Prevalence of Brucella antibodies in rural and suburban communities in three provinces of Turkey: need for improved diagnosis and prevention.

OBJECTIVE: To determine the seroprevalence of Brucella-specific antibodies in rural and suburban communities in different provinces of Anatolia. METHOD: Cross-sectional seroepidemiological study on serum samples collected in communities in two relatively developed provinces in west Anatolia with an official low prevalence of brucellosis and in one province in southeast Anatolia with a high prevalence. RESULT: The seroprevalence of brucellosis in the two provinces in the west Anatolia appears to be high and ranged from 2.9 to 8.5% in Rose Bengal test and from 0 to 5.6% in Wright serum agglutination test at a titer equal or higher than 1:100. The seroprevalence in communities in the province in southeast Anatolia was lower and this might well be attributed to vaccination of livestock in the year preceding the survey. CONCLUSION: Adherence to traditional farming practices and lifestyle, and a preference for fresh dairy contribute to the high seroprevalence of brucellosis. Vaccination of livestock is of utmost importance and the consumption of fresh milk and dairy products prepared from unpasteurised milk should be halted. Better access to laboratory testing is needed for the confirmation and management of brucellosis.

Non-heart-beating donors: is it worthwhile?

OBJECTIVE: Due to organ shortage in renal transplantation, many transplant centers attempt to increase the donor pool. Non-heart-beating donors seem to be a promising alternative. PATIENTS AND METHODS: We performed 14 renal transplantations from 8 non-heart-beating donors. 2, 1, 3 and 2 donors were from groups 1, 2, 3 and 4, respectively, according to Maastricht classification. RESULTS: In 6 of the patients' warm ischemia time was over 30 minutes. Three of them had primary non-function. In 6 patients delayed graft function was seen. The remaining 5 kidneys functioned immediately. Two patients whose kidney functioned returned to hemodialysis in the I I and 13th months after their transplantations. One of the patients with primary non-function died. 9 kidneys function well in their follow-up period between 5-111 months. 1 and 5-year graft survival rates were 69.8 and 61.1 percent, respectively. The mean graft survival time is 69.9 +/- 14.5 months. CONCLUSION: Despite the high primary non-function rate, we think that non-heart-beating donors especially in Maastricht classification 3 and 4 should be used due to dramatic shortage of organs.

Prevalence of latex sensitization in healthy blood donors in Izmir, Turkey.

Although latex is a common allergen, the prevalence of natural rubber latex allergy in the general population in Turkey is unknown. The aim of this study was to assess the prevalence of sensitization to natural rubber latex in a population of healthy blood donors. As skin prick testing of a sample of the general population was not feasible, the subgroup of adult healthy blood donors was chosen instead. Skin testing with commercially available reagents (Stallergenes) was performed on 952 volunteers attending the Blood Centre of the Social Security Hospital in Izmir, Turkey in 2001. The study population completed a questionnaire involving history of personal allergy and symptoms. In 20 blood donors (2.1%) the latex skin prick test (SPT) was positive. No relationship by age or gender was found. There was no relationship between previous allergic symptoms and SPT positivitiy to latex except for the systemic reaction related to any allergen (deep wheezing or any difficulty breathing, nausea, vomiting, palpitations, faintness or edema). The prevalence of SPT positivity to natural rubber latex was 2.1% in a Turkish blood donor group, which can be related to the latex sensitivity within the general Turkish population.
