[L-arginine in reperfusion solution as a heart protecting factor].

UNLABELLED: Ischemia-reperfusion damage causes injury to both cardiomyocytes and endothelial cells. Loss of endothelium function and degeneration of cardiomyocytes can significantly deteriorate postoperative efficiency of cardiac muscle and in this way can threaten patients' life. The goal of the study was examination how addition of L-arginine to the reperfusion solution influences development of ultrastructural exponents of cardiac muscle injury during reperfusion. MATERIAL AND METHODS: The study was conducted with the use of isolated rat's hearts (Wistar strain), undergoing 20-minute introductory perfusion, 30-minute cardioplegic pause, and then reperfusion conducted on non-working and working heart model, with (study group) and without (control group) the use of L-arginine. RESULTS: Microscopic exponents of injury in cardiomyocytes were most pronounced in the control group. After addition of L-arginine to the reperfusion solution, the decrease in the number of construction points, reduction in overhydration of intra- and extracellular spaces, with diminishing of endothelium vessels' pinocytic activity and decrease in the degree of sarcolemma plication were observed in the study group in comparison with the control group. Increasing of L-arginine doses enhanced slightly its protective activity on the ultrastructure of cardiac muscle. CONCLUSION: Addition of L-arginine to the reperfusion fluid protects cardiomyocytes and endothelium vessels. These positive changes are not recorded in all cardiac muscle specimens.

[The influence of L-arginine on secretion of nitric oxide and haemodynamic function of isolated rat's heart subjected to cold cardioplegic ischemia and reperfusion].

BACKGROUND: L-arginine is a precursor of nitric oxide (NO), which is formed endogenously as a result of reaction catalyzed by synthase NO (NOS). Administration of L-arginine to cardioplegic solution St. Thomas Hospital N 2 or used during reperfusion of heart can facilitate the return of left ventricle function after hypothermic ischaemia through increase of release of nitric oxide and preservation of endothelial cells functions. Positive effects of these researches could have extremely beneficial application in clinical practice, improving results of cardiosurgical operations conducted in extracorporeal circulation. OBJECTIVE: How addition of L-arginine to cardioplegic solution or during reperfusion influences secretion of nitric oxide, coronary flow and haemodynamic function of heart. METHODS: The research was conducted on isolated heart model of rats (Wistar inbreeding). The research was carried out in order: 20-minutes initial perfusion, 30-minutes cold cardioplegic arrest with or without addition of L-arginine and reperfusion with Krebs-Henseleit's solution with or without addition of L-arginine. RESULTS: Addition of L-arginine in the preischemic period and during reperfusion stimulates increase of nitric oxide secretion, what positively correlates with increasing of coronary flow. Addition of L-arginine to cardioplegic solution significantly increases coronary flow and cardiac output what improves postischaemic haemodynamic function of heart.

Effect of L-arginine on overhydration and ultrastructure preservation of rat's heart exposed to cold cardioplegic ischaemia.

UNLABELLED: ISCHAEMIA: Reperfusion damage causes injury both cardiomyocytes and endothelial cells. Loss of endothelium function and heart cells degeneration can be essential for deterioration of postoperative efficiency of heart and in this way can threaten patients' life. OBJECTIVE: How addition of L-arginine to cardioplegic solution influences development of ultrastructural degeneration indications of rat's heart exposed to cold cardiolegic ischemia. METHODS: The research was conducted on isolated heart model of 56 rats, divided into seven equal groups. The hearts were prepared with modified Neely's method and were perfused with the use of apparatus in accordance with modified Langedorf's method. The research was carried out in order: initial perfusion of the non-working and working heart, perfusion with cardioplegic solution, cold cardioplegic arrest with or without addition of L-arginine. RESULTS: In the microscopic preparations of rat's heart in the control group (P) numerous focal performing contraction nodes were found. Intumescent vessels' endothelium included a large number of pinocytal vesicles. In the ultrastructural examination of group DI heart preparations there were less contraction nodes than in control group. The widening of endoplasmatic reticulum's channels and system T was essentially smaller comparing with group P. Less widening of perivascular zone and less number and size of pinocytal vesicles were also observed. In group 2 further decreasing of contraction nodes number was revealed. The structure of capillary was preserved and number of pinocytal vesicles was still decreasing. Further standardization of ultrastructural picture of cardiomyocytes and perivascular zone was found in group 3. CONCLUSION: In control group (P) microscopic degeneration indications of rat's heart were the most intensified. Addition of L-arginine to cardioplegic solution influenced decreasing of intensification of heart degeneration indications. Increasing of L-arginine doses added to cardioplegic solution positive correlates with protective effect of L-arginine on heart ultrastructure.

[Population genetics of the D1S80 system in a Central Poland region].

