Activation of blood coagulation after cardiac arrest is not balanced adequately by activation of endogenous fibrinolysis.

BACKGROUND: Animal studies have demonstrated that hemostatic disorders occurring after cardiac arrest affect outcome. We investigated hemostatic changes during and after cardiopulmonary resuscitation (CPR) in humans. METHODS AND RESULTS: The prospective study included 23 patients (29 to 86 years) who underwent out-of-hospital CPR for nontraumatic causes. Blood samples were drawn immediately and 15 and 30 minutes after initiation of CPR. In the case of restoration of spontaneous circulation (ROSC; n = 7), additional blood samples were taken immediately, 30 minutes, and 2, 8, 24, 48, and 72 hours after ROSC. A marked activation of blood coagulation was found in all patients. The specific markers of activated blood coagulation and fibrin formation, thrombin-antithrombin complex (TAT; median during CPR, 260 micrograms/L; median after ROSC, 57 micrograms/L; normal range, 1.0 to 4.1 micrograms/L), and fibrin monomers (FM; median during CPR, 34.3 micrograms/mL; median after ROSC, 65.4 micrograms/mL; normal range, 0 to 3.6 micrograms/mL) were markedly increased during and in the early phase after CPR. When patients survived for 48 hours, TAT and FM values returned to the normal range. In most patients, the plasma levels of D-dimer, an indicator of endogenous fibrinolytic activity, were not markedly increased during CPR (median, < 0.25 microgram/mL; normal range, < 0.25 microgram/mL) but increased moderately after ROSC (median, 0.56 microgram/mL). Levels of plasminogen activator inhibitor type 1 (normal range, 0.3 to 3.5 U/mL), a marker for endogenous inhibition of fibrinolytic activity, were moderately increased in most patients (median during CPR, 4.22 U/mL; median after ROSC, 8.08 U/mL). CONCLUSIONS: Our data clearly demonstrate that there is a marked activation of blood coagulation and fibrin formation after prolonged cardiac arrest and CPR in humans that is not balanced adequately by concomitant activation of endogenous fibrinolysis. These changes may contribute to reperfusion disorders, such as the cerebral "no-reflow" phenomenon, by inducing fibrin deposition and formation of microthrombi.

Local effects of different perfluorochemical agents.

PURPOSE: To clarify whether the prolonged presence of perfluorochemicals (PFC) in the vitreous cavity causes oxidative tissue damage and inflammatory response of the retina and, if so, what the process is. METHODS: After three different perfluorochemicals [perfluorodecalin (C10F18), perfluorooctane (C8F18) and Fluosol-DA (corresponding to a 20% emulsion of 70% PFD and 30% perfluorotripropylamine, C9F21N)] had been in the vitreous cavity of rabbits for 2 weeks, lipid peroxide concentration and myeloperoxidase activity in the retina were determined. RESULTS: Whereas only Fluosol-DA showed significant oxidative damage, the inflammatory activity was significantly increased in all groups. CONCLUSION: The increased myeloperoxidase activity and the observed oxidative damage of the retina seem to be the effect of both perfluorochemical-loaded macrophages and inflammatory-induced lipid peroxidation.

Systemic effects of different perfluorochemical agents.

PURPOSE: An intravitreal injection of Fluosol-DA leads to a higher alteration of the macrophage system of the retina than perfluorooctane and perfluorodecalin administered by the same route. The difference may be due to different kinds of oxidative damage caused by the three chemicals. To test the validity of this assumption, the degree of cell alteration, expressed as reduction of cytoplasmic motility, caused by these three perfluorochemicals was examined. METHODS: Using the hepatic macrophage system of the rat, cell alteration was examined by magnetometry after intravenous application of various perfluorochemicals [emulsified perfluorodecalin (C10F18), perfluorooctane (C8F17Br) and Fluosol-DA (corresponding to a 20% emulsion of 70% perfluorodecalin and 30% perfluorotripropylamine, C9F21N)]. RESULTS: After administration of high doses, all perfluorochemicals led to cytoskeleton alteration. This alteration, expressed as retardation of the relaxation period of ferromagnetic iron oxide particles, was most pronounced after administration of Fluosol-DA. CONCLUSION: The compromising effect of perfluorochemicals is dose dependent and differs among the three compounds tested, with Fluosol-DA showing the greatest decrease in cytoplasmic motility.

