Experimental study of an adjustable-length prosthesis in a temporal bone model.

CONCLUSIONS: This prosthesis has the advantage of rapid adjustment at the time of insertion in order to achieve optimal tension and, as a result, optimal sound transmission. OBJECTIVE: To test the acoustic performance of a new, adjustable incus replacement prosthesis in a human temporal bone model. MATERIAL AND METHODS: Experiments were performed in seven human temporal bones, before and after removal of the incus and insertion of the prosthesis. The input comprised 406 pure tones ranging in frequency between 0.1 and 10 kHz at an intensity of 80 dB SPL at the tympanic membrane. The output measurement was stapes footplate displacement, determined by means of a laser Doppler vibrometer. Three lengths of the prosthesis were investigated: optimal, optimal +0.2 mm and optimal +0.4 mm. RESULTS: The optimal-length prosthesis produced similar results to those of an intact middle ear. The slightly longer prostheses decreased middle ear sound transmission at all test frequencies, except those near 1.5 kHz.

Vestibulotomy above a severely displaced facial nerve.

CONCLUSIONS: Vestibulotomy above a severely displaced facial nerve represents a new surgical approach to achieve serviceable hearing. The lack of facial nerve injury and the potential for hearing restoration make this procedure feasible in patients who would otherwise be marginal or poor surgical candidates. OBJECTIVE: To investigate the feasibility of vestibulotomy above a severely displaced facial nerve. MATERIAL AND METHODS: Eight patients with severe congenital conductive hearing loss underwent vestibulotomy above a severely displaced facial nerve and hearing reconstruction between January 2000 and January 2002. All patients had congenital middle ear deformities. The facial nerves overhung and concealed the oval window niche or lay inferior to the oval window. The facial nerves were transposed in order to reach the oval window niche in four of eight cases. RESULTS: Hearing gain was 15 dB in 2 ears, 16-25 dB in 3 and > 26 dB in 3. There were no cases of postoperative facial paresis.

[Surgical experience by 95 malformed stampedes and otosclerosis].

OBJECTIVE: To investigate the effect of stapes surgery for conductive hearing loss with intact tympanic membrane and eustachian tube. METHOD: Stapes surgery data of 95 conductive hearing loss patients, which with intact tympanic membrane and eustachian tube, was retrospective analyzed,and the surgical effect of otosclerosis, middle ear deformities and external canal stenosis was summarized. RESULT: Postoperative (4 weeks after the surgery) pure tone threshold test demonstrated that the effect of Vestibulotomy was better than incus-footplate connection (incus and footplate is connected by a cortical bone ) ( P <0.05). The surgery to the patients with external canal stenosis was better than those with otosclerosis or middle ear deformities ( P <0.05). There was no statistic significance between physiological stapedectomy and Vestibulotomy. The follow-up of 1-3 years showed that the pure tone threshold of 8 out of 14 cases reverted to the preoperative level, while the hearing improvement in the other cases still remained. CONCLUSION: Stapes surgery to the patients with stapes-oval window deformities is of significance compared with those with otosclerosis.

[Korner's septum and congenital aural atresia].

OBJECTIVE: To discuss the anatomic characters and clinical importance of aural atresia coexisted with Korner's septum. METHOD: Fifty patients with aural atresia coexisted with Korner's septum were analyzed retrospectively, in which the reconstruction surgery was performed, and high resolution temporal bone CT scan in the coronal and axial plane were taken preoperatively. RESULT: Among 50 patients, 39 had well-developed Korner's septum. The others (11/50) had incomplete Korner's septum, which were porotic continued with antrums, under powerful microscopic inspection. The ossicular reconstruction was performed successfully in 41 patients, while that of the other 9 had to suspend because of displacement such as low tegmen and backward processus condylaris of mandible. CONCLUSION: The high resolution temporal bone CT is indispensable to the operation of aural atresia coexisted with Korner's septum.

[Computerized tomographic evaluation of traumatic ossicles disruption].

OBJECTIVE: To investigate the importance of the high resolution computerized tomography of temporal bone in evaluation of traumatic lesions, particularly of the ossicular disruption. METHOD: Ten patients with traumatic ossicles dislocation underwent preoperative CT scanning followed by surgical exploration of the middle ear. RESULT: The radiographic finding was consistent with the operative demonstration for malleoincudal disarticulation (3 ears), incudostapedial dislocation (7 ears), and the fracture of temporal bone (8 ears). CONCLUSION: The incus was the most vulnerable ossicle in the trauma to the middle ear. CT scans can diagnose exactly the damage position in middle ear.
