[Osteopontin as a potential biomarker of metastasis and recurrence for hepatocellular carcinoma].

BACKGROUND & OBJECTIVE: Osteopontin (OPN), a secretory phosphorylated glycoprotein and integrin-binding protein, has been involved in tumorigenesis, progression, invasion, and metastasis. OPN is over-expressed in a variety of human tumors. This study was to evaluate the value of OPN detection in forecasting the postoperative metastasis and recurrence of hepatocellular carcinoma (HCC). METHODS: Plasma OPN level of 56 patients with HCC was detected before operation by enzyme-linked immunosorbent assay (ELISA), and plasma samples of 23 healthy volunteers were also analyzed as control group. The correlation of plasma OPN level to metastasis and recurrence of HCC was analyzed. RESULTS: Plasma OPN level was significantly higher in HCC group than in control group [(864.4+/-391.9) microg/L vs. (382.5+/-169.4) microg/L, P<0.001]. During the follow-up, 25 patients had tumor recurred. Plasma OPN level was significantly higher in the patients with early HCC recurrence than in those without recurrence [(961.0+/-411.6) microg/L vs. (744.6+/-323.9) microg/L, P=0.039]. Cox multivariate analysis showed that plasma OPN level, cirrhosis, and tumor size were independent predictors for HCC recurrence and metastasis. CONCLUSION: Plasma OPN level may be a potential diagnostic marker for predicting metastasis and recurrence of HCC.

[Prophylaxis and clinical treatment for surgical margin recurrence of small primary hepatocellular carcinoma].

BACKGROUND & OBJECTIVE: Along with the advance of medical technology and instruments, many patients with small primary hepatocellular carcinoma (HCC) had been found, who are mainly treated with surgical resection with high postoperative recurrence rate of over 60%. This study was to explore prophylaxis and clinical treatment for these patients to reduce postoperative recurrence rate. METHODS: Clinicopathologic data of 283 patients with small HCC, treated in Central Hospital of Kaiping and Cancer Center of Sun Yat-sen University from 1991 through May 2003, were analyzed. Over 85% patients suffered cirrhosis with liver reserve dysfunction, and were mainly treated with irregular hepatectomy. Of the 283 patients, 140 received application of anhydrous alcohol injection or alcohol-soaked gelatin sponge on surgical margin (test group), 143 received no application of alcohol on surgical margin (control group). The 2 groups were comparable in age, gender, tumor location, clinical stage, and Child-Pugh stage. RESULTS: The 1-year surgical margin recurrence rate was significantly higher in test group than in control group (21.4% vs. 4.4%, P0.05). Intrahepatic metastasis or recurrence rate, 5-year overall survival rate, and tumor-free survival rate were not different between the 2 groups. Both groups had no severe complications. CONCLUSIONS: Application of anhydrous alcohol on surgical margin or anhydrous alcohol injection in remnant liver after irregular hepatectomy may benefit to reduce postoperative recurrence in surgical margin for small PLC, but has no impact on intrahepatic metastases and recurrence in remnant liver.

Sleep patterns in college students: gender and grade differences.

OBJECTIVE: Since gender effect is inconsistent and grade effect has not been addressed in previous studies, we investigated both effects on the daily sleep patterns in a group of young college students. METHODS: The sample consisted of 237 students aged 18-24 years. Each subject completed a 7-day sleep log. RESULTS: Gender differences were found in several sleep variables and those were mostly not dependent on weekday/weekend difference. The female students went to bed and rose earlier and had longer sleep latency, more awakenings, and poorer sleep quality than the male. Gender differences were also shown in the relationship between sleep quality and other sleep variables. The correlation between sleep quality and rise time, time in bed, and sleep efficiency was stronger in men than in women. In contrast, grade differences were mostly dependent on weekday/weekend difference. The freshmen rose earlier and had shorter sleep time than did the other students on weekdays only. Sleep latency was the longest in seniors on weekdays only. CONCLUSION: This study showed that gender differences in sleep patterns and sleep difficulties were remarkable in the group of young college students. Alarmed by the high prevalence of sleep difficulties among general college students, it is recommended that the students should be informed of their sleep problems and the consequences.

Sleep education in college: a preliminary study.

In this study we evaluated the effect of a two-credit (100 min./week) "Sleep Management" course on the sleep patterns of college students as the course progressed over an 18-wk. semester. Curricular activity included lectures, group discussions, and practice of self-evaluation of sleep. Instead of giving the students the whole list of sleep hygiene at the outset of the course, each concept of sleep hygiene was introduced and discussed under related lecture topics. A total of 241 students (131 men and 110 women) took the course and kept 7-day sleep logs three times. Concurrently, sleep-log data were collected from 65 students (32 men and 33 women) who were not taking the course. Both groups showed similar varieties of academic backgrounds and characteristics of sleep patterns at the beginning. Similarly, their sleep patterns, namely, rise time, nighttime awakenings, time asleep, time in bed, sleep efficiency, and rise time regularity, changed over the semester. Women in both groups had more nighttime awakenings. In contrast, sleep quality was progressively better for the group in the course but not for the control group. Only women in the course decreased their nap time in the second and third months. Thus, the course of "Sleep Management" only had a mild and limited effect on sleep patterns. The course content needs refinement to maximize influence on students' sleep patterns and habits, particularly, on reduction of insufficient sleep and daytime sleepiness which are the highest ranking sleep problems among college students.

