Limb threatening neuropathic complications from ankle fractures in patients with diabetes.

Although less common than neuropathic (Charcot) deformities in feet, ankle deformities can occur and produce significant limb threatening complications after injury in patients with longstanding diabetes. A series of six such complications, including one that resulted in amputation and several near amputations, are presented. The authors' primary purpose is to alert or remind physicians and orthopaedic surgeons of the often unanticipated but significant complications of these rare and seemingly mild ankle injuries. A secondary purpose is to point out that complications may be avoided or mitigated with early stabilization and possibly with treatment of bone resorbing, inflammatory processes characteristic of neuroarthropathies.

Bilateral spontaneous patellar tendon rupture.

This report describes a bilateral spontaneous patellar tendon rupture in a patient without pre-existing systemic disease. A possible association is recognized that may predispose a patient to bilateral spontaneous patellar tendon rupture.

Upper extremity compartment syndromes following pit viper envenomation.

The treatment of pit viper envenomation by fasciotomy continues to be controversial because elevated intracompartment pressures have never been previously documented. Two patients with severe pit viper envenomation of the thumb had signs and symptoms of elevated intracompartment pressures. Elevated intracompartment tissue pressures were documented in both cases. Immediate fasciotomies were necessary to decompress the thenar and forearm compartments and, in one case, the upper arm as well. Ischemic tissues were noted at the time of the operation and confirmed the clinical and objective tissue pressure measurements of compartment syndromes. Both patients made complete functional recoveries following delayed primary closure. Intracompartment pressure monitoring should be routinely used in the management of pit viper envenomation to avoid disastrous sequellae.
