Severe acute respiratory syndrome epidemic and change of people's health behavior in China.

Severe Acute Respiratory Syndrome (SARS) has become a new worldwide epidemic whose origin was until recently unknown. It is the unpredictable nature of this epidemic that makes people want answers to some important questions about what they can do to protect themselves. This study presents an inquiry into people's knowledge and self-reported changes in behavior in response to the epidemic. Respondents were drawn from seven major occupational groups in the large central city of Wuhan. Although most respondents knew of SARS, there was still 8.4% who did not know about it. Knowledge was lowest among farmers who had come to the city for temporary work. Most respondents took action to avoid being infected by SARS, including, most commonly, efforts to improve indoor ventilation, to disinfect the indoor environment and to increase hand-washing frequency. Self-reported increases in hand-washing frequency were significant; however, among the seven occupational groups, reports of increased hand-washing were consistently greater among commercial service workers, students and farmers. While it seems that possible fears induced by the epidemic led to these changes, there are still about one-third of respondents who do not wash their hands as frequently as desired. There is also the challenge of devising strategies for maintaining the desired frequency of hand-washing among those who did change.

A retrospective study of 78 patients with severe acute respiratory syndrome.

OBJECTIVE: To summarize the clinical features of severe acute respiratory syndrome (SARS) and to discuss diagnosis and management of the disease. METHODS: A retrospective study was conducted on 78 cases of SARS referred to the Guangzhou Institute of Respiratory Diseases(GIRD) between December 22, 2002 and near the end of March 2003. Items reviewed cover all data concerning clinical manifestations, laboratory investigation and radiology. RESULTS: The patients on the study consisted of 42 males and 36 females, aged 20-75 yrs (mean age 37.5 +/- 11.6 yrs), including 44 affected health-care professionals. Clinical symptoms seen in the group were fever (100.0%), cough (88.5%), and dyspnea (79.5%). There were 12 cases (15.3%) with WBCs < 4.0 x 10(9) /L, 49 cases (62.8%) ranging between (4.0 -10.0) x 10(9) /L and 17 cases (21.8%) over 10.0 x 10(9) /L. The average was (7.58 +/- 4.96) x 10(9) /L, with 0.75 +/- 0.14 (neutrophil) and 0.18 +/- 0.11 (lymphocyte). Chest films and CT scanning revealed changes related to pneumonia. The transmission of the disease was likely via close contact with contagious droplets. The prevalences of acute lung injury (ALI, in 37 cases) and acute respiratory distress syndrome (ARDS, 21 out of 37 cases) were considerably high among the patients. Seven patients who developed ARDS complicated with multiple organs dysfunction syndrome (MODS) died. CONCLUSIONS: A history of close contact, fever, sign of pneumonia by X-ray and normal-to-lowered WBC counts are favorable for the diagnosis of SARS. Recognition of ALI as the important index for critical SARS and comprehensive supportive management are of paramount in decreasing the mortality of the disease.

Evaluation of the combination of multiple subpial transection and other techniques for treatment of intractable epilepsy.

OBJECTIVE: Multiple subpial transection (MST) is one approach to the surgical treatment of intractable epilepsy with epileptogenic lesion located in functional areas. To verify the effect of MST, an experimental study was performed first, followed by clinical application. METHODS: On the basis of the experimental study, MST was performed in 200 intractable epileptic patients from 1991 to 2000. Of them, 80 cases underwent MST only while 120 others underwent MST combined with other techniques, such as corpus callosotomy, temporal lobectomy and focus resection. A series of modifications of the surgical techniques were made. RESULTS: The results of the experimental study indicated that MST could inhibit the formation and spreading of epileptic discharge and limit the damage to neurons in a minimal area on the epileptogenic agent injected cortex. MST does not impair major functions of the cortex. After the clinical application and modifications, 160 patients were followed up for 1 to 8 years. Complete control of seizure was obtained in 100 cases (62.5%), significant reduction (more than 75%) in 32, reduction (more than 50%) in 20 and no change in 8. The total rate of effectiveness was 95.0%, and the significant rate of effectiveness was 82.5%. No functional defects were found in any patients. CONCLUSIONS: The results indicate that MST is an effective approach to the surgical treatment of intractable epilepsy. MST can be combined with other approaches. The outcome of the subdivision of the MST only group indicates that MST on local epileptogenic lesion without structural changes is as effective as that of the combined operation group. To evade hemispheric disturbance, MST should be done first to avoid severe complications. Hemispherectomy should be performed only on poor effected cases of MST.

