Electrocardiographic features of Korean carbon disulfide poisoned subjects after discontinuation of exposure.

OBJECTIVE: Mass poisoning by carbon disulfide (CS(2)) was reported at a viscose rayon factory in Korea in the late 1980s; the factory was subsequently closed in 1993. In this study, we evaluated the late electrocardiographic features in the CS(2) poisoned subjects. METHODS: The cases were composed of 198 retired workers with a history of CS(2) poisoning. An age- and sex-matched reference group was randomly selected from people visiting a hospital for regular health checkups. Intervals, amplitudes and abnormal findings from the electrocardiograms (ECGs) of the two groups were compared. RESULTS: Except for the PQ interval, there were no significant ECG differences between the two groups. The cases exhibited ECG abnormalities more frequently than the reference group. Multiple logistic regression analysis revealed that CS(2) poisoning was positively associated with the presence of abnormal ECG findings (OR = 1.603; 95% CI, 1.008-2.549). CONCLUSION: This study suggests that CS(2) poisoned subjects may have an increased risk of ECG abnormalities even after the exposure to CS(2) is no longer present.

Tumor necrosis factor alpha promoter polymorphism associated with increased susceptibility to secondary hemophagocytic lymphohistiocytosis in the Korean population.

Secondary hemophagocytic lymphohistiocytosis (HLH) can be associated with various diseases including infections and lymphoma. The clinical findings of HLH can be explained by an increased production of cytokines such as tumor necrosis factor alpha (TNF-alpha). As not all the patients with infection or lymphoma have secondary HLH, we investigated the relationship between susceptibility to secondary HLH and TNF-alpha promoter polymorphisms to identify genetic factors of secondary HLH. We determined the alleles of four promoter sites (-1031/-857/-308/-238) of TNF-alpha gene by using Taqman-based allelic discrimination assays in the 66 Korean patients with secondary HLH and 100 healthy Korean controls. We found that the frequency of the TNF-alpha -1031C allele, which is associated with higher-plasma TNF-alpha levels, was enriched in patients with secondary HLH compared with healthy controls (OR=2.00, 95% CI 1.20-3.30, P=0.007). In haplotype analysis of TNF-alpha polymorphisms, the haplotype H6 (CTGG) was detected only in the patient group, and the haplotype group (H2 or H5 or H6) including TNF-alpha -1031C allele was overexpressed in secondary HLH patients (OR=2.52, 95% CI 1.33-4.77, P=0.004). These results suggested that TNF-alpha -1031C allele and its associated haplotypes in Koreans may enhance susceptibility to secondary HLH.

Psychometric properties of the Korean version of the job content questionnaire: data from health care workers.

OBJECTIVES: To test the validity and reliability of selected scales, namely, decision latitude, psychological job demand, social support, job insecurity, and macro-level decision latitude from the Korean version of the job content questionnaire (K-JCQ), as part of a psychosocial epidemiological study among university hospital workers. METHODS: K-JCQ was developed by translation and back translation complying with the JCQ usage policy, and its psychometric properties were explored among 338 workers (290 females and 48 males) in a university hospital in Korea. Internal consistency was examined using Cronbach's alpha correlation coefficients. Factorial validity was tested using exploratory factor analysis. Pearson's correlation coefficients were used for test-retest reliability among a subset of 157 workers who responded to a repeat survey. Criterion-related validity was assessed by investigating the effects of the scales on job satisfaction and self-identity through work in multiple regression models. RESULTS: Cronbach's alpha for all selected scales was higher than 0.6, except for job insecurity (0.53) and macro-level decision authority (0.52), indicating appropriate internal consistency. Correlation coefficients between test and retest scales of decision latitude, psychological job demand, and social support were 0.60, 0.41, and 0.35, respectively. Exploratory factor analysis found three- and four-factor models, i.e., with and without macro-level decision latitude, respectively, closely corresponding to the theoretical constructs. High levels of decision latitude and social support, and low levels of psychological job demand and job insecurity were significantly associated with high level of job satisfaction. Higher self-identity through work was positively related to decision latitude and social support. CONCLUSIONS: These findings suggest that K-JCQ is valid and reliable for assessing psychosocial job stress among Korean workers. Macro-level decision latitude showed a separate factorial structure and was strongly associated with task-level decision latitude.

