[Non-invasive methods of respiratory monitoring during resuscitation in neonatal and pediatric care].

The authors review the non-invasive methods currently used in paediatric intensive care units to monitor the oxygenation and ventilation of neonates and infants. They deal successively with the qualities and defects technique (transcutaneous measurement of PO2, SaPO2 and PCO2) and with the usefulness of capnography. These various methods of investigation and monitoring must complete each other to ensure the safety of children in situations of distress.

Study of cost by type of diagnosis in Switzerland.

Lengths of stay, for a given diagnosis, are poorly correlated between Switzerland, the USA., and Australia; and between 5 hospitals within Switzerland. Three different sets of DRG cost weightings (U.S., French A, French B) are also poorly correlated. Hospital operating efficiencies, standardized by DRG, were estimated and compared. The three sets of cost weightings used all yielded very similar estimates of hospital operating efficiency. The use of current existing systems of cost weightings may, therefore, provide a rapid and cheap means of comparing hospital operating efficiency. In the long run, more precise and detailed cost data will enable national cost weightings to be obtained. Associated issues, such as costing methodologies and perspectives on utilization, are also discussed.

[Facial cellulitis and Haemophilus meningitis in infants].

Two cases of facial cellulitis due to Haemophilus influenzae serotype b are reported in 2 infants aged 3 and 6 months. Bacteriological diagnosis relied on blood cultures. Association with bacterial meningitis emphasizes the necessity of a systematic cerebrospinal fluid examination and the choice of antibiotherapy.

[Human cardiac transplantation. Apropos of a case followed for more than 6 years].

A cardiac transplantation was performed on November 28, 1968. This was the only one undertaken at Marseilles. After a survival period, now exceeding 6 years, the patient has had an almost normal activity. The course of this patient, the follow-up elements, the incidents and complications during this period were analysed. It was insisted upon the interest of following up the coagulation tests for the preclinical detection of rejection or menacing rejection. Practically uninterrupted heparin treatment during the 6 years might have been an element for this prolonged survival.
