Efficacy and safety of gatifloxacin for urinary tract infection in nonspecialized Korean urologic practice.

To investigate the efficacy and safety of gatifloxacin (400mg/day) on chronic prostatitis or cystitis, 453 patients with prostatitis (NIH category II or IIIa) (N=149, mean 45.8+/-13.3 years) and cystitis (N=304, mean 53.8+/-14.3 years) were enrolled. Total NIH CPSI score and symptom score for cystitis decreased from 20.3 to 9.9 (response rate 86.7%, 95% CI 80.2-93.2%) and from 6.2 to 1.8 (response rate 83.2%, 95% CI 78.7-87.7%), respectively. In the overall clinical efficacy, 71.2% and 88.4% of the patients with prostatitis and cystitis were responders, respectively. Of the patients, 15.7% reported insignificant adverse events. These results suggest that gatifloxacin was well tolerated and improved the clinical outcomes in patients with chronic prostatitis or cystitis.

Primary large cell neuroendocrine carcinoma of the urinary bladder.

Primary large cell neuroendocrine carcinomas (LCNEC) of the urinary bladder are rare. Reported herein is a case of a primary, pure LCNEC occurring in a man. The patient was a 32-year-old man who presented with hematuria of 1 week's duration. On cystoscopic examination, a solitary mass measuring 3 cm in diameter was detected protruding from the anterosuperior wall of the urinary bladder. Two months after the primary transurethral resection, significant regrowth of the remnant mass was noted on CT, and the patient underwent a partial cystectomy. A diagnosis of LCNEC was made based upon histological and immunohistochemical findings. Tumor cells were positive for synaptophysin, chromogranin A, CD56, epithelial membrane antigen, and cytokeratin. Histologically, the tumor penetrated the deep muscle and perivesical fat. In spite of three cycles of chemotherapy, the patient developed multiple metastases in the lung and liver 10 months postoperatively. LCNEC of the urinary bladder are uncommon entities, which have a possible fatal outcome.

Eosinophilic cystitis causing spontaneous rupture of the urinary bladder in a child.

We report a unique case of eosinophilic cystitis causing intraperitoneal bladder perforation in a child diagnosed by chance with no signs or history of trauma. To our knowledge, this is the first case of eosinophilic cystitis complicated by bladder rupture in children. The patient was successfully treated with primary repair. For children with non-traumatic bladder perforation, eosinophilic cystitis must be considered in the differential diagnosis.

Post-transplant lymphocele: an unusual cause of acute urinary retention mimicking urethral injury.

Retroperitoneal pelvic lymphoceles are one of the most common complications following renal transplantation, and usually present with a palpable mass, ipsilateral leg edema, hydronephrosis caused by ureteral obstruction, decreased renal function and cutaneous lymphatic fistula. However, lymphocele rarely causes acute urinary retention. In this study, we describe a case of a patient who developed acute urinary retention after renal transplantation mimicking urethral injury. When a transplanted patient demonstrates the inability to void, one should consider bladder outlet obstruction resulting from lymphocele as a possible cause.

Intrascrotal extratesticular arteriovenous malformation.

We describe a case of intrascrotal-extratesticular arteriovenous malformation in an adult that presented as a hard scrotal mass mimicking post-traumatic hematoma. This is the first reported case of an arteriovenous malformation at the intrascrotal-extratesticular region.

Penile strangulation treated with the modified string method.

Penile strangulation by a nonmetallic or thin metallic ring is easily overcome by severing the object, but a heavy metal ring causing penile strangulation is very difficult to sever. Here we report 2 cases of penile strangulation by metal rings, involving a 38-year-old man and a 44-year-old man, both of whom were treated with the modified string method and glandular puncture.

Diabetes induced alteration of clitoral hemodynamics and structure in the rabbit.

PURPOSE: We investigated the effect of diabetes on clitoral hemodynamics and structures in the rabbit. MATERIALS AND METHODS: A total of 25 New Zealand White female rabbits weighing 3 to 3.5 kg. were divided into 2 groups, including 5 in the control and 20 in the experimental group. Experimental animals received intravenous injection of alloxan hydrochloride (Sigma Chemical Co., St. Louis, Missouri) (100 mg./kg.). The development of diabetes was verified by measuring body weight and blood glucose levels. After 12 weeks clitoral cavernous blood flow in ml. per minute per 100 gm. tissue was measured with a laser Doppler flowmeter. Cross sections of the clitoris were used for histochemistry and histomorphometric image analysis. RESULTS: After 12 weeks 5 animals were included in the diabetes group. Mean baseline flaccid and peak clitoral cavernous blood flow plus or minus standard deviation significantly decreased in the diabetic group compared with the control group (3.9 +/- 1.6 and 5.8 +/- 2.2 versus 7.2 +/- 2.5 and 12.9 +/- 5.8 ml. per minute per 100 gm. tissue, respectively, p <0.05). Histology revealed diffuse clitoral fibrosis in the diabetic group. On histomorphometry the mean proportion of clitoral cavernous smooth muscle in the diabetic group was significantly decreased compared with the control group (51.9% +/- 4.9% versus 62.3% +/- 3.1%, p < 0.05). CONCLUSIONS: These results show that diabetes mellitus produces significant adverse effects on the hemodynamic mechanism of clitoral engorgement and leads to diffuse clitoral cavernous fibrosis. It implies that decreased sexual arousal in diabetic women may result from structural changes in the clitoris.