Predictors of interest in nutrition topics and willingness to participate in local nutrition programmes.

OBJECTIVE: To investigate the predictors of interest in nutrition topics and willingness to participate in weight management courses and nutrition-related chronic disease management courses. METHODS: Data from a cross-sectional survey (n = 1007 Koreans aged >or=30) on nutrition-related conditions, nutrition behaviours, interest in nutrition topics and willingness to participate in two types of nutrition education courses, and general characteristics. Binary logistic regression was used to identify predictors of the participants' interest in the topic and willingness to participate. RESULTS: Younger, richer and more-educated participants were significantly more likely to be willing to participate in 'nutrition-related chronic disease management'. Having a higher income and being male increased the odds of being interested in this topic. Younger, more obese, high-income females and those interested to keep their weight and reduce fatty foods were more likely to be willing to participate in 'weight management'. CONCLUSION: There is a need to increase awareness about the link between nutrition and related health problems, especially regarding 'hidden' health problems to tailor the programmes especially for older, low-income and less-educated people and to focus on increasing their interest in nutrition topics.

Acquired dyschromatopsia among petrochemical industry workers exposed to benzene.

Exposure to organic solvents, which are widely used in industry, can lead to dysfunction of the nervous system. However, controversy continues about the nature of early-stage damage to the nervous system from low-grade chronic exposure to organic solvents. Since loss of color-vision can be a sensitive early marker of neurotoxic damage, the main aim of this study was to investigate the association between low-level chronic exposure to organic solvents, especially benzene, and acquired dyschromatopsia. The study initially comprised 1236 workers who were employed at a large petrochemical distillation factory. After excluding those workers who may have had color-vision impairment due to congenital or acquired eye diseases and those with other medical conditions, 908 males who had worked for at least 6 months were included in the final analysis. Those who worked only in the office were categorized as nonexposed, while those who worked at outside facilities were divided into three groups of approximately equal size according to their estimated cumulative exposure levels to benzene (low, medium, high). Color-vision was assessed using the Lanthony D-15 desaturated panel color test. The results showed that the color-confusion index (CCI) was positively related to age. In the qualitative assessment of types of color-vision loss, the prevalence of total dyschromatopsia was significantly higher with increasing cumulative exposure levels in the left eye (p<0.05) but not in the right eye. The significance for the prevalence of type III dyschromatopsia was borderline in the left eye (p=0.0571). The relationship between acquired dyschromatopsia and exposure level also showed an increase in the odds ratio in the left eye but not in the right eye. Taken together, these results suggest that chronic low-level exposure to benzene can lead to acquired dyschromatopsia.

The association between neurobehavioral performance and lung function.

This study investigated the relationship between neurobehavioral performance and pulmonary function in young and middle adults. Data were collected from 178 subjects (88 male, 90 female) aged 20-50 years. Neurobehavioral performance was assessed by the Korean version of The Swedish Performance Evaluation System (SPES-K), on five items: simple reaction time, symbol digit, digit span, and finger tapping speed for dominant and non-dominant hands. Pulmonary function, as indicated by the 1-s forced expiratory volume (FEV1), was measured according to American Thoracic Society criteria and the potential association was analyzed by linear regression. For simple reaction time, symbol digit, and digit span, the performance of subjects with FEV1 levels exceeding 3.29 l was 2-4% greater than those with the lowest levels of less than 2.60 l, after controlling for the effects of age, sex, education, smoking, and height. Finger tapping speed, however, did not show any significant association. These findings suggest that pulmonary function may influence neurobehavioral performance, especially cognitive abilities, throughout adulthood.

The relationship between nonstandard working and mental health in a representative sample of the South Korean population.

In light of escalating job insecurity due to increasing numbers of nonstandard workers, this study examined the association between nonstandard employment and mental health among South Korean workers. We analyzed a representative weighted sample of 2086 men and 1194 women aged 20-64 years, using data from the 1998 Korean National Health and Nutrition Examination Survey. Nonstandard employment included part-time work, temporary work, and daily work. Mental health was measured with indicators of self-reported depression and suicidal ideation. Based on age-adjusted prevalence of mental health, nonstandard employees were more likely to be mentally ill compared to standard employees. Furthermore, nonstandard work status was associated with poor mental health after adjusting for socioeconomic position (education, occupational class, and income) and health behaviors (smoking, alcohol consumption, and exercise). However, the pattern of the relationship between nonstandard work and mental health differed by gender. Female gender was significantly associated with poor mental health. Although males tended to report more suicidal ideation, this difference was not statistically significant. Considering the increasing prevalence of nonstandard working conditions in South Korea, the results call for more longitudinal research on the mental health effects of nonstandard work.

