[Redistribution of three alpha1-adrenergic receptor subtypes in the stably transfected HEK 293A cells upon agonist stimulation.].

To investigate the subcellular distribution of three alpha(1)-adrenergic receptor subtypes and their internalization and trafficking upon agonist stimulation in human embryonic kidney (HEK) 293A cell line, saturation radioligand binding assay, laser confocal imaging, and Western blot were applied to examine the distribution and changes in localization of three alpha(1)-AR subtypes in transfected HEK 293A cells prior to and after treatment with phenylephrine. The results are as follows: (1) The transfection efficiency was over 90%and was equal among three alpha(1)-AR subtypes. alpha(1B ) -AR expression in cell membrane was the highest, and alpha(1D ) -AR was the lowest, as determined by (125)I-BE2254 binding assay, however, K(d)s were not significantly different among the three receptor subtypes. (2) Without agonist stimulation, alpha(1A ) -AR was detected not only on the cell surface but also in the cytosol, alpha(1B ) -AR was predominantly located on the cell surface, whereas alpha(1D ) -AR was mostly detected in the cytosol. (3) After 1 h of stimulation with phenylephrine, as observed using confocal microscope, less alpha(1A)- and alpha(1B ) -AR were detected on the cell surface but more in the cytosol. The change was more remarkable in alpha(1B)-AR than that in alpha(1A)-AR, whereas no change of distribution was detected in alpha(1D)-AR in response to phenylephrine. However, when examined by Western blot, no change in distribution was detected in alpha(1A)- and alpha(1D)-AR, only alpha(1B)-AR showed the same change as that shown in confocal imaging. It is suggested that the characteristics of localization and changes of distribution are different among three alpha(1)-AR subtypes in HEK293A cells upon phenylephrine stimulation.

Psychometric evaluation of the Chinese version of the Pittsburgh Sleep Quality Index (CPSQI) in primary insomnia and control subjects.

In order to effectively study the population experiencing insomnia, it is important to identify reliable and valid tools to measure sleep that can be administered in the home setting. The purpose of this study was to assess psychometric properties for the Chinese version of the Pittsburgh Sleep Quality Index (CPSQI) in community-dwelling adults with primary insomnia. The CPSQI had an overall reliability coefficient of 0.82 -0.83 for all subjects. "Subjective sleep quality" was the component most highly correlated with the global score. Overall, the CPSQI showed acceptable test-retest reliability over a 14- to 21-day interval with a coefficient of 0.85 for all subjects and 0.77 for primary insomniacs. The two contrasting groups had significantly different global and component scores. A CPSQI of greater than 5 yielded a sensitivity and specificity of 98 and 55% in primary insomniacs vs. controls. A CPSQI of greater than 6 resulted in a sensitivity and specificity of 90 and 67%. Results suggest that the CPSQI is a psychometrically sound measure of sleep quality and disturbance for patients with primary insomnia. It may not be an effective screening tool because of its low specificity, but it can be a sensitive, reliable, and valid outcome assessment tool for use in community-based studies of primary insomnia.

Sheehan's syndrome presenting with early postpartum congestive heart failure.

We report the case of a young woman with Sheehan's syndrome who presented with ventricular arrhythmia and congestive heart failure. The patient was admitted because of postpartum hemorrhage and hypovolemic shock; a massive blood transfusion was required to restore blood volume. After initial stabilization, the patient developed acute respiratory distress and congestive heart failure accompanied by hemodynamic instability 2 weeks after delivery. Episodes of ventricular tachycardia of the torsade de pointes type and a prolonged QT interval were noted on baseline electrocardiogram. A low cortisol level was found incidentally, which led to the suspicion of hypopituitarism. The diagnosis was later supported by laboratory findings of multiple pituitary hormone deficiencies. After administration of corticosteroids and thyroxine, the patient's clinical condition improved dramatically. A pituitary magnetic resonance imaging scan 32 days after delivery revealed a diminished and flattened pituitary gland with prominent intrasellar cerebrospinal fluid loculation, which was compatible with the clinical diagnosis of empty sella with panhypopituitarism. The syndrome of acute anterior pituitary necrosis secondary to postpartum hemorrhage and shock was first described by Sheehan in 1939. Although the occurrence of Sheehan's syndrome is now rare, it should still be considered in any woman with a history of peripartum hemorrhage who develops manifestations of pituitary hormone deficiency. Appropriate hormone replacement therapy is essential and always results in dramatic clinical improvement.

