Role of Korean red ginseng total saponins in rat infertility induced by polycystic ovaries.

OBJECTIVE: To investigate the effect of Korean red ginseng total saponins (GTS) on ovarian morphology and nerve growth factor (NGF) expression in the ovaries, pituitary, and hippocampus. DESIGN: Polycystic ovary (PCO) rat model induced by estradiol valerate (EV). SETTING: University research laboratory. PATIENT(S): Thirty sexually mature female Sprague-Dawley rats weighing 190-210 g. INTERVENTION(S): Female Sprague-Dawley rats (190-210 g) were separated into three groups: EV control (n = 10), EV plus GTS (n = 10), and oil control (n = 10). MAIN OUTCOME MEASURE(S): Ovarian morphology and NGF protein expression. RESULT(S): Polycystic ovary was fully developed in rats with a single intramuscular injection of EV. Increased expression of NGF was noted in the ovaries and the brain of rats with PCO. GTS administration attenuated NGF expression in the ovaries but not in the brain. CONCLUSION(S): Our findings suggest a role for GTS in the regulation of NGF expression in female rats with PCO.

Does hypercapnic acidosis, induced by adding CO2 to inspired gas, have protective effect in a ventilator-induced lung injury?

To investigate whether hypercapnic acidosis, induced by adding CO2 to inspired gas, would be protective effect against ventilator-induced lung injury (VILI), we ventilated 55 normal white rabbits for 6 hr or until PaO2/FIO2 <200 mmHg. Control group (n=15) was ventilated with peak inspiratory pressure (PIP) of 15 cm H2O, positive end-expiratory pressure (PEEP) of 3 cm H2O, an inspiration-to-expiration ratio of 1:2, and an inspired oxygen fraction (FIO2) of 0.40. High pressure hypercapnic group (HPHC; n=20) was ventilated with PIP of 30 cm H2O, PEEP of 0 cm H2O, and FIO2 of 0.40. Carbon dioxide was introduced into the inspiratory limb of the ventilator circuit, as necessary to maintain hypercapnia (PaCO2, 65 to 75 mmHg). High pressure normocapnic group (HPNC; n=20) was ventilated with same setting of HPHC, except normocapnia (PaCO2, 35 to 45 mmHg). Bronchoalveolar lavage fluid (BALF) lactate dehydrogenase, aspartate aminotransferase, interleukin-8 were significantly higher in high pressure ventilator group than control group (p<0.05). Wet weight to dry weight (WW/DW) and histologic scores were significantly higher in high pressure ventilator group than control group (p<0.05). However, there were no significant differences in oxygenation, BALF inflammatory markers, WW/DW and histologic scores between HPHC and HPNC groups. These findings suggest that hypercapnic acidosis at least induced by CO2 insufflation would not be protective effect against VILI in this model.

Asian pear pectin administration during presensitization inhibits allergic response to ovalbumin in BALB/c mice.

OBJECTIVE: A type of respiratory disorder resembling some aspects of human allergic asthma can be induced in mice using ovalbumin. The factors that influence the etiology of asthma are poorly understood even though cytokines are known to play a pivotal role. The purpose of this study was to test the hypothesis whether an administration of Asian pear pectin during presensitization could suppress allergic response to ovalbumin in BALB/c mice. DESIGN: High-dose (100 microg) of pectin-sol was used and values were compared to those from the control. Ovalbumin and aluminum hydroxide were utilized for sensitization while ovalbumin aerosol was used for provocation 2 weeks later. The bronchoalveolar lavage (BAL) and assessment of tracheal smooth muscle responsiveness to electrical field stimulation or acetylcholine were performed 1 day after ovalbumin provocation. Two main cytokines of interferon (IFN)-gamma and interleukin (IL)-5, and serum immunoglobulin E (IgE) were assayed. SETTINGS: Laboratory of the Chosun University Medical School SUBJECT: Male BALB/c mice RESULTS: Antigen dose of 5 microg for sensitization generated TH1 type cytokines in the lungs with a high level of IFN-gamma and a low level of IL-5. In contrast, TH2 type cytokines were produced in splenocytes including a high level of IL-5 and a low level of IFN-gamma. Asian pear pectin-sol administration during presensitization significantly inhibited (p < 0.05) sensitivity of airway smooth muscle to electrical field stimulation and acetylcholine. Further, IFN-gamma production significantly decreased (p < 0.05) in BAL fluids while it significantly increased (p < 0.05) in splenic cells. On the other hand, IL-5 production significantly increased (p < 0.05) in BAL fluids while it was a significant decrease (p < 0.05) in splenic cells. For the histopathologic changes in the lung, pear pectin-sol recovered ovalbumin (OVA)-induced abnormal signs to an almost normal state. As a correlate, IgE production significantly decreased (p < 0.05) in pectin-sol-treated animals compared to the control. CONCLUSIONS: It is possible from these data that BALB/c mice have different susceptibilities to different doses of OVA regulated by pulmonary TH1 and TH2 type cytokines, independent of splenic TH1 and TH2 type cytokines production. These results also indicate that administration of Asian pear pectin-sol in presensitized mice suppresses allergic asthmatic reaction.