[Larger sclerotomies for use of the multiport illumination system do not increase the complication risk of vitrectomy].

PURPOSE: To free both of the surgeon's hands for bimanual work in the vitreous cavity, Koch and coworkers developed the multiport illumination system, which, however, requires enlarged sclerotomies. PATIENTS AND METHODS: To evaluate the safety of the multiport illumination system we analyzed the first postoperative year of those 104 vitrectomies, that were performed using this system between January and December 1991. It was used only for patients suffering from advanced vitreoretinal changes that most likely require bimanual surgery. RESULTS: No specific intraoperative or postoperative complications were encountered that might possibly have been caused by the multiport illumination system. Postoperative fibrinous reactions were noted in the vitreous cavity of 7 patients, and in the anterior chamber of 15 patients. Rebleeding was observed in a total of 17 patients. CONCLUSION: When these complications were compared to data from other retrospective studies of vitrectomy and its complications, the multiport illumination, system was found to be as safe as conventional devices.

Familial adenomatous polyposis: desmoid tumours and lack of ophthalmic lesions (CHRPE) associated with APC mutations beyond codon 1444.

An earlier study has shown that FAP patients with mutations in codons 136-302 of the APC gene do not develop congenital hypertrophy of the retinal pigment epithelium (CHRPE), whereas those with mutations in codons 463-1387 regularly do. Here we present data on 36 patients from 20 families with mutations in codons 1445-1578. These patients lack CHRPE. Furthermore, with the exception of three prepubertal children all patients with mutations in codons 1445-1578 developed desmoid tumours. This relationship between certain extracolonic manifestations and site of the APC mutation points to a specific role of the APC protein in different tissues.

Macular damage following lightning strikes.

Two men with recent history of lightining strike were referred to our hospital. Both patients complained of metamorphosia in one eye and reduced visual acuity. Funduscopy revealed target-like alterations at the fovea. Fluorescein angiography showed window defects of the central retinal pigment epithelium in both patients. One patient developed an anterior subcapsular cataract. If the eye is part of the current-circuit, the melanin granules of the iris, pigment epithelium, and choroid might act as a resistor. The resulting accumulation of heat may lead to damage of the surrounding tissues.

Indicators of oxidative tissue damage and inflammatory activity in epiretinal membranes of proliferative diabetic retinopathy, proliferative vitreoretinopathy and macular pucker.

PURPOSE: To evaluate whether oxidative tissue damage and inflammatory reactions occur in epiretinal membranes of eyes suffering from proliferative diabetic retinopathy (PDR), proliferative vitreoretinopathy (PVR), and macular pucker (MP). METHODS: Epiretinal membranes were removed during surgery and frozen at -80 degrees C (PDR, n = 26; PVR, n = 24; MP, n = 15), and oxidative tissue damage and inflammatory activity (MPO) of the membrane tissue were determined. The values are expressed as means of thiobarbituric acid-reactive substances (TBARS, nmol/mg) and MPO (units/mg). RESULTS: Both TBARS and MPO activity were significantly elevated (P < 0.05) in membranes of eyes suffering from PVR and PDR compared to MP. The myeloperoxidase activity in PDR membranes was significantly increased (P < 0.05) compared to PVR. CONCLUSIONS: Inflammatory cells and oxidative metabolites lead to oxidative reactions in PDR and PVR. The proliferation of membrane tissue may be enhanced through products of the lipoxygenase pathway, which is active in inflammatory cells and leads to oxidative tissue damage. Chemoattraction of leukocytes by oxidative metabolites renders the process self-progagating. Since patients suffering from MP have an intact vitreous body with an intact antioxidative system, this reaction occurs to a lesser degree, resulting in less membrane-growth activity. The cell-growth-enhancing properties of inflammatory cells should be further investigated in PDR and PVR with regard to new therapeutic interventions.

Ultrasound biomicroscopy for examination of the sclerotomy site after pars plana vitrectomy.