Neurobehavioral function in children with low blood lead concentrations.

Debate continues as to whether low blood lead level, even below 5 microg/dl, impairs neurodevelopmental function in children. This study assessed the association between neurodevelopmental performance and lead exposure below 5 microg/dl in Korean children. Data were collected from 61 children, aged 7-16 years with a mean blood lead level of 2.9 microg/dl, who participated with their mothers. Neurodevelopmental function was measured with computer-based neurobehavioral tests. The analyzed data included children's performances on five neurobehavioral tests as the dependent variables, and children's blood lead level, sex, age, and mother's performance as the four independent variables. For simple reaction time and digit span, which, respectively reflect attention and short-term memory, there was a small and significant association between the children's neurodevelopmental performance and their blood lead levels (p=0.05, 0.08, respectively). Our study suggests that low blood lead below 5 microg/dl can influence children's neurobehavioral performance.

The effects of genetic polymorphisms of IL-1 and TNF-A on Helicobacter pylori-induced gastroduodenal diseases in Korea.

OBJECTIVE: To evaluate whether or not genetic polymorphism of IL-1B, IL-1RN, and TNF-Alpha is an important factor in the different expression of gastroduodenal diseases after Helicobacter pylori infection. METHODS: This study consisted of 1360 subjects: control, gastric cancer (GC, intestinal type, and diffuse type), benign gastric ulcer (BGU), duodenal ulcer, and first-degree gastric cancer relative (GCR). IL-1Beta-511 and TNF-A-308 biallelic polymorphism were genotyped by 5' nuclease polymerase chain reaction (PCR) assays, and PCR-restriction fragment length polymorphism (PCR-RFLP). IL-1RN penta-allelic variable number of tandem repeats was genotyped by PCR. RESULTS: There was no difference in the genetic polymorphism of IL-1Beta-511, IL-1RN and TNF-A in the patients with gastric cancer regardless of H. pylori positivity compared with control. However, the frequencies of IL-1B-511 C/T (OR: 0.5, 95% CI: 0.3-0.7) and T carrier (OR: 0.6, 95% CI: 0.4-0.8) were lower in the H. pylori-positive BGU patients. The IL-1RN 2/2 was higher (OR: 5.5, 95% CI: 1.1-28.5) in the H. pylori-positive GCR. There was no significance in the polymorphism of TNF-Alpha-308 regardless of H. pylori-induced gastroduodenal diseases. CONCLUSIONS: The IL-1Beta-511 T-carrier polymorphism has a negative effect on the development of H. pylori-positive BGU, and high frequency of IL-1RN 2/2 was found in the H. pylori-positive relatives of GC patients, which suggest that this genetic polymorphism could play some role in the H. pylori-induced gastroduodenal diseases in Korea.

Clonorchis sinensis infection and increasing risk of cholangiocarcinoma in the Republic of Korea.

To evaluate the prevalence and risk factors of Clonorchis sinensis infection, an epidemiologic survey was performed (3,169 residents; age range: 30-87 years; mean age = 59; 1,973 women) in three areas with different mortality rates of cholangiocarcinoma. C. sinensis prevalence was 2.1% in Chuncheon, 7.8% in Chungju, and 31.3% in Haman, where cholangiocarcinoma incidence rate was 0.3, 1.8, and 5.5 per 100,000 persons, respectively. Of the four major risk factors for C. sinensis (male sex, alcohol consumption, raw freshwater fish intake, and area of residence), area was the strongest. Efforts to discourage raw freshwater fish intake, treat egg carriers, and protect water sources from C. sinensis contamination should be strengthened in the Republic of Korea.

Obesity has a greater impact on cardiovascular mortality in younger men than in older men among non-smoking Koreans.

