Efficacy of MR mammography (MRM) in providing preoperative locoregional information on breast cancer: correlation between MRM and histological findings.

RATIONALE AND OBJECTIVES: To assess the efficacy of MRM in providing preoperative locoregional information on patients with breast cancer. METHODS: MRI was performed on 62 female breast cancer patients. A 1.5T MR-system was used to acquire fat-suppressed T(1)WI, T(2)WI and dynamic-contrast-enhanced images with an SPGR pulse sequence. The extent of the cancer measured with MRM was confirmed histologically in all patients. RESULTS: The size obtained from MRM correlated well with the size obtained histologically, including intraductal spread of cancer (R: 0.853). As for shape, the round/oval type evident from MRM correlated more accurately (R: 0.934) than the ill-defined type associated with a linear and/or clumped enhanced area (R: 0.744). The difference between the size obtained from MRM and the size obtained histologically, including IDS, was less than 15 mm in the majority of patients (93.5%). CONCLUSIONS: MRM accurately reveals the extent of cancer, including IDS, and is effective at providing preoperative locoregional information for breast-conserving therapy for breast cancer.

Cancer-related intrusive thoughts as an indicator of poor psychological adjustment at 3 or more years after breast surgery: a preliminary study.

Intrusive thoughts are one of the re-experiencing symptoms in posttraumatic stress disorder, and have been suggested as a predictor for the continuous presence of psychological distress in cancer survivors. The aim of this preliminary study was to examine the possibility of using cancer-related intrusive thoughts (CITs) as an indicator of psychological distress and adjustment after breast surgery. A consecutive series of ambulatory breast cancer survivors at 3 or more years after surgery were given the Structured Clinical Interview for DSM-IV (SCID) and self-report questionnaires, including Profile Of Mood States (POMS), Impact of Event Scale (IES) and Mental Adjustment to Cancer (MAC) scale, to measure psychological distress and/or adjustment. The SCID identified a history of CITs in 34 (46%) of the 74 participants. No significant differences were found in the POMS and IES psychological distress scores between subjects with and without a history of CITs. Subjects with a history of CITs showed significantly higher levels of anxious preoccupation, one of the MAC subscale scores. The association continued to be significant after controlling for potential confounders such as social support, depression, avoidance, arousal, and neuroticism. The results indicated that CITs might be useful for indicating poor psychological adjustment, but not distress, in patients at 3 or more years after breast surgery.

Prognostic significance of fibrotic focus in invasive ductal carcinoma of the breast: a prospective observational study.

We have already reported invasive ductal carcinomas (IDCs) with fibrotic focus (FF) to be associated with significantly poorer survival than IDCs without FF. The purpose of this study was to prospectively investigate the effect of the presence of FF on the outcomes of 439 patients with IDCs to confirm the prognostic significance of FF, by the multivariate analysis, employing the Cox proportional hazard regression model, as compared with well-known clinicopathological parameters. We also precisely evaluated the prognostic significance of FF from the viewpoint of FF characteristics. The present study demonstrated that the presence of FF is a very useful parameter predicting tumor recurrence (TR), as well as initial distant organ metastasis (IDOM), in lymph node-negative IDCs (P =.024 and P =.026) and in IDCs positive for either or both estrogen receptor (ER) or progesterone receptor (PR) (P =.007 and P =.015), respectively. In addition, FF of >8 mm in diameter was found to be an independent prognostic parameter for TR and IDOM in lymph node-negative patients and patients with IDC positive for either or both ER or PR (P =.005 and P =.018). We conclude that the presence of FF is a very important histologic prognostic parameter for patients with IDCs of the breast.

Characteristics of tumors in lymph vessels play an important role in the tumor progression of invasive ductal carcinoma of the breast: a prospective study.

It is unknown whether the characteristics of tumor cells in lymph vessels play an important role in the tumor progression of invasive ductal carcinoma (IDC) of the breast. The purpose of this study was to investigate the significance of the characteristics of tumor cells in the lymph vessels in relation to the tumor progression in 393 IDC patients in comparison with well-known histological parameters. The dimensions of lymph vessel tumor emboli were measured, and their structural features, nuclear atypia, and numbers of mitotic and apoptotic figures were also assessed. Multiple regression analysis showed the dimension, the distance, the number of mitotic figures, the number of apoptotic figures, and papillary features of lymph vessel tumor emboli to be significantly associated with the increased number of cells invading the lymph vessels (P < .05). The Cox proportional hazard multivariate analyses showed that more than six apoptotic figures in lymph vessel tumor emboli significantly increased the hazard rates (HRs) of tumor recurrence and death in IDCs without nodal metastasis and that more than four mitotic figures in lymph vessel tumor emboli significantly increased the HRs of tumor recurrence and death in IDCs with nodal metastasis (P < .05). The present study showed that the histological characteristics of tumor cells in lymph vessels play a very important role in the tumor progression of IDCs.

Relationship between distressing cancer-related recollections and hippocampal volume in cancer survivors.

OBJECTIVE: Having cancer is extremely stressful, and distressing cancer-related recollections are frequently reported by cancer survivors. Smaller hippocampal volume has been observed in stress-related neuropsychiatric disorders, such as posttraumatic stress disorder (PTSD) and major depression. The aim of this study was to determine whether there is a similar association between distressing cancer-related recollections and hippocampal volume. METHOD: The subjects were 67 women who had had breast cancer surgery 3 or more years earlier and had no history of PTSD or major depression before the cancer. Each woman was evaluated with a semistructured interview to determine whether she had a history of distressing cancer-related recollections. Hippocampal volume was measured by three-dimensional magnetic resonance imaging, and memory function was assessed by the Wechsler Memory Scale-Revised. RESULTS: The volume of the left hippocampus was significantly smaller (5%) in the subjects with a history of distressing cancer-related recollections (N=28) than in those without any such history (N=39). There was no significant difference in right hippocampal volume or whole brain volume measured as a control. There were no significant differences in delayed memory or percentage retention. However, significantly worse immediate visual memory, but not verbal memory, was observed in the subjects with a history of distressing cancer-related recollections. CONCLUSIONS: Having distressing cancer-related recollections is associated with smaller left hippocampal volume in survivors of breast cancer.

[Sentinel node biopsy in breast cancer].

Sentinel node biopsy is a promising surgical technique to avoid unnecessary axillary lymph node dissection for breast cancer patients with histologically negative nodes. Several randomized phase III trials in Western countries are underway to assess sentinel node biopsy in cases of breast cancer in comparison with conventional axillary lymph node dissection. Other investigators have also started observational studies in cases when sentinel lymph nodes are proven histologically negative. In Japan, many issues regarding this minimally invasive surgery remain unresolved: optimal procedure, pathologic work-up, radiation exposure, health insurance coverage, and indication for sentinel node biopsy. The current status and problems of sentinel node biopsy in breast cancer are discussed herein.

Biomedical and psychosocial determinants of intrusive recollections in breast cancer survivors.

The authors' aim was to identify the determinants of intrusive recollections related to receiving a cancer diagnosis in women after cancer treatment. A consecutive series of breast cancer patients at 3-15 months after surgery, consisting of subjects with (N=63) and without (N=92) intrusive recollections was examined. Logistic regression analysis revealed that neuroticism, precancer intrusive recollections, and the number of cancer patients in relatives by marriage were final significant determinants, and receiving radiotherapy was an associated factor. These results suggest that biopsychosocial factors contribute to intrusive recollections and have clinical implications for the use of these characteristics in screening and early intervention.