PURPOSE/METHODS: The pars plana sclerotomy has been considered to be a potential site for complications. Ultrasound biomicroscopy was used for in vivo examination of the entry sites in 20 eyes of 19 patients after vitrectomy. RESULTS/CONCLUSIONS: In all patients the entry sites were identified by ultrasound biomicroscopy. Extensive membranes at the sclerotomy were found in eyes treated for complications after vitrectomy. Ultrasound biomicroscopy allows in vivo examination of the pars plana sclerotomy and gives further insight into postoperative complications.

Increased lipid peroxide level and myeloperoxidase activity in the vitreous of patients suffering from proliferative vitreoretinopathy.

BACKGROUND: Retinal pigment epithelium cells and activated phagocytes are believed to be involved in the pathogenesis of proliferative vitreoretinopathy (PVR). Both cell types are capable of producing oxygen free radicals and other molecules with a high oxidative potential which can lead to a propagation of oxidative damage. It was the aim of this study to investigate whether products of oxidative reactions are detectable in the vitreous body of patients suffering from PVR. METHODS: In vitreous aspirates of patients vitrectomized because of PVR (n = 27), macular pucker (n = 9), or other reasons (controls, n = 31), the following parameters were determined: lipid peroxides (LPO), determined as malondialdehyde-like substances (MDA) and as thiobarbituric acid-reactive substances (TBARS), and myeloperoxidase activity (MPO). RESULTS: Compared with the controls, both LPO levels and MPO activities were significantly elevated in the vitreous of patients suffering from PVR. Vitreous of patients with macular pucker did not reveal any significant differences from controls in the parameters analyzed. CONCLUSION: Our results suggest that both oxygen free radicals and inflammation-related reactions participate in the process of PVR. Oxidative tissue damage is obviously not involved in the pathogenesis of macular pucker.

No cataract two years after pneumatic retinopexy.

PURPOSE: This study was conducted to examine possible adverse effects on lens transparency caused by pneumatic retinopexy using sulfur hexafluoride gas. METHODS: In a prospective study, both lenses of 33 patients undergoing pneumatic retinopexy in one eye were monitored up to 24 months postoperatively by slit-lamp examination and Scheimpflug photography. RESULTS: Pneumatic retinopexy does not cause significant lens opacifications up to 2 years after surgery. CONCLUSION: Temporary gas tamponade for pneumatic retinopexy is not associated with permanent changes in lens transparency.

Increased lipid peroxides and inflammatory parameters in the retina adjacent to choroidal melanoma.

Oxidative tissue damage and inflammatory reaction were investigated in the neurosensory retina of five eyes with malignant choroidal melanoma and correlated to the distance from the tumour. The results showed elevated levels of both lipid peroxides (expressed as thiobarbituric acid reactive substances) and myeloperoxidase in the retina adjacent to the tumour. The values declined with distance from the tumour. The results indicate that production of oxygen free radicals contributes to tumour associated retinopathy.

Free radical scavenging and antioxidant activity of allopurinol and oxypurinol in experimental lens-induced uveitis.

PURPOSE: In addition to the inhibition of xanthine oxidase, allopurinol is known to act, dependent on the dose, as a free radical scavenger, an antioxidant, and a "scavenger" of hypochlorous acid. This activity was investigated using a model of lens-induced uveitis. METHODS: Lipid peroxides (LPO) were determined in aqueous humor and in retinal tissue. Reduced and oxidized glutathione (GSH and GSSG) of the aqueous humor and myeloperoxidase (MPO) activity in the iris-ciliary body complex were analyzed. Allopurinol and oxypurinol concentrations were determined by high-performance liquid chromatography in aqueous humor and retinal tissue of both control eyes and eyes with uveitis. These measurements were performed 6 hours after intravenous application of allopurinol. RESULTS: In lens-induced uveitis, LPO are significantly elevated, GSH is reduced, and GSSG and MPO are increased. A xanthine oxidase inhibition dose (< 10 mg/kg body weight) of allopurinol showed no effects on oxidative tissue damage in the model used in this study. Higher doses, however, were able to reduce the oxidative damage. Allopurinol (20 mg/kg body weight) had slight effects on GSH and GSSG. All parameters improved using a dose of 50 mg/kg body weight; a dose of 100 mg/kg body weight only showed additional improvement in GSH and GSSG. There was no further change in the other parameters. Allopurinol and oxypurinol concentrations in aqueous humor and retinal tissue showed a dose dependency reaching scavenger concentrations after application of 50 mg/kg body weight of allopurinol. CONCLUSIONS: These results suggest that the xanthine oxidase mechanism plays a minor role in the oxidative tissue damage due to lens-induced uveitis. Free radicals and oxidants are generated by activated leukocytes; therefore, the effect of higher doses of allopurinol is due to its free radical scavenging and antioxidative activity.

