Ginkgo nut intoxication in a 2-year-old male.

This report describes a case of ginkgo nut intoxication in a 2-year-old male. The patient presented with vomiting and afebrile convulsion 4 hours after eating a large number of roasted gingko nuts. There was a large volume of ginkgo nuts in his vomited matter, and on admission the concentrations of 4-O-methoxypyridoxine in his serum and urine were elevated. The patient was diagnosed as having ginkgo nut intoxication, and diazepam and pyridoxal phosphate were administered intravenously. After the treatment, his symptoms were resolved. The neurotoxicity of ginkgo nuts should be recognized by pediatricians and parents who have infants.

Congenital cystic adenomatoid malformation complicated by esophageal duplication cyst in a 6-month-old girl.

We report a case of congenital cystic adenomatoid malformation (CCAM) complicated by an esophageal duplication cyst in a 6-month-old girl. The patient presented with recurrent pneumonia. Magnetic resonance imaging revealed two cystic lesions in the upper and lower lobes of the right lung. After cystectomy, histopathological investigation revealed that the lower cyst was a CCAM Type I, and the upper cyst was an esophageal duplication cyst. The coexistence of these complex anomalies supports the concept that the esophageal duplication cyst is one entity of a broad spectrum of developmental abnormalities caused by abnormal budding of the primitive foregut.

Cholangiojejunal fistula caused by bile duct stricture after intraoperative injury to the common hepatic artery.

A 68-year-old man, admitted for the treatment of recurrent cholangitis after a pancreatoduodenectomy (PD) performed 3 years previously was diagnosed as having multiple hepaticolithiasis. On laparotomy, the hepatic artery was not recognized. The anastomosed common hepatic duct was obstructed, and a fistula had been formed between the right hepatic duct and the Roux limb of the jejunum. Lithotripsy was performed from this fistula and it was reanastomosed. Angiography was performed postoperatively and it revealed common hepatic artery injury, most likely to have occurred during the previous PD. The patient's postoperative course was uneventful and he has been asymptomatic for 8 months after the operation, indicating that reanastomosis of the fistula can be an effective method. The stricture of the anastomosis was suspected to be mainly due to cholangial ischemia, because no episode of anastomotic leak or retrograde biliary infection had occurred during the PD perioperative period. There are several reports of late stricture of anastomosis 5 or more years after cholangiojejunostomy. This patient, therefore, requires further long-term follow up.

Theophylline inhibits NF-kappaB activation in human peripheral blood mononuclear cells.

BACKGROUND: Theophylline not only dilates the bronchi, but also modulates the production of proinflammatory cytokines and inhibits inflammation. Theophylline exerts an antiinflammatory effect on allergic inflammation through inhibition of NF-kappaB activation in mast cells. However, the action of theophylline on monocytes/macrophages and T cells is unknown. METHODS: We examined whether or not theophylline inhibits tumor necrosis factor (TNF)-alpha-induced activation of the nuclear transcription factor NF-kappaB, a factor that is essential for the expression of proinflammatory cytokines, in human monocytic U-937 cells, a T cell line (Jurkat) and peripheral blood mononuclear cells (PBMC). The inhibitory effect of theophylline on TNF-alpha-induced NF-kappaB activation was evaluated by Western blotting, flow cytometry and chloramphenicol acetyltransferase (CAT) assaying. Expression of the IkappaBalpha protein was evaluated by Western blotting. RESULTS: Western blotting demonstrated that theophylline inhibits NF-kappaB activation in U-937 and Jurkat cells and PBMC. Flow cytometry demonstrated that theophylline inhibits NF-kappaB activation in U-937 and Jurkat cells in a dose-related manner. CAT assaying indicated that NF-kappaB-dependent reporter gene expression is inhibited in U-937 cells pretreated with theophylline. Western blotting of cytoplasmic extracts of U-937 cells revealed that this inhibition was linked to theophylline-induced preservation of expression of the IkappaBalpha protein. CONCLUSIONS: These findings are consistent with the idea that theophylline suppresses the production of proinflammatory cytokines via inhibition of NF-kappaB activation through preservation of the IkappaBalpha protein in monocytes/macrophages and T cells.