[Computer-assisted planning for neurosurgery: a clinical study].

OBJECTIVE: To investigate the clinical effect of computer-assisted planning for neurosurgery (CAPN). METHODS: 100 groups of directional targets were randomly sampled. The targets' directional three-dimension coordinates measured with CAPN and those measured by traditional reading device were analyzed statistically. CAPN was actualized in 137 cases of stereotactic operation. RESULTS: The data measured with CAPN and those with traditional reading device were analyzed with two samples. The paired t test showed no statistically significantly difference between the data of targets measured by both methods (t < t(0.05/2,99)=1.984, P >< 0.05). Different kinds of operation based on CAPN were performed upon 137 cases success fully. CAPN was proved reliable. All operations were successful and the clinic effects satisfactory. CONCLUSION: CAPN is accurate and reliable, and has positive effect on the improvement of stereotactic neurosurgery.

[Neuropathy in nearby cranial nerves after acoustic schwannoma gamma knife radiosurgery, a follow-up study].

OBJECTIVE: To investigate the risks of facial, trigeminal and acoustic neuropathies after acoustic schwannoma gamma knife radiosurgery. METHODS: The clinical data of forty-three patients with 46 masses of acoustic schwannoma who underwent gamma knife radiosurgery with the dose of 12 approximately 15 Gy to the tumor margin between January 1997 and October 2000 and were followed up for 6 approximately 24 months (on average 16.9 months) were studied. The tumor diameter was 10 approximately 20 mm in 12 cases, 21 approximately 30 mm in 23 cases, >/= 31 mm in 11 cases, with the average value of 28 mm. RESULTS: The general tumor control rate was 91.3%. The useful hearing preservation rate was 100% immediately after radiosurgery, 87% 6 months later and 78% 2 years later. The hearing preservation rate was high for small tumors. The facial and trigeminal neuropathies began to appear after 6 months. The incidence rates of facial neuropathy was 15.3%, 7.6%, and 3.8% 6 months, 1 year and 2 years after radiosurgery respectively. The incidence rates of trigeminal neuropathy was 11.4%, 3.8%, and 3.8% respectively 6 months, 1 year, and 2 years after radiosurgery. The incidence of neuropathy was 3.8% for tumors with a diameter < 30 mm for both facial and trigeminal nerves. The hearing in 2 out of 15 cases with dysaudia began to improve 6 months after radiosurgery. The incidence of neuropathy for tumors with the diameter > 30 mm was 3.8% for both nerves 2 years after raadiosurgery. The preservation rate of useful hearing for tumors with the diameter < 20 mm was 100% after radiosurgery. CONCLUSION: Stereotactic radiosurgery using gamma knife with a dose of 12 approximately 15 Gy to the tumor margin succeeds in controlling acoustic schwannoma and preserving useful hearing. The incidence of facial and trigeminal neuropathies are low. The neuropathy caused by gamma knife radiosurgery is sub-lethal and can be recovered gradually.

[Stereotactic biopsy for intracranial deep lesions].

OBJECTIVE: To investigate the histological diagnostic efficacy and accuracy of stereotactic biopsy for intracranial deep lesions under computerized tomography (CT) and magnetic resonance imaging (MRI) guidance. METHODS: CT or MRI-guided stereotactic biopsy was performed in 605 cases with intracranial deep lesions. All the procedures were done under local anesthesia with Leksell stereotactic system. CT or MRI film was used to determine the coordinate in 450 cases and the computer-assisted planning for neurosurgery (CAPN) in 155 cases. RESULTS: Brain tumors, inflammatory lesions, and abnormal but nonspecific lesions were diagnosed pathologically in 537 cases (88.75%), 30 cases (4.96%), and 13 cases (2.15%) respectively. In 20 cases (3.31%) the biopsy procedure failed to provide useful pathological evidence. The total positive rate of biopsy was 96.69%. Complications associated with biopsy occurred in 18 patients (2.98%), and 2 patients (0.33%) died following the procedure. CONCLUSION: Stereotactic biopsy using MRI-guided stereotactic approach or CT-guided technique is a safe and reliable method for histological diagnosis of intracranial deep lesions.