[Preliminary study on orthotopic liver transplantation for treatment of advanced hepatocellular carcinoma without extra-hepatic metastasis - a report of 10 cases].

BACKGROUND & OBJECTIVE: Most cases of hepatocellular carcinoma (HCC) are unresectable even without extra-hepatic metastasis. In addition, about 80%-90% patients in China are accompanied with cirrhosis. Liver transplantation is probably more proper for these patients than other treatments. This study was to explore the value of liver transplantation for treatment of advanced HCC without extra-hepatic metastasis, and to summarize the experience of liver transplantation performed in Cancer Center of Sun Yat-sen University. METHODS: Ten patients with advanced HCC without extra-hepatic metastasis who underwent liver transplantation in Sun Yat-sen University Cancer Center from Sept. 2003 to Apr. 2004 were followed up to May 2004, and their clinical records were reviewed retrospectively. RESULTS: The mortality within 1 month after operation was 0.0% for 10 cases of liver transplantation. One patient was alive for 7 month with tumor recurrence, and the remaining 9 patients were alive for 1-6 months without recurrence. Alpha fetoprotein (AFP) decreased to normal range within 2 months in 2 cases with macro tumor thrombus in the trunk of portal vein, and the left branch of portal vein, respectively. CONCLUSION: Advanced HCC without extra-hepatic metastasis, even with macro tumor thrombus in portal vein, is still indicated for liver transplantation.

[Chimeric T cell receptor N29gamma redirect T lymphocytes response specific to p185HER2 in A murine model of metastatic breast cancer].

BACKGROUND & OBJECTIVE: Chimeric T-cell receptors (chTCR) are recombinant immune receptors with the characteristics of combining exquisite antigen specificity of a monoclonal antibody and activating T lymphocyte function by signal transduction element. Compared to "classic" TCR in mediating T cells immune response to target cells,a significant potential advantage of chTCR is the lack of major histocompatibility complex (MHC) restriction and antigen processing. N29gamma is a chTCR specific for p185HER2. The current study is to investigate the efficacy of N29gamma redirect T cells in response to p185HER2 in a murine model of metastatic breast cancer. METHODS: Splenic T cells from Balb/c mice were purificated by negative selection over sterile brushed nylon wool fiber, activated by immobilized purified anti-mouse CD3 and CD28 antibodies. Then the recombinant retrovirus pRet6N29gamma were transduced by centrifuging (1 300 g for 90 min, at 32 Centigrade). The transduction efficiency was indirectly defermined by green fluorescence protein (GFP) expression of T cells in control group tested by flow cytometry. Mice bearing 3 days or 8 days MT901 or MT901/HER2 were randomized into experiment group or control group to receive IV infusions of polyclonal activated Balb/c splenic T cells transduced with the N29gamma retroviral vector or a control GFP vector(5 x 10(6)-40 x 10(6) of T cells per mouse). Each mouse was injected intraperitoneal with IL-2 of 3 x 10(4) IU every 12 h for 10 times. Mice were sacrificed at 11 days (Day 3 model) or 13 days (Day 8 model) after T cells transfer to enumerate the lung metastases. RESULTS: In Day 3 model, more than 200 lung metastases were present in mice in mock group, received non-transduced T cells group as well as GFP transduced T cells group. Treatment of mice bearing HER2 positive tumor with N29gamma transduced T cells resulted in a significant reduction in the number of lung metastases (3.4+/-3.3/lung). In contrast,N29gamma transduced T cells failed to induce the regression of HER2 negative MT901 pulmonary metastases (>200/lung). In Day 8 model, treatment with N29gamma modified T cells had a cell dose dependent effect on HER2+ lung metastases. Compared with 4 x 10(7) GFP T cells, 107 of N29gamma T cells were not enough to reduce the number of lung metastases (167+/-15.3) (P=0.198). With increasing dosage of N29gamma modified T cells from 2,3 to 4 x 10(7),there was a progressive decrease in the number of pulmonary metastases from 64+/-12.1,34+/-6.3,to 8+/-4.3,respectively (P< 0.01). CONCLUSIONS: Gene engineered T cells expressing chimeric TCR N29gamma induced the HER2-specific regression of lung metastases in breast cancer. Higher doses of gene-modified T cells were necessary to eliminate more advanced lung metastases.