INTRODUCTION: We investigated the effects of age on the relationship between body mass index (BMI) and cardiovascular risk factors and cardiovascular mortality in non-smoking Korean men. METHODS: We performed a prospective cohort study of 246,146 non-smoking Korean men aged 20-69 years at baseline (1992) who were initially without history of cancer or weight change. The associations between BMI and cardiovascular risk factors and mortality during an 9-year follow-up period (2000) were stratified by age group after adjustment for family history, alcohol consumption, exercise habits, and economic status. RESULTS: Calculations of odds ratios (ORs) revealed that younger men (<40 years) with greater BMI (>or=28 kg/m2) were at greater risk of high blood pressure, high blood glucose, and high total cholesterol than older men. The ORs for cardiovascular risk factors associated with greater BMI declined linearly with age. The relative risks for mortality from stroke and from all cardiovascular diseases associated with greater BMI were also higher among younger men and declined linearly with age. CONCLUSIONS: The cardiovascular risk factors and mortality associated with greater BMI were higher among younger than older non-smoking Korean men. These findings indicate that obesity has a greater impact among younger men with respect to premature cardiovascular related deaths.

Multiple metabolic risk factors and total and cardiovascular mortality in men with low prevalence of obesity.

We investigated the association between the multiple metabolic risk factors and cardiovascular and overall mortality among Korean men, a population with a low prevalence of obesity. This prospective cohort study involved 682,597 Korean men, aged 30-69 years at baseline (1992), who were initially without cancer or debilitating diseases. Death due to ischemic heart disease (IHD), stroke, cardiovascular disease (CVD), and any cause among men relative to metabolic risk factors, including overweight, high blood pressure, high fasting glucose, and high total cholesterol was analyzed. There were 17,785 deaths during the 8.5-year follow-up, of which 874 were due to IHD, 1644 to stroke, and 3306 to CVD. As a function of the number of metabolic risk factors, the relative risk of death from CVD was 2.0 (1.7-2.2), 2.9 (2.5-3.3), 3.5 (3.0-4.1), and 5.0 (3.9-6.4) for 1, 2, 3, and 4 risk factors, respectively, whereas the relative risk of death from all causes was 1.3 (1.2-1.4), 1.5 (1.4-1.6), 1.6 (1.5-1.7), and 1.9 (1.6-2.2) for 1, 2, 3, and 4 risk factors, respectively. The relative risk of IHD, stroke, CVD and all-cause mortality increased linearly with the number of metabolic risk factors. Early identification and strict management of metabolic risk factors should be reinforced in Koreans, even though there is a low prevalence of obesity among this population.

Familial aggregation of the metabolic syndrome in Korean families with adolescents.

We investigated the familial aggregation of the metabolic syndrome in Korean families with adolescents. In a cross-sectional observational study, the body mass index, waist circumference, blood pressure, fasting glucose, serum triglyceride, HDL-cholesterol, total cholesterol and fasting insulin concentrations, and homeostasis model assessment (HOMA) score, were examined in each individual in 132 Korean nuclear families. Most variables of the metabolic syndrome in offspring were significantly correlated with those of parents. Compared with sons, daughters had more significant difference for the metabolic parameters according to clustering of risk factors of their parents. Especially, daughters showed higher correlations with their parents for waist circumference, with their mothers for fasting glucose and HDL-cholesterol, and with their fathers for fasting insulin than sons. Compared with children whose parents did not have the metabolic syndrome, the odds ratios in children with at least one parent with the metabolic syndrome were 4.1 (1.6-10.6) for overweight, 3.6 (1.3-10.2) for abdominal obesity, 5.0 (2.0-12.3) for high triglycerides, and 4.8 (1.1-21.0) for the metabolic syndrome. We also observed significant correlations in variables of the metabolic syndrome between siblings and between spouses. In Korean families with adolescents, there is a familial aggregation of the metabolic syndrome, with daughters resembling their parents more than sons. These findings may have significant implications for clinical interventions directed at adolescents at high risk for the metabolic syndrome.

Gender differences in job strain, effort-reward imbalance, and health functioning among Chinese physicians.

To examine the association between work stress measured by job strain and effort-reward imbalance (ERI) and health functioning in a sample of hospital-based Chinese physicians, a self-reported survey with a standardized questionnaire was conducted in three hospitals in China, among 256 men and 266 women. It was found that both job strain and ERI were associated with impaired health functioning in men and women, but that ERI indicated a stronger association. Men's job control was significantly higher, and was related to men's physical health; whereas women perceived relatively higher job reward which predicted women's mental health. The findings provide evidence of the adverse effects on health functioning of both job strain and ERI, but ERI appears to have more explanatory power as a model of work stress in this sample of Chinese physicians. In addition, gender differences of work stress with respect to health are present.

