Telepathology as an aid in mohs micrographic surgery.

BACKGROUND: Mohs surgeons are occasionally confronted by challenging pathology ideally requiring the advice of a dermatopathologist. The Internet transmission of digital images of the pathology (telepathology) allows for such opinions to be easily and rapidly obtained. OBJECTIVE: The objective was to obtain images utilizing a digital camera focused directly through one microscope eyepiece with subsequent e-mail to a pathologist for an immediate opinion. METHODS: The particular area of interest on the slide is selected. The lens of the digital camera is placed directly on one eyepiece of the microscope and using the zoom and autofocus options of the camera a sharp image is obtained. The camera flash must be disabled. The images are immediately downloaded to computer and e-mailed to the pathologist for an opinion. RESULTS: Three case reports illustrate that the pathologist is able to offer suggestions on a diagnosis with a high degree of confidence using the e-mailed images. CONCLUSION: Digital camera technology now allows for images to be taken directly through an eyepiece of the microscope. These images can be almost instantaneously e-mailed to a pathologist anywhere in the world for an immediate opinion. The technique provides for greater surety where doubt exists about the pathology during Mohs surgery.

Primary non-Hodgkin's lymphoma of the nose and paranasal sinuses: a case report.

Non Hodgkin's Lymphomas of the sinonasal tract are uncommon neoplasms that can be morphologically difficult to distinguish from destructive non-neoplastic process or other malignant neoplasm in this site. Non-Hodgkin's Lymphomas of the sinonasal tract are heterogeneous diseases that can be clinically aggressive. Immunophenotypic pattern of Non-Hodgkin's Lymphomas shows marked variations. We are reporting a case of B-Cell type of Non-Hodgkin's Lymphoma of the nasal cavity and paranasal sinus with complete clinicopathological and immunohistochemical study as it is rare in Indian literature and review of the literature is done.

Memory protective effect of indomethacin against electroconvulsive shock-induced retrograde amnesia in rats.

BACKGROUND: Nonsteroidal antiinflammatory drugs (NSAIDs) have been suggested to retard cognitive decrements in patients with Alzheimer's disease. We postulated that NSAIDs also may protect acute disruption of memory. METHODS: We studied the effect of indomethacin (4 mg/kg/day) administered daily for 19 days on retrograde amnesia induced by two once-daily electroconvulsive shocks in rats. RESULTS: Indomethacin produced statistically significant prolongation of recall latency in a passive avoidance task using a step-down apparatus. CONCLUSIONS: Our study suggests that NSAIDs may prevent memory disruption through other mechanisms apart from attenuating chronic inflammation. In patients receiving electroconvulsive therapy, as in those diagnosed with Alzheimer's disease, antiinflammatory drugs may hold promise in the attenuation of cognitive impairments.

Effect of potassium salts and distillery effluent on carbon mineralization in soil.

Distillery effluent, a rich source of potassium, is used for irrigation at many places in the world. A laboratory experiment was conducted to study the influence of potassium salts present in post-methanation distillery effluent (PME) along with two other salts, KCl and K2SO4, on mineralization of carbon in soil. PME oxidized with H2O2, raw PME, KCl and K2SO4 solutions containing K equivalent to 10%, 20%, 40% and 100% of K present in PME were added to the soil separately, maintaining four replications for each treatment and control. Addition of salts up to a certain concentration stimulated C mineralization but a decline was noticed at higher concentrations. All the levels of salts caused higher CO2 evolution than the control suggesting that the presence of K salts enhanced the microbial activity resulting in increased CO2 evolution. The influence of K2SO4 was significantly higher than KCl in stimulating C mineralization in soil. Oxidized effluent had a higher stimulating effect than inorganic salts, showing the influence of other salts accompanying K in the PME. Raw PME, which contained excess organic C, increased CO2 evolution even at the highest salt level (100% PME) signifying the effect of added C on alleviating the salt stress on microbial activity.

Primary cutaneous zygomycosis due to Mucor circinelloides.

A 62-year-old woman with myelodysplastic syndrome presented with a 4-week history of a large indurated ulcer with a black eschar on the forearm following trauma. On biopsy a diagnosis of zygomycosis was made as broad, sparsely septate, thin-walled hyphae were seen in the deep dermis and subcutaneous fat. The zygomycete fungus Mucor circinelloides was cultured from tissue. Further investigation confirmed that the infection was localized to the skin. The 6 x 4 cm lesion was excised and the defect closed with a neurovascular island flap. No other treatment was undertaken. The patient died 6 months later from her haematological disease without recurrence of the fungal infection.

Necrobiotic xanthogranuloma occurring within linear morphoea.

A 32-year-old woman with linear morphoea of her left arm was noted to have a yellowish atrophic plaque within the linear morphoea. Review of photographs taken on her first visit 3 years previously revealed that the lesion had been present and had increased in size over the 3 years. Necrobiotic xanthogranuloma (NXG) was diagnosed on biopsy. Paraproteinaemia was not detected. The NXG was surgically excised and it has not recurred in 12 months of follow up.

Benign nodular lymphoid hyperplasia of colon: a report of two cases.

Benign nodular lymphoid hyperplasia of the colon in children is a rare entity, distinct from lymphoid polyps, and must be differentiated from malignant lymphomas. We report two girls, 9 years old and 5 years old, in whom nodular hyperplasia caused intestinal obstruction. No etiologic cause was found in them. Immunochemistry (Pan B and Pan T) markers in one case showed staining in the respective areas of the lymphoid aggregates. Viral marker studies were not done. Surgical resection was done.