Prevalence and determinants of prehypertension status in the Taiwanese general population.

OBJECTIVE: The purpose of this study was to determine the prevalence rate of prehypertension and to compare the differences in demographic factors, anthropometric measurements, lifestyles and metabolic profiles between normotensive and prehypertensive individuals. DESIGN: Data were based on the Nutrition and Health Survey in Taiwan (NAHSIT), a cross-sectional survey that adopted a multi-stage, stratified clustering sampling scheme. The data collection period was from 1 January 1993 to 31 December 1996. METHODS: This study was a secondary analysis of data previously collected. Data from 1039 men and 1186 women, aged 18-96 years, were analyzed. RESULTS: Approximately 34% of Taiwanese adults had prehypertension. The prevalence rate of prehypertension was higher among men (36%) than women (32%). In univariable logistic analysis, sex, age group, age, waist-hip ratio (WHR) group, body mass index (BMI) group, waist circumference, blood sugar, triglyceride, high-density lipoprotein cholesterol (HDL), low-density lipoprotein cholesterol (LDL), cholesterol/HDL and smoking status were significantly associated with prehypertension status. Multivariable logistic regression revealed that age [odds ratio (OR) = 1.014, 95% confidence interval (CI) = 1.004-1.0249] and BMI (OR = 1.106, 95% CI = 1.051-1.165) were the determinants of prehypertension status in men. For women, age (OR = 1.033, 95% CI = 1.022-1.044), waist circumference (OR = 1.031, 95% CI = 1.012-1.051) and triglyceride (OR = 1.003, 95% CI = 1.000-1.005) were the determinants of prehypertension. CONCLUSIONS: This study underscores the importance of general obesity and central obesity as risk factors for prehypertension in the Taiwanese adult population. These two indices of obesity have different impacts on men and women.

Cognitive behavioural therapy for primary insomnia: a systematic review.

AIM: This paper reports a systematic review of seven studies evaluating the efficacy of cognitive behavioural therapy (CBT) for persistent primary insomnia. BACKGROUND: Insomnia is one of the most common health complaints reported in the primary care setting. Although non-pharmacological treatments such as the CBT have been suggested to be useful in combating the persistent insomnia, the efficacy and clinical utility of CBT for primary insomnia have yet to be determined. METHOD: A systematic search of Ovid, MEDLINE, psychINFO, PsycARTICLES, CINAHL, and EMBASE databases of papers published between 1993 and 2004 was conducted, using the following medical subject headings or key words: insomnia, primary insomnia, psychophysiological insomnia, sleep maintenance disorders, sleep initiation disorders, non-pharmacological treatment, and cognitive behavioural therapy. A total of seven papers was included in the review. FINDINGS: Stimulus control, sleep restriction, sleep hygiene education and cognitive restructuring were the main treatment components. Interventions were provided by psychiatrists except for one study, in which the CBT was delivered by nurses. Among beneficial outcomes, improvement of sleep efficacy, sleep onset latency and wake after sleep onset were the most frequently reported. In addition, participants significantly reduced sleep medication use. Some studies gave follow-up data which indicated that the CBT produced durable clinical changes in total sleep time and night-time wakefulness. CONCLUSIONS: These randomized controlled trial studies demonstrated that CBT was superior to any single-component treatment such as stimulus control, relaxation training, educational programmes, or other control conditions. However, heterogeneity in patient assessment, CBT protocols, and outcome indicators made determination of the relative efficacy and clinical utility of the therapy difficult. Therefore, the standard components of CBT need to be clearly defined. In addition, a comprehensive assessment of patients is essential for future studies.

[Significance of ceramide from precancerous lesion to carcinoma of larynx].

