Altered findings of hepatic arteriography after radiofrequency ablation of hepatocellular carcinoma: comparison of pre-ablation and post-ablation angiograms.

OBJECTIVE: To evaluate the altered findings of hepatic arteriography after radiofrequency (RF) ablation of hepatocellular carcinoma which can potentially influence subsequent transcatheter arterial chemoembolization. MATERIALS AND METHODS: Hepatic arteriograms of 26 index hepatocellular carcinomas in 24 patients treated only by RF ablation (M:F = 22:2, mean age 55 years), in which hepatic arteriography was performed before and after RF ablation, were retrospectively compared for the altered findings. RESULTS: The altered findings of hepatic arteriography after RF ablation of the hepatocellular carcinoma were arterio-portal shunt (n = 3), periablational enhancement (n = 5), varied caliber of the feeding artery to the index tumor (n = 12) among which eight decreased, and occlusion of an adjacent arterial branch (n = 8). Residual unablated or locally progressed tumor was not detected in post-RF ablation arteriography (n = 5) due to the arterio-portal shunt (n = 2) or the periablational enhancement (n = 3). The possibility of not detecting the residual unablated or locally progressed tumor was higher within 24 weeks after RF ablation (Mann-Whitney test, P = 0.041). CONCLUSION: The findings of hepatic arteriography are altered after RF ablation, and the altered findings may increase the difficulty in performing super-selective transcatheter arterial chemoembolization due to undetected tumor staining, decreased caliber, or occlusion of the feeding artery.

Electrocochleography is more sensitive than distortion-product otoacoustic emission test for detecting noise-induced temporary threshold shift.

OBJECTIVE: We investigated and compared the usefulness of the electrocochleography and distortion product otoacoustic emission tests for detecting the earliest noise-induced damage by analyzing the sensitivity and specificity of the 2 tests. STUDY DESIGN: A prospective study. METHODS: After listening to music at 90.3 +/- 4.2 dB in the same night-club for 2 hours continuously, 23 healthy normal ears experienced a temporary threshold shift exceeding 5 dB. Pure-tone audiometry, the distortion product otoacoustic emission test, and electrocochleography were performed before, immediately after, and 24 hours after the exposure. RESULTS: Before exposure, the measured distortion product/noise floor was 9.8 +/- 10.4, 23.5 +/- 6.4, 18.7 +/- 6.4, and 19.1 +/- 5.6 dB sound pressure level (SPL) at frequencies of 1, 2, 3, and 4 kHz, respectively. Immediately after exposure, it decreased significantly at 2, 3, and 4 kHz to 16.6 +/- 7.6, 12.5 +/- 6.8, and 14.8 +/- 7.7 dB SPL, respectively. Marked increases in the amplitude of the summating potential and summating potential/action potential ratio were recorded from 0.15 +/- 0.06 to 0.32 +/- 0.11 and 0.23 +/- 0.06 to 0.44 +/- 0.08, respectively. The respective sensitivity and specificity of electrocochleography were 76.7% to 88.5% and 91.0% to 100%. Those of the distortion product otoacoustic emission test were 54.8% to 62.2% and 75.5% to 87.0%, respectively. CONCLUSION: Electrocochleography appears to provide more sensitive and specific information than the distortion product otoacoustic emission test for detecting a noise-induced temporary threshold shift.

Ruptured aneurysm of the anomalous cerebellar artery originating from internal carotid artery presenting with carotid cavernous fistula: a case report.

We report a case of a ruptured aneurysm of the anomalous cerebellar artery originating from internal carotid artery presenting with a carotid cavernous fistula (CCF). An initial angiogram showed the typical appearance of a CCF with a direct fistulous tract with focal aneurysmal dilation from the ascending cavernous segment of the right internal carotid artery. A postembolization angiogram of the right internal carotid disclosed the anomalous cerebellar artery originating from the fistula point of the internal carotid artery.

