Grip posture and forces during holding cylindrical objects with circular grips.

Individual finger position and external grip forces were investigated while subjects held cylindrical objects from above using circular precision grips. Healthy females (n = 11) and males (n = 15) lifted cylindrical objects of various weights (0.5, 1.0 and 2.0 kg), and varied diameters (5.0, 7.5 and 10.0 cm) using the 5-finger grip mode. The effects of 4-, 3- and 2-finger grip modes in the circular grip were also investigated. Individual finger position was nearly constant for all weights and for diameters of 5.0 and 7.5 cm. The mean angular positions for the index, middle, ring and little fingers relative to the thumb were 98 degrees, 145 degrees, 181 degrees, and 236 degrees, respectively. At the 10-cm diameter, the index and middle finger positions increased, while the ring and little finger positions decreased. There were no differences in individual finger position with regard to gender, hand dimension, or hand strength. Total grip force increased with weight, and at diameters greater or lesser than 7.5 cm. Total grip force also increased as the number of fingers used for grasping decreased. Although the contribution of the individual fingers to the total grip force changed with weight and diameter, the thumb contribution always exceeded 38% followed by the ring and little fingers, which contributed approximately 18-23% for all weights and diameters. The contribution of the index finger was always smallest (> or = 11%). There was no gender difference for any of the grip force variables. The effects of hand dimension and hand strength on the individual finger grip forces were subtle.

[A case of pneumonitis induced by PL granules].

We report a case of pneumonitis induced by PL granules. A 45-year-old man took PL granules and other drugs for fever and headache. Because he subsequently developed high grade fever, cough and diarrhea, he was admitted to our hospital. His chest X-ray film revealed multiple patchy shadows in both lung fields. Analysis of bronchoalveolar lavage fluid (BALF) disclosed a high number of cells (total), lymphocytes, and a high CD4/CD8 ratio. Microscopic examination of transbronchial lung biopsy (TBLB) specimens showed infiltration of mononuclear cells and thickening of the alveolar wall. After discontinuation of drugs, his condition (symptoms, laboratory data, and chest X-ray findings) promptly improved. Lymphocyte stimulation tests (LST) for PL granules and acetaminophen were positive and an oral challenge test with PL granules was also positive. Based on these findings, we diagnosed this as a case of pneumonitis and enteritis due to PL granules. To our knowledge, this is the first reported case of pneumonitis due to PL granules.

[Experimental study on comparative efficacy of hepatic arterial injection of anticancer agent under temporary occlusion of portal vein].

To compare the efficacy of intra-hepatic arterial infusion of anti-cancer drug under temporary occlusion of portal vein, the following two experiments were performed. 1. VX2 tumor cells (about 1 x 10(5) cells) were transplanted through the superior mesenteric vein of the rabbits, after 2 weeks, 4'-0-Tetrahydropyranyl doxorubicin (2 mg/kg, 2 ml) aqueous solution (THP) was administered into hepatic artery of rabbits with metastatic VX2 liver tumor. 2. THP was injected into hepatic artery under temporary occlusion of portal branch of median left lobe with metastatic liver. Then the THP levels of normal liver tissue and tumor of median left lobe and lateral right lobe. The THP levels of tumor under portal occlusion tended to be about 2.63-fold higher than without occlusion. These results suggest that hepatic arterial injection under portal occlusion is more useful to increase the levels of anticancer agents of metastatic liver tumor.

Malignant lymphoma of the stomach associated with systemic sarcoidosis.

The case of a patient with gastric malignant lymphoma associated with systemic sarcoidosis is reported. The patient's sarcoidosis was identified by the findings of bilateral hilar lymphadenopathy and secondary glaucoma. Biopsy of a cervical lymph node showed epithelioid cell granuloma; the sarcoidosis was diagnosed 15 years previously. She had been treated with prednisolone for 5 years until 1979. Proximal gastrectomy, splenectomy, and lymph node dissection were done in 1989. The tumor was diagnosed as malignant lymphoma of the diffuse large cell type according to the Working Formulation. There was no infiltration of lymphoma cell in the 45 dissected lymph nodes, but sarcoid nodules were observed in all nodes. Only three cases of gastric malignant lymphoma complicating sarcoidosis have been reported to the authors' knowledge. Further study should be done in support of the concept of a sarcoidosis-lymphoma syndrome.

A new technique in endoscopic ultrasonography of the upper gastrointestinal tract.

Endoscopic ultrasonography (EUS) of the upper GI tract was unsuccessful in 42 out of 274 patients (15%) due to inaccurate guidance of the probe to small, shallow lesions, or to difficulty in clearly demonstrating the surrounding layer structure and boundary of the lesion. We applied the saline injection technique in these 42 patients. Saline was injected into the submucosal layer of the esophagogastric wall. The ultrasonographic image of adjacent saline accurately guided the probe to the lesion. Injected saline enabled the mucosal layer to be distinguished from the muscularis propria surrounding the lesion and provided clear ultrasonographic imaging of the boundary between the lesion and the surrounding layer structure. This new method was found to be effective in 38 out of 42 patients in whom EUS was unsuccessful. A study comparing the depth of tumor invasion diagnosed by the saline injection technique with histological findings obtained in 12 cases showed good correlation. These results indicate that the saline injection technique would seem to be very useful as an adjuvant to conventional EUS.

Duodenal mucosal hemodynamics in patients with liver cirrhosis.

Clinical studies show that patients with liver cirrhosis associated with portal hypertension have a high incidence of duodenal ulcer and duodenitis. However, little information is available concerning pathophysiological process of such duodenal diseases in liver cirrhosis. Hemodynamics of the duodenal mucosa was studied in cirrhotics with esophageal varices (68 cases) and in noncirrhotics with non-ulcer dyspepsia (37 cases) as well. In each group, hemoglobin concentration in the peripheral venous blood was measured, and mucosal hemodynamics was examined in 4 regions of the duodenum by endoscopic reflectance spectrophotometer. No significant intergroup difference was noted in the mean age or sex ratio. Hemoglobin concentration in the peripheral venous blood was significantly lower (p less than 0.01) in the cirrhotics. There were no significant intergroup differences in duodenal mucosal blood volume. However, the cirrhotics showed significantly lower oxygen saturation of hemoglobin in all regions of the duodenum (p less than 0.01). These results show that the cirrhotics with esophageal varices had relative increase in blood volume and decrease in oxygen saturation of hemoglobin in the duodenal mucosa. Such microcirculatory disturbances seem to predispose liver cirrhosis patients to duodenal injury.

[A primary choriocarcinoma of the stomach with a review of 45 cases in Japan].

A case of a primary choriocarcinoma of the stomach is reported together with review of 44 cases in the Japanese literature. The patient, a 79-year-old male complaining of severe anemia, was admitted to hospital. As a result of a gastric endoscopy, a carcinoma was diagnosed and a laparotomy was performed. Because of an extensive metastasis, however, the surgery was limited to a palliative proximal gastrectomy. Pathologically, the tumor was a choriocarcinoma, and the HCG in the blood was slightly elevated. Immunohistologically, however, no HCG was detected. Although most cases reported in Japan support the theory of differentiations in gastric carcinomas, this choriocarcinoma suggests a separate type of differentiation in both the function and the morphology or a morphological mimicking.