The role of the lactate dehydrogenase and the effect of prone position during ventilator-induced lung injury.

To examine the impact of lactate dehydrogenase (LDH) as an early marker of ventilator-induced lung injury (VILI) and the effect of prone position during the VILI, we ventilated 28 normal white rabbits (10 supine, 10 prone, 8 controls) for 6 hr or until PaO2/FIO2 ratio was<200 mmHg. We applied an identical injurious ventilatory pattern (peak inspiratory pressure of 35 cmH2O with a PEEP of 3 cmH2O, I:E ratio of 1:2, and FIO2 of 0.40) in the supine and prone group. VILI was assessed by oxygenation, gravimetric analysis and histologic grading. Serum levels of LDH progressively increased significantly during the VILI (supine and prone groups) as compared with controls. There was a significant negative correlation between oxygenation and LDH levels (r=-0.619, p<0.001). Wet weight/dry weight ratios (WW/DW) and histologic scores for dependent regions were significantly higher in the supine than the prone group. There were no differences in WW/DW and histologic scores for nondependent regions between the supine and prone group. These findings suggest that serum LDH levels might be an early marker of severity of lung injury. The prone position resulted in a less severe and more homogenous distribution of VILI.

Electro-acupuncture reverses nerve growth factor abundance in experimental polycystic ovaries in the rat.

Polycystic ovary syndrome (PCOS) remains one of the most common causes of anovulation in women of reproductive age. There is some evidence that nerve growth factor (NGF) is involved in the pathogenesis of PCOS. Therefore, seeking the pathogenesis of PCOS is important for controlling fertility. In traditional Oriental Medicine, acupuncture has been used for the function of ovaries. The present study was designed to determine whether electro-acupuncture (EA) could affect experimentally induced polycystic ovary (PCO) in the rat. The two acupoints Sp-6 and E-128 were stimulated to test for efficacy in the protein expression of NGF. Polycystic ovaries were induced by a single injection of estradiol valerate (4 mg i.m.). During the experimental period of 8 weeks, some of the rats were treated with EA twice weekly; this group was compared with a vehicle-treated control group and an estradiol-injected group not subjected to EA. At day 60, the protein expression of NGF was examined by immunohistochemistry in the ovaries, the adrenal glands and some parts of the brain. The estradiol treatment induced a clear PCO appearance, and was associated with a robust increase in NGF expression in the ovaries, the adrenal glands and the brain. EA treatment partly reversed the NGF abundance, particularly in the ovaries, but not in the brain. Our data show that EA affects the NGF involvement in ovarian dysfunction.

Intrarenal epidermal cyst.

An epidermal cyst is a very unusual cause of a renal mass, with only three cases reported in English literature. The authors report a case of this in a 51-year-old man with left flank pain and hematuria. A 5 x 4 cm-sized cystic mass, having an irregular margin with stippled calcification, was identified in the lower pole of the left kidney. A retroperitoneal laparoscopic simple nephrectomy was performed under the assumption that it was a renal tumor. Histologically, the tumor was identical in appearance to an epidermal cyst within the skin. Aberrant ectodermal implantation during embryogenesis has been postulated as a histogenesis of intrarenal epidermal cyst. However, the present case had a history of renal stone, which was treated with extracorporeal shock wave lithotripsy. Therefore, the authors suspected the pathogenetic mechanism of this lesion was a metaplasia of traumatic origin. The diagnosis of an epidermal cyst was not made preoperatively in the present case, or in the cases described in the literature. This diagnosis should be included in the differentiation of calcified intrarenal masses.

The effect of herbal medicine on nerve growth factor in estradiol valerate-induced polycystic ovaries in rats.

A type of polycystic ovary resembling some aspects of human polycystic ovarian syndrome (PCOS) can be induced in the rat with a single injection of long-acting estradiol valerate. Among several theories behind the development of polycystic ovaries (PCO), the involvement of the sympathetic nervous system draws much attention, and herbal medicine is known to relieve the abnormal symptoms of PCO. Two herbal formulas, Changbudodam-Tang (cang fu dao tan tang) and Yongdamsagan-Tang (long dan xie gan tang), were used in the present study. The administration of herbal medicine was done every other day for 60 days. The morphological changes of ovaries from herbal medicine treatment were compared to those from an oil-treated control group and an estradiol valerate-injected group. This study also examined the possible hypothesis of neurogenic participation in terms of nerve growth factor (NGF) in the pathology of ovarian dysfunction. The nerve growth factor was analyzed in the central nervous system and ovaries by immunohistochemistry. The main findings of the present study were: (1) PCO were fully developed in rats with a single intramuscular injection of estradiol valerate, (2) PCO resulted in the expression of NGF in the ovaries and the brain tissues, and (3) herbal medicine administration significantly decreased the elevated NGF staining in the ovaries without affecting the brain tissues significantly.