Population data concerning the D1S80 system based on the analysis of 775 persons from Central Poland region was presented. Accordance with the Hardy and Weinberg equilibrium (HWE) was proved using two different statistical approaches. The parameters for the appropriateness of this marker for investigations in the analysed population was also assessed: heterozygosity--0.79, PIC--0.77, PD--0.93, PE--0.62, PI--12.8 and frequency of mutation events--0.27%.

[Sobriety testing in case of lack of cooperation of the examined person--the legal basis of the physician's activities].

The legal aspect of blood sample taking for alcohol concentration from suspicious, uncooperative persons is described in this paper. The Polish law allows for a possibility to take blood samples for alcohol concentration from suspects only by a physician or another skilled medical worker and only when consent is given to the medical procedure. The traffic act makes an exception for cases when the suspects are drivers and the suspicion concerns driving under the influence of alcohol. Taking blood samples is the medical worker' obligation in accordance with the law. The physician can refuse to do this only when the procedure threatens the life or health of the suspect.

[New Polish regulations regarding accidents in the workplace].

With the new year 2003 the new Polish "Law on Social Security Insurance in case of Workplace Accidents and Occupational Illness" came into force, which annulled the previous "Law on Benefits in case of Workplace Accidents and Occupational Illness", dating from 1975, together with its executive provisions, which contained as an Appendix a table for determining the percentage of health loss in a given case. This article, comparing the previous and current laws, analyzes their influence on the process of delivering certificates in legal medicine. In the area of direct interest to the legal medicine practitioner there is practically no difference caused by the introduction of the new law, but because of the lack of executive provisions to the new law, the former directive concerning the principles and mode of issuance of loss-of-health certification, including the health loss percentage table, will continue to be applied.

[Iatrogenic injury of the trachea--a case report].

The case of iatrogenic trachea injury during endotracheal intubation is reported in this article. The case concerns a 36 years old woman, with diagnosed DIC resulting from postpartum complications. Injury of the brachiocephalic trunk and the tracheoarterial fistula were found during autopsy. The authors considered the time and the mechanism of both injuries.

[Effect of ethylene glycol on antioxidative enzymes and lipid peroxidation activity in erythrocytes].

Investigations were aimed at demonstrating changes in the activity of enzymes forming an antioxidative barrier and oxidative stress parameters in erythrocytes of rats which were administrated ethylene glycol during 4 weeks. Superoxide dismutase activity (CuZn-SOD) was evaluated by the Misra and Fridovich method, catalase (EC1.11.1.6.) by the Beers and Sizer method and malonyl dialdehyde concentration (MDA) with Placer et all were assessed in the obtained material through the evaluation of TBARS compounds concentration. In the 4th week of the experiment a decrease in catalase (9.3 U/gHb to 5.7 U/gHb) and superoxide dismutase (2378 U/gHb to 1759 U/gHb) activity was observed. Malonyl dialdehyde concentration increased from the initial 0.14 mumol/gHb to 0.24 mumol/gHb. The investigations carried out have demonstrated that long-term intoxication with ethylene glycol leads to a constant generation of free radicals (increase of MDA concentration) and gradual exhaustion of the antioxidative system.

[Analysis of selected changes in project the penal code].

In this paper the authors have analysed selected proposals of changes in the project of amendments in the penal code. Special attention has been placed on problem of the legality of the "comma" in art. 156 of the penal code. In this matter also a review of court jurisdiction has been made.

[Evolution of Supreme Court settlements relating to DNA evidence in paternity investigations].

The evolution of Supreme Court settlements relating to DNA evidence in paternity investigations was presented in this paper. It was found that its opinions have essentially changed during the last years as follows: 1) superiority of DNA evidence over other proof was perceived by the possibility of positive paternity confirmation, 2) attention has been turned to advisability of statistical analysis and its possible positive influence on emotional ties between parents and child, 3) opinion has been expressed about crossing of borders of free judicial estimations in cases of questioning DNA expertise, 4) attention has been turned to the possibility of utilisation of other tissues than blood or indirect genotyping.

[Myocardial infarction as a work accident in Polish Supreme Court decisions. Part I. Excessive stress at work].

This paper deals with Polish Supreme Court opinions on the acceptance of myocardial infarction as work accident. From them the principle emerges that excessive physical or mental work stress can be accepted as the external cause of myocardial infarction in an employee, whereby the excess strain should be judged in each case individually and with reference to the individual situation.

[Myocardial infarction as a work accident in Polish Supreme Court decisions. Part II. Job contraindications due to an employee's state of health].

This paper presents the second part on opinions given by the Polish Supreme Court on the acceptance of myocardial infarction as work accident. This it can be seen that, in certain cases, the judgements of the Court are contradictory. For in some cases, an external cause is recognized in the act of permitting employees to work whose state of health would contraindicate the performance of their duties, while in others, the principle is accepted that work duties not considered harmful for healthy people cannot be deemed an external cause for people with health problems. An external cause according to the Supreme Court may also be the act of allowing an employee to work without a valid medical examination, or even by not conducting routine required work-related medical examinations on time.

