Pharmacokinetics of delayed (DR) versus sustained release (SR) theophylline formulations following single (SD) and repetitive dosing (RD).

The present study compares the pharmacokinetic characteristics of equivalent theophylline doses of a delayed release formulation of oxtriphylline and a sustained release formulation given as single or six hourly repetitive dosings in six adult asthmatics. Both formulations maintained serum theophylline levels within therapeutic range for eight hours following six hourly repetitive dosings. The same dose of theophylline given as a single dose or after reaching steady states does not produce comparable serum levels. Effective and safe theophylline therapy in the management of chronic asthma not only requires frequent monitoring of serum theophylline levels but also requires clear understanding of the formulation used, the time and the clinical condition under which sampling is done.

Comparison of cromoglycate (cromolyn) and theophylline in controlling symptoms of chronic asthma. A collaborative study.

28 children with chronic asthma (15 in Denver and 13 in London) completed a 12 wk double-blind trial of treatment with sodium cromoglycate (cromolyn sodium), theophylline, and a combination of both. The three regimens were administered, each for 4 wk, in random sequence as part of a collaborative investigation of the relative efficacy of the two antiasthmatic agents. Cromoglycate was administered by inhalation in standard doses of 20 mg q.i.d. Theophylline dosage was individualized with the assistance of serum-theophylline measurements and averaged 6 mg/kg/dose q.i.d. (range 3-8--8-5 mg/kg/dose). Peak expiratory-flow rates measured twice daily on all patients averaged 75% of that predicted during cromoglycate administration, 79% during theophylline, and 81% during the combined-drug regimen (P less than 0.05). Patients had an average of 59% of days free of symptoms while on cromoglycate and 71% of days symptom-free when on both the theophylline and the combination regimens (P less than 0.025). None of the 13 patients whose asthmatic symptoms were previously controlled with cromoglycate was unable to complete the 4 wk trial with theophylline alone; 1 patient whose symptoms had been previously controlled with theophylline twice developed severe asthmatic symptoms while receiving cromoglycate, and he had to be withdrawn from that study period. No significant differences in adverse effects of the medication were observed during the 12 wk trial.

Cytology of diffuse mesothelioma in the thorax of a horse.

Examination of effusions from body cavities must include a search for neoplastic as well as inflammatory cells. Neoplastic cells found in the thorax are generally derived from adenocarcinomas or mesotheliomas. Mesotheliomas are relatively uncommon and the neoplastic cells found in effusions are difficult to differentiate from activated cells in inflammatory effusions. An ante-mortem diagnosis of mesothelioma was made in a mare on the basis of the large volume of fluid produced, the pleomorphic mesothelial cells, the polyp formation and the absence of PAS staining material after digestion of the cell impression with maltase diastase.

Repigmentation of vitiliginous skin by cultured cells.

Epidermal cells from pigmented areas of a patient with vitiligo were cultured in MCDB-153 medium, which supports the clonal growth of undifferentiated keratinocytes and melanocytes. The cells were grown on collagen-coated substrata. After the cells reached semiconfluence, the composite of substratum and cells was emplaced onto dermabraded vitiliginous areas as a graft. Re-epithelialization of the grafted areas was complete after 2 weeks. Repigmentation was evident after 1 month and continued over the observation period of several months. There was complete and normal differentiation of the graft, including a normal distribution of melanocytes in the basal layer. Ultrastructural studies showed a normal distribution of melanosomes in the melanocytes and showed keratinocytes that were indistinguishable from the uninvolved skin.
