[Malnutrition common in Swedish hospitals. Risk of complications and prolonged care increases].

The occurrence of malnutrition has been investigated at four hospitals and the underlaying factors evaluated. The results suggest that malnutrition is still a manifest clinical problem at Swedish hospitals, and that it is associated with an increased risk of complications and prolonged care. During hospitalisation a negative nitrogen and energy balance is observed. It is important to identify patients at risk, as the prevention of undernourishment would appear to be a more effective approach than the treatment of established malnutrition.

Comparison between the lactulose/mannitol and 51Cr-ethylenediaminetetraacetic acid/14C-mannitol methods for intestinal permeability. Frequency distribution pattern and variability of markers and marker ratios in healthy subjects.

Urinary excretion of lactulose and mannitol, determined by gas-liquid chromatography, was compared with that of 51Cr-ethylenediaminetetraacetic acid (EDTA) and 14C-mannitol for measurement of intestinal permeability in 28 healthy humans. The 0- to 6-h excretion values for unlabelled and labelled mannitol (marker of transcellular permeability) were normally distributed, whereas excretion values for lactulose and 51Cr-EDTA (markers of paracellular permeability) were skewly distributed, as were the lactulose to mannitol and 51Cr-EDTA to 14C-mannitol ratios. Excretion of the transcellular markers but not of the paracellular markers was significantly correlated to urinary volume; correction for urinary volume resulted in decreased test variability. Significant correlation was found between lactulose and 51Cr-EDTA excretion (p < 0.01) and between mannitol and 14C-mannitol excretion (p < 0.001) but not between the lactulose to mannitol and 51Cr-EDTA to 14C-mannitol ratios (p = 0.11). Inter- and intraindividual test variability was greater for each chemically determined marker than for the corresponding isotope-labelled marker. Similarly, variability was greater for each paracellular marker than for the corresponding transcellular marker and for each paracellular/transcellular marker ratio than for the transcellular marker alone. Variability of mannitol excretion was increased by the frequent presence of food-derived mannitol in the urine.

Bacterial translocation after non-lethal hemorrhage in the rat.

Translocation of enteric bacteria has been suggested to compromise patients in severe catabolic stress. Mechanisms for this route of infection are not known. In this study, rats were subjected to hemorrhage without reinfusion during 60 min, total blood loss was 3.28 +/- 0.14 ml/100 g BW. Control groups consisted of sham-operated animals without bleeding, and rats not operated at all. The mean number of viable bacteria found in mesenteric lymph nodes (MLN) of bled animals was 168 +/- 45 colony forming units (c.f.u./MLN), significantly higher compared to sham operated (5 +/- 3 c.f.u./MLN) and not operated (0 +/- 0 c.f.u./MLN) controls (P < 0.01). Cultures from MLN were positive in 7/9 rats after bleeding, in 3/9 of sham operated, and in 0/6 of non-instrumented control animals. No positive blood cultures were isolated. Escherichia coli was the dominant species found in MLN. A biochemical fingerprinting method (the PhP system) was used to identify translocating strains of E. coli among strains found in cecum. The method was also used to compare translocating strains between different animals. Our findings reveal that bacteria translocate to MLN after hemorrhage. Some phenotypes of E. coli strains translocate more frequently than others, suggesting that they have properties facilitating translocation.

Colectomy-proctomucosectomy with S-pouch: operative procedures, complications, and functional outcome in 69 consecutive patients.

Sixty-nine patients were operated upon in a three-stage procedure. Early complications occurred in 29 percent after colectomy-ileostomy, in 25 percent after proctomucosectomy with ileoanal anastomosis and loop ileostomy, and in 9 percent after closure of loop ileostomy. Only three of these were considered serious. Seventy-one percent of the patients were readmitted into the hospital between the three operations or after the last one. Total hospital stay was 49 days (median); the range was 20 to 345 days. Reconstruction of the reservoir was performed in four patients owing to defecation problems, with satisfying functional results in two patients, while two emptied by catheter. There was no postoperative mortality or pelvic sepsis, and no pouches were excised. Ileostomy was re-established in two patients. At histopathologic re-evaluation of colectomy specimens, the diagnosis was changed from ulcerative colitis to Crohn's disease in three patients and to indeterminate colitis in five. Median follow-up was 4.3 years. Continent anal defecation without ileostomy was achieved in 67 patients (97 percent), with 4.1 bowel movements per day and 0.6 per night. Perfect continence was achieved in 55 percent in the daytime and in 43 percent at night. The low rate of reservoir-threatening complications is attributed to the three-stage procedure and the technical details in the surgical procedures.

Non-traumatic rupture of the iliac vein.

Two cases of non-traumatic rupture of the iliac veins have invited a review of eight previous cases reported in the literature. The condition is worthwhile reviewing as the clinical signs and findings very much resemble the picture of a ruptured aneurysm including sudden or fairly rapid onset of hypovolaemia and a tender, lower abdominal mass. In addition, concomitant iliac vein thrombosis is a common finding which is likely to contribute to rupture. The management is resuscitation and repair of the vein.

Squamous-cell carcinoma in a pilonidal sinus. Case report.

Squamous-cell carcinoma arising in a pilonidal sinus is a rare occasion. The present case stresses the importance of recognizing the condition. Malignancies of this location often has a more aggressive clinical course than squamous-cell carcinoma of other skin locations.