Microcatheter placement through a side hole created in a 5-F catheter into proximal subclavian arterial branches causing hemoptysis.

Five patients with moderate to massive hemoptysis who had a bronchial artery of anomalous origin or a nonbronchial systemic artery originating from the proximal subclavian artery underwent microcatheter placement through a created side hole of a 5-F catheter. All patients had pulmonary tuberculosis and had undergone bronchial artery embolization for hemoptysis. The side holes were made in the lesser (n = 2) or greater curvature sides (n = 3) of 5-F nonbraided Headhunter catheters. A microcatheter was passed through the side hole of the 5-F catheter into the target artery for embolization. Polyvinyl alcohol particles were used as the embolic material. The technical success rate was 100%, and immediate control of hemoptysis was achieved in all patients without complication.

[A case of the ruptured splenic artery aneurysm treated with transcatheter embolization].

Splenic artery aneurysms are the most common visceral artery aneurysms, which are usually found incidentally. The most common complication of splenic artery aneurysms is spontaneous rupture into the peritoneal cavity, which leads to acute peritonitis and shock. Less commonly, it may rupture directly into the stomach, small bowel or pancreatic duct and may present with acute gastrointestinal hemorrhage. We report a rare case of the ruptured splenic artery aneurysm, presenting as acute massive hematochezia which was treated with transcatheter embolization.

Supplemental transcatheter arterial chemoembolization through a collateral omental artery: treatment for hepatocellular carcinoma.

PURPOSE: To evaluate the therapeutic efficacy and safety of supplemental transcatheter arterial chemoembolization (TACE) through the extrahepatic collateral omental artery (OA) for the treatment of hepatocellular carcinoma (HCC). METHODS: We studied 21 patients with extrahepatic collaterals of the OA, among 1,512 patients with HCC who had undergone angiography. HCCs supplied by collateral OAs were located at: segment IV in seven, segment V in five, segment III in three, segment VI in three and segment VIII in three patients (Couinaud classification of segments). On preoperative CT scans, every HCC was abutting the liver surface. Adjacent omental infiltration or engorgement was noted in 11 patients. Celiac and hepatic arteriograms showed hypertrophy of the feeding OA in all patients. TACE of the OA was performed in 19 patients with an emulsion of iodized oil and doxorubicin hydrochloride. Embolization with gelatin sponge particles was added in five patients. RESULTS: Collaterals of the OA to the HCC were found on the first to seventeenth sessions of TACE. On follow-up CT scans, five patients showed complete uptake of iodized oil in the tumor. Partial uptake of iodized oil was noted in 13 patients and no uptake in one patient. There was no serious complication that related to the omental embolization, such as omental or bowel ischemia. The cumulative survival rates from the time of the TACE of the OA were 81% at 6 months and 68% at 12 months. CONCLUSION: TACE of the OA is safe and has a potential therapeutic effect in the treatment of HCC.

Extrahepatic collateral supply of hepatocellular carcinoma by the intercostal arteries.

PURPOSE: To evaluate computed tomographic (CT) and angiographic findings of extrahepatic collateral supply of hepatocellular carcinoma (HCC) by the intercostal artery (ICA) and the efficacy of transcatheter arterial chemoembolization (TACE) in the ICA. MATERIALS AND METHODS: The CT and angiographic findings of 30 ICA collateral supplies of HCC in 19 patients were evaluated. TACE of the ICA collaterals was performed in 10 patients. The clinical outcomes and complications were evaluated. RESULTS: ICA collaterals were found at the first to 18th sessions of TACE of HCC. The CT findings were: large HCC (mean diameter, 10.3 cm), subcapsular location (94.7%), defect in iodized oil retention or progression of HCC at subcapsular region (31.6%), HCC abutting the abdominal wall in a broad area with or without abdominal wall invasion (63.2%), hypertrophied ICA (31.6%), and branching collateral vessels coursing from the abdominal wall to the HCC (26.3%). On angiograms, all ICA collaterals originated from the right side at levels of T8 (6.7%), T9 (30.0%), T10 (46.7%), or T11 (16.7%). Twelve sessions of TACE of the ICA collaterals were performed in 10 patients. Follow-up angiography was performed in six patients and showed persistent obliteration in one, recanalization in three, and progression in two. Complications were shoulder pain (n = 2), itching sensation (n = 1), erythema of skin (n = 1), and skin necrosis (n = 1). CONCLUSION: ICA collateral supply of HCC usually occurs in advanced HCC or after multiple sessions of TACE. When there are suggestive CT findings, ICA collaterals should be sought when TACE is performed in the management of HCC.