Effects of air pollutants on childhood asthma.

Epidemiologic studies have suggested the association between environmental exposure to volatile organic compounds (VOCs) and polycyclic aromatic hydrocarbons (PAHs) and the increased risk of incurring asthma. Yet there is little data regarding the relationship between personal exposure to air pollution and the incidence of asthma in children. This study was designed to evaluate the effect of exposure to air pollution on children with asthma by using exposure biomarkers. We assessed the exposure level to VOCs by measuring urinary concentrations of hippuric acid and muconic acid, and PAHs by 1-OH pyrene and 2-naphthol in 30 children with asthma and 30 children without asthma (control). The mean level of hippuric acid was 0.158 +/- 0.169 micromol/mol creatinine in the asthma group and 0.148 +/- 0.249 micromol/mol creatinine in the control group, with no statistical significance noted (p=0.30). The mean concentration of muconic acid was higher in the asthma group than in the control group (7.630 +/- 8.915 micromol/mol creatinine vs. 3.390 +/- 4.526 micromol/mol creatinine p=0.01). The mean level of urinary 1-OHP was higher in the asthma group (0.430 +/- 0.343 micromol/mol creatinine) than the control group (0.239 +/- 0.175 micromol/mol creatinine), which was statistically significant (p=0.03). There was no difference in the mean concentration of 2-NAP between the two groups (9.864 +/- 10.037 micromol/mol in the asthma group vs. 9.157 +/- 9.640 micromol/mol in the control group, p=0.96). In conclusion, this study suggests that VOCs and PAHs have some role in asthma.

Effort-reward imbalance at work and job dissatisfaction in Chinese healthcare workers: a validation study.

OBJECTIVE: The aim of this study was to test the reliability and validity of the Chinese version of the 23-item effort-reward imbalance (ERI) questionnaire and to analyze its association with job dissatisfaction in a sample of Chinese healthcare workers. METHODS: A self-reported survey was conducted, in university hospitals of China, among 192 male and 608 female healthcare workers. RESULTS: Appropriate internal consistencies of the three scales: effort, reward, and overcommitment, were obtained. Exploratory factor analysis replicated the theoretically assumed structure of the ERI construct in men and women. Evidence of criterion validity was obtained from cross-correlations of the scales and from their correlations with gender, education and job dissatisfaction. Finally, all three scales were associated with an elevated odds ratio of job dissatisfaction, and the effect was strongest for the ERI ratio as predicted by theory. CONCLUSION: Based on the results of this study the Chinese version of the ERI questionnaire is considered a reliable and valid instrument for measuring psychosocial stress at work. It is applicable to Chinese working populations and, in particular, to the healthcare sector.

Feulgen reaction study of novel threadlike structures (Bonghan ducts) on the surfaces of mammalian organs.

Threadlike structures on the surfaces of internal organs, which are thought to be part of the Bonghan duct system, were first reported about 40 years ago, but have been largely ignored since then. Recently, they were rediscovered, and in this study we discuss the Feulgen reaction that specifically stains DNA in order to identify these structures on the surface of rabbit livers as part of the Bonghan system. The distribution, shapes, and sizes of their nuclei are found to be similar to those of intravascular threadlike structures. The endothelial nuclei are rod-shaped, 10-20 mum long, and aligned in a broken-line striped fashion. The threadlike structure consists of a bundle of several subducts, which is a characteristic feature of Bonghan ducts and distinguishes them morphologically from lymphatic vessels. In addition, the Feulgen reaction clearly demonstrates that the subducts pass through a corpuscle, which is usually irregular or oval-shaped and is connected to two or several threadlike structures that form a web on the surfaces of organs. Furthermore, spherical granules of about 1 mum in diameter are detected in the subducts. These granules were well stained by using the Feulgen reaction, which implies that they contain DNA. According to previous reports, a granule is a type of microcell and plays an essential role in the physiology and therapeutic effect of the Bonghan system and acupuncture. This role has yet to be elucidated.