[Allele distribution of the ten STR loci in Central Poland population].

Allele frequency data for ten STR loci: D3S1358, vWA, D16S539, D2S1338, D8S1179, D21S11, D18S51, D19S433, TH01, FGA were estimated in a sample of 350 unrelated individuals in Central Poland population (Lodz region). The accordance with the Hardy and Weinberg equilibrium (HWE) for all investigated loci was proved. The allele distributions were compared to the similar data for other populations.

[Selected alcohols on the pro- and anti-oxidative processes in rat erythrocytes].

In forensic medicine practice poisonings are rather frequent, and among them, those caused by fatal "substitution" of ethyl alcohol. One of the most frequently encountered "substitutes" for ethyl alcohol is methanol. The purpose of our research was to determine the concentration of malonic dialdehyde as the expression of lipid peroxidation and antioxidant enzyme activity after dosed chronic ethyl and methyl alcohol intoxication. The experiment was conducted on approx. 6 month-old male inbred Lewis rats each weighing approx. 250 g. Ethanol and methanol solution was given in the concentration 1.0 M. The control group of rats received water. Each experimental group numbered 30 rats, this number was divided into three sub-groups, which were put-down at 4, 8 and 12 weeks. The activity of superoxide dismutase (CuZu-SOD) was determined by the Misra-Fridovich method, catalase (CAT) by the Beers-Sizer method. The concentration of malonic dialdehyde (MDA) was determined using the method of Placer et al. by assessing the concentration of TBARS compounds. Results are expressed as a mean +/- SD. The paired Student's test for small groups were used. Superoxide dismutase SOD1 activity decreased compared with the control group throughout the duration of the experiment from 2212 U/gHb to 1676 U/gHb for ethanol and from 2212 U/gHb to 945 U/gHb for methanol. Catalase activity for methanol decreased from 9.1 U/gHb to 5.1 U/gHb, for ethanol to 7.4 U/gHb. In the 4th week of the experiment increase of malonyl dialdehyde concentration for methanol group was observed--from 0.14 umol/gHb to 0.34 umol/Hb; after 8th weeks it decreased to 0.2 umol/gHb and in the 12th week increased to 0.23 umol/gHb. For ethanol these changes was less visible and reached the level of 0.24 umol/l. The statistical processing of the results was performed on the basis of parametric tests (the t-Student test for small experiments) and computer software Statistica. The statistical significance was set for p<0.05.

[Long-term realistically life-threatening disease].

In this paper the authors discuss a legal code description of a wrongly included (as a result of a legislative error) "long-term really life-threatening disease" (dlugotrwala choroba realnie zagrazajaca zyciu) in the Criminal Code. This category of disease impossible to apply in practice since its terms "long-term" and "realistically life-threatening" are mutually exclusive--is nonetheless applicable to crimes committed from Sept. 1, 1998 to Dec. 8, 2003. In effect this causes a change in the qualification of certain acts in Art. 156 of the Criminal Code, to include those in Art. 157, Paragraph 1, and in some cases, even extending to acts named in Art. 157, Paragraph 2 of the Criminal Code.

[Malpractice in internal medicine in the case material of the Forensic Medicine Department, Medical University of Lódź, Poland].

Legal opinions in the area of internal medicine, opinionated by the Forensic Medicine Department, Medical University of Lódź, Poland in 2002, were selected for further analysis and assessment in the course of medical treatment. During this period opinions were given in 20 cases, most of which concerned cardiovascular, secondly respiratory, followed by, endocrine diseases. In half of the cases analyzed it was determined that medical treatment had been incorrect. All of the objectionable diagnostic and therapeutic procedures evidenced some form of decisional error. In most cases errors were made in the diagnosis, as a result of neglect in performing necessary examinations, or of erroneous interpretation of results obtained.

[STR and RFLP polymorphism in paternity determination based on family investigation].

This paper shows a paternity examination of a deceased putative father carried out on the basis of his family investigation. Statistical evaluation proved the defendant's paternity using STR and RFLP analyses with the probability of more than 99,999% (PI > 100 000). The restriction analysis has proven considerably more informative than the amplification analysis, which means seven RFLP markers correspond with the similar paternity indices of the fifteen STR markers.

[Changes of the concentration of lipid peroxidation products and thiol groups in rat brains with long-term intoxication with methanol].

The aim of this paper was to define the influence of long-term methanol intoxication on the changes of end products of lipid peroxidation, reacting with thiobarbituric acid (TBA-reactive products) and free SH-group concentration in rat brains. The experiment was conducted on male Lewis rats. The experimental groups received a 1-molar methanol solution and the control group received tap water. The animals were killed after 4, 8 and 12 weeks of intoxication and their brains were collected for further examination, which encompassed measurement of the concentration of TBA-reactive products and free SH-groups. The revealed changes of TBA-reactive products and free SH-group concentration were not statistically significant.