MR cholangiography in symptomatic gallstones: diagnostic accuracy according to clinical risk group.

PURPOSE: To determine the diagnostic accuracy of magnetic resonance (MR) cholangiography in the detection of common bile duct (CBD) stones in patients with symptomatic gallstones. MATERIALS AND METHODS: Single-shot half-Fourier MR cholangiographic images were obtained in 121 consecutive patients who were referred for MR cholangiography prior to cholecystectomy for symptomatic gallstones. One radiologist interpreted the MR cholangiographic images without prior knowledge of laboratory test results or findings from examinations with other imaging modalities. Patients were retrospectively divided into three groups with high, moderate, and low risk (n = 70, 22, and 29, respectively) for CBD stones. The sensitivity, specificity, and accuracy of MR cholangiography in the detection of CBD stones in each group were calculated. The accuracy of laboratory and sonographic findings in the identification of CBD stones was also calculated. RESULTS: CBD stones were diagnosed in 49 (70%) of 70 patients in the high-risk group. One false-positive diagnosis was made, and two false-negative diagnoses were made. Of the 22 patients in the moderate-risk group, seven (32%) had CBD stones, and there was one false-positive and one false-negative diagnosis. In the low-risk group, CBD stones were correctly identified with MR cholangiography in one patient, and there was one false-positive diagnosis. A combination of positive laboratory and sonographic findings yielded a sensitivity of 98% but a specificity of only 34% if any unusual finding in any one of the tests was regarded as a positive finding. CONCLUSION: MR cholangiography is highly accurate in the detection of CBD stones in all patients, regardless of the grade of risk, and should be considered in patients with a moderate to high risk of CBD stones before cholecystectomy.

The arterial folding point during flexion of the hip joint.

PURPOSE: Endovascular stents placed in periarticular vessels may be at a greater risk of neointimal hyperplasia and eventual occlusion than those placed in non-periarticular vessels. The purpose of this study was to investigate the location of maximal conformational change along the iliac and femoral artery, the folding point, during flexion of the hip joint and its location relative to the hip joint and the inguinal ligament. METHODS: Seventy patients undergoing femoral artery catheterization were evaluated. The patients were 47 men and 23 women and ranged in age from 26 to 75 years (mean 54 years). The arteries (right:left=34:36) were measured using a marked catheter for sizing vessels. Fluoroscopic images were obtained in anteroposterior and lateral projections in neutral position, and in the lateral projection in flexed position of the hip joint. The folding point was determined by comparing the lateral projection images in the neutral and flexed positions. The distance from the acetabular roof to the folding point and the distance from the inguinal ligament to the folding point was evaluated. RESULTS: The folding point was located 42.8 +/- 28.6 mm cranial to the acetabular roof and 35.1 +/- 30.1 mm cranial to the inguinal ligament. As the patient's age increased, the folding point was located more cranially (p < 0.001). CONCLUSIONS: The folding point during flexion of the hip joint was located 42.8 +/- 28.6 mm cranial to the acetabular roof and 35.1 +/- 30.1 mm cranial to the inguinal ligament. As the patient's age increased, the folding point was located more cranially. When a stent is inserted over this region, more attention may be needed during follow-up to monitor possible occlusion and stent failure.

Accuracy in differentiation of mucinous and nonmucinous rectal carcinoma on MR imaging.

