We report a case of perioperative management of a toddler with plastic bronchitis complicated by tracheal obstruction. We discuss our management of this case as well as the diverse group of patients who may present with this disease. We also reviewed the literature regarding medical management of cast bronchitis.
BACKGROUND: The aim of this study was to determine the success of fasciotomy of lower limb compartments with elevated intra-compartment pressure. METHODS: The present report is a retrospective cohort study at No. 3 Combat Support Hospital involving a survey and case note review of patients who had undergone fasciotomy for compartment syndrome at least 6 months earlier (December 1997-December 2000). RESULTS: Fourteen patients met the inclusion criteria. Eleven reported success with complete resolution of symptoms and return to military fitness levels. Two reported no change and significant residual symptoms. One reported worse symptoms and one required re-operation with a successful result. CONCLUSION: Surgical decompression is the accepted treatment of compartment syndrome as proven by pressure manometry, after failure of conservative treatment.
The senior house officer grade contains the largest group of junior doctors in the United Kingdom. For many years there has been concern over their poor training, long hours of work, poor supervision, little or no feedback (often given inappropriately), and their levels of psychological distress. Evaluation of senior house officer posts has been cumbersome and inconsistent. This paper describes the origins, design, piloting, testing for reliability and subsequent use of a simple 10-point evaluation form for senior house officers in the West Midlands. The process has now been computerized, and has been incorporated into the postgraduate dean's database of all junior doctor posts in the West Midlands. The evaluation of all 1500 senior house officer posts can now be performed every 6 months. Detailed results can be obtained and fed back to clinical tutors, hospitals, and Royal Colleges. The uses of such data in improving the lot of the senior house officer are discussed.
