervical cord compression. It also adds to the literature the oldest patient of Hurler's syndrome to undergo any form of spinal surgery.We present here a 59-year-old lady with Hurler's syndrome, who underwent a successful cervical spine decompression and stabilization for cervical myelopathy. At 59 years of age, she was the oldest case of Hurler's to undergo spinal surgery. The case highlights the importance of undertaking major spinal surgeries in these difficult inherited disorders with reasonable success, as against common belief.

Hepatoprotective activity of two plants belonging to the Apiaceae and the Euphorbiaceae family.

The different extracts of Apium graveolens Linn. (Apiaceae) and Croton oblongifolius Roxb. (Euphorbiaceae) were tested for their hepatoprotective activity against CCl(4) induced hepatotoxicity in albino rats. The degree of protection was measured by using biochemical parameters like serum transaminases (SGOT and SGPT), alkaline phosphatase, total protein and albumin. The methanolic extracts showed the most significant hepatoprotective activity comparable with standard drug silymarin. Other extracts namely petroleum ether and acetone also exhibited a potent activity.

Tuberculosis of the sternoclavicular joint.

A rare case of tuberculosis of the stemoclavicular joint in a 13-year-old girl is presented. The occurrence of tubercular infection in the sternoclavicular joint is extremely rare.