PURPOSE: The purpose of this work was to determine the diagnostic accuracy of MRI for the differentiation of mucinous and nonmucinous carcinomas in patients with rectal carcinomas. METHOD: Seventy-six patients with pathologically proven rectal adenocarcinomas were evaluated with MRI. Both T2-weighted fast spin echo and T1-weighted spin echo images were independently reviewed by two radiologists who were blinded for tumor histology. Of the 76 cases, mucin pools were found in 28 (37%), and 16 had a mucin pool occupying >or=50% of the tumor (mucinous carcinoma). The sensitivity, specificity, accuracy, and positive and negative predictive values of distinguishing between mucinous and nonmucinous carcinomas were calculated. The interobserver agreement was estimated using kappa statistics. RESULTS: The accuracies of the diagnosis of mucinous carcinoma were 97% for Radiologist 1 and 96% for Radiologist 2. The sensitivities and specificities for diagnosing a mucinous carcinoma were 94 and 98% for Radiologist 1 and 100 and 95% for Radiologist 2, respectively. Interobserver agreement was good (kappa = 0.61) for the identification of a mucin pool and was excellent (kappa = 0.81) for the diagnosis of a mucinous carcinoma. CONCLUSION: Differentiation of mucinous and nonmucinous rectal carcinoma is highly accurate on preoperative MRI.

Spontaneous pneumomediastinum and hemopneumothoraces secondary to cystic lung metastasis.

We report a case of a cystic metastasis to the lung from an angiosarcoma of the scalp in a 75-year-old man who complained of hemoptysis. A chest CT scan showed multiple thin-walled pulmonary cysts, bilateral pneumothoraces, small nodules and pneumomediastinum. Histologic examination revealed pleural infiltration of angiosarcoma cells. One month later, a high-resolution CT scan showed that the cysts had rapidly developed into large lesions.

Internal thoracic artery collateral to the external iliac artery in chronic aortoiliac occlusive disease.

OBJECTIVE: To evaluate the incidence and angiographic findings of the collateral pathway involving the internal thoracic artery in patients with chronic aortoiliac occlusive disease. MATERIALS AND METHODS: Between March 2000 and Februrary 2001, 124 patients at our hospital underwent angiographic evaluation of chronic aortoiliac occlusive disease, and in 15 of these complete obstruction or severe stenosis of the aortoiliac artery was identified. The aortograms and collateral arteriograms obtained, including internal thoracic arteriograms, as well as the medical records of the patients involved, were evaluated. RESULTS: In nine patients there was complete occlusion of the infrarenal aorta, or diffuse stenosis of 75% or more in the descending thoracic aorta, and in the other six, a patent aorta but complete occlusion or stenosis of 75% or more of the common iliac artery was demonstrated. Collateral perfusion via hypertrophied internal thoracic arteries and rich anastomoses between the superior and inferior epigastric arteries, reconstituting the external iliac artery, were noted in all fifteen patients, regardless of symptom duration, which ranged from six months to twelve years. CONCLUSION: In patients with chronic aortoiliac occlusive disease, the internal thoracic artery, along with visceral collaterals and those from the contralateral side, is one of the major parietal collateral pathways.

Multislice CT cholangiography using thin-slab minimum intensity projection and multiplanar reformation in the evaluation of patients with suspected biliary obstruction: preliminary experience.

Thirty-three patients with suspected biliary obstruction were prospectively evaluated with multislice CT cholangiography using thin-slab minimum intensity projection (MinIP) and multiplanar reformation (MPR) to determine its usefulness and to compare with the comparative studies of endoscopic retrograde cholangiography (ERC), magnetic resonance cholangiography (MRC), or percutaneous transhepatic cholangiography (PTC). CT cholangiography made correct diagnoses in all biliary obstructions except in two patients with common bile duct stones. The correspondence with the comparative study was 93.9%. Multislice CT cholangiography may be favorable in noninvasive evaluation of biliary obstructions.