Solitary retinal granuloma as a presenting sign of sarcoidosis.

A 31-year-old white patient was referred with a recent history of decreased visual acuity in his left eye. A raised, greyish-white lesion close to the optic nerve was the only ophthalmological finding. Upon systemic work-up a chest X-ray revealed enlarged pulmonary lymph nodes. A biopsy obtained during bronchoscopy led to the diagnosis of sarcoidosis. No other manifestation was noted. Following systemic steroid treatment the lesion regressed and visual acuity improved. Granulomatous inflammatory diseases have to be ruled out in cases of prominent greyish-white fundus lesions.

Results and prognostic factors in pneumatic retinopexy.

We analyzed 133 consecutive cases of primary retinal detachment treated by pneumatic retinopexy (PR). The postoperative follow-up period was at least 6 months (mean, 16.7 months; maximum, 36 months). In all, 97 cases (72.9%) were successfully treated by a single PR. In 19 cases (14.3%) primary reattachment was not achieved. In another 17 cases (12.8%) redetachment occurred. Redetachment was never observed later than 6 months following PR. It was caused by missed or new retinal tears in 8 eyes (6.0%) and by proliferative vitreoretinopathy in 7 eyes (5.3%). A final reattachment rate of 98.5% was achieved. Preoperative factors suggesting primary failure or redetachment were (1) extensive retinal detachment (P < 0.01), (2) pseudo- or aphakia (P = 0.01), (3) the retinal status of the fellow eye (P < 0.05), and (4) poor visual acuity in cases of attached macula (P < 0.01). In cases with preoperatively detached macula, visual acuity (VA) recovered considerably better following PR than following conventional buckling procedures. As compared with cases of successful PR, recovery of VA in all eyes requiring reoperation was significantly reduced. It was, however, still equivalent to the VA of eyes treated by scleral buckling. Surgical success is influenced by preoperative factors such as pseudo- or aphakia, extensive retinal detachment, poor initial VA in cases of attached macula, or the state of the fellow eye. These factors have to be taken into consideration in decisions for or against PR. The anatomical and functional outcome proves the value of this technique in the treatment of primary rhegmatogenous retinal detachment.

Increased lipid peroxide levels and myeloperoxidase activity in the vitreous of patients suffering from proliferative diabetic retinopathy.

Lipid peroxide (LPO) levels, as determined by high-performance liquid chromatography (HPLC) and by the thiobarbituric acid (TBA) method, and myeloperoxidase (MPO) activity in vitreous of patients vitrectomized because of proliferative diabetic retinopathy were compared with LPO levels and MPO activity in vitreous of patients with no vitreoretinal proliferation. Both LPO levels and MPO activity were significantly elevated in the vitreous of patients with fibrovascular vitreoretinal proliferations secondary to diabetes. The TBA method produced higher values for LPO levels than did the HPLC method. The correlation between the two methods was 0.94. Our results suggest that both oxygen-free radicals and inflammation-related reactions can participate in the pathogenesis of diabetic retinopathy.

Refractive error and choroidal perfusion characteristics in patients with choroidal neovascularization and age-related macular degeneration.

The refractive error of 186 eyes with choroidal neovascularization (CNV) secondary to age-related macular degeneration (ARMD) was determined. It was found that the risk for the development of this form of ARMD increased with increasing degrees of hyperopia. The 96 patients afflicted with bilateral CNV were significantly older than the unilaterally affected patients (P = 0.027). Abnormal choroidal perfusion was identified in 66.4% of the fluorescein angiograms. These findings support the concept that hyperopic shortening in eyes afflicted with ARMD might cause secondary changes in the choroidal vasculature, predisposing them to choroidal neovascularization.