OBJECTIVE: To investigate the expression of ceramide produced by sphingomyelin in normal laryngeal mucosa, laryngeal precancerous lesion and laryngeal carcinoma. METHODS: One hundred and seventy-eight consecutive patients with leukoplakia larynx were identified from the archived pathology files of Eye Ear Nose and Throat Hospital of Fudan University from 1991 to 2001. Among them, 31 patients developed laryngeal carcinoma. Flow cytometry (FCM) and immunohistochemistry were performed to test DNA content and ceramide expression on normal tissue, precancerous lesions and laryngeal carcinoma RESULTS: Among thirty-one patients with laryngeal carcinoma, thirty-one cases are all aneuploids, diploids in all normal laryngeal mucosa and three diploids, twenty-eight aneuploids in precancerous lesions. Expression of ceramide decreased gradually from normal tissue, precancerous lesions to laryngeal carcinoma Cell staining per high-power field: (400 +/- 30, 180 +/- 20, 10 +/- 10), t test: P < 0.01. The expression of ceramide in DNA diploid cell (400 +/- 20) is more than that in aneuploid cell (150 +/- 10), t test: P < 0.01. CONCLUSIONS: Ceramide, the second messenger in apoptosis, plays a significant role from precancerous lesion to carcinoma of larynx. Reduction of ceramide may be the key factor contribution to laryngeal carcinogenesis.

Study on ceramide expression and DNA content in patients with healthy mucosa, leukoplakia, and carcinoma of the larynx.

OBJECTIVE: To investigate the expression of ceramide produced by sphingomyelin and DNA content in patients with healthy laryngeal mucosa, leukoplakia, and laryngeal carcinoma. DESIGN: A retrospective review of the clinical and surgical records of 178 patients with leukoplakia of the larynx; 23 of them developed laryngeal carcinoma. SETTING: University medical center. PATIENTS: One hundred seventy-eight consecutive patients with leukoplakia of the larynx were identified from the archived pathology files of the Eye Ear Nose and Throat Hospital of Fudan University from January 1, 1990, to December 30, 2001. Among them, 23 developed laryngeal carcinoma. Flow cytometry and immunohistochemistry were performed to test DNA content and ceramide expression in healthy tissue, tissue with leukoplakia, and tissue with laryngeal carcinoma from the same patient. RESULTS: Among 23 patients with leukoplakia, 20 had aneuploidy and 3 had diploidy. The healthy tissues were all diploids, and the tissues with laryngeal carcinoma were all aneuploids. The expression of ceramide decreased gradually from healthy tissue to tissue with leukoplakia to tissue with laryngeal carcinoma (0, no staining; 1+, weak staining; 2+, mild staining; 3+, moderate staining; 4+, strong staining; and 5+, the highest staining intensity). The expression of ceramide in DNA diploid cells is stronger than in aneuploid cells. CONCLUSIONS: Ceramide, the second messenger in apoptosis, may associate with the progress of leukoplakia to carcinoma of the larynx. The reduction of ceramide may contribute to laryngeal carcinogenesis.

Symptom distress changes during first postoperative month in newly diagnosed Taiwanese breast cancer patients: a longitudinal study.

The purpose of this longitudinal study was to explore changes in symptom distress in newly diagnosed Taiwanese breast cancer patients during the initial 4-week postoperative period. The research instruments, including a demographic questionnaire and the Symptom Distress Scale, were used to obtain data on postoperative day 2 and at weeks 2, 3, and 4. In total, 39 patients with a mean age of 48 years participated in this study. Data were analyzed using descriptive statistics, t tests, one-way ANOVA, and repeated-measures ANOVA. Results revealed that the level of symptom distress significantly decreased from postoperative day 2 to week 4. Loss of appetite and a poor outlook increased; nausea frequency, fatigue, and insomnia decreased then increased; and frequency and the level of pain, coughing, tightness/tenderness in the chest wall, weakness, and numbness in the arm of the operative side all decreased over the 4-week study period. Age, stage of disease, and type of surgery were all related to symptom distress. Results of this study may provide reassurances about what can be expected after breast cancer surgery.
