[Examination to possibilities of a physiological fitting of cochlear implants].

BACKGROUND: Cochlear implants (CI) convert acoustic events into electrical pulses. The auditory nerve picks these tiny electrical pulses up and sends them to the brain. The dynamics of the audible sound is compressed considerably. The limits for stimulation are determined with the patient. A map law determines which sound pressure level is assigned to which stimulation level. A sufficient speech understanding requests an allocation of high stimulation levels for weak sound signals. The higher the sound level, the lower the increase. Unfortunately, with such kind of map law unwanted background noise is also presented as well audible stimulation. These stimuli are often annoying to CI users in everyday situations. PATIENTS AND METHOD: The possibility to give an s-shaped course to these map laws was examined in 9 patients. After the fitting procedure their speech understanding were tested. The results were compared with the results of former tests. RESULTS: 8 patients reported definite improvement of their hearing situation. Such map laws seem, therefore, suitable to optimise speech processor programming.

Normal voice production: computation of driving parameters from endoscopic digital high speed images.

OBJECTIVES: A central point for quantitative evaluation of pathological and healthy voices is the analysis of vocal fold oscillations. By means of digital High Speed Glottography (HGG), vocal fold oscillations can be recorded in real time. Recently, a numerical inversion procedure was developed that allows the extraction of physiological parameters from digital high speed videos and a classification of voice disorders. The aim of this work was to validate the inversion procedure and to investigate the applicability to normal voices. METHODS: High speed recordings were performed during phonation within a group of five female and five male persons with normal voices. By using knowledge based image processing algorithms, motion curves of the vocal folds were extracted at three different positions (dorsal, medial, ventral). These curves were used to obtain physiological voice parameters, and in particular the degree of symmetry of the vocal folds based upon a biomechanical model of the vocal folds. RESULTS: The highest degree of symmetry was observed for the medial motion curves. While the dorsally and ventrally extracted motion curves exhibited similar results concerning the degree of symmetry the performance of the algorithm was less stable. CONCLUSIONS: The inversion algorithm provides reasonable results for all subjects when applied to the medial motion curves. However, for dorsal and ventral motion curves, correct performance is reduced to 85%.

Toxic epidermal necrolysis: differential diagnosis of an epidermolytic dermopathy in a hematopoietic stem cell transplant recipient.

Toxic epidermal necrolysis (TEN) is an epidermolytic skin disorder associated with drug administration. It is associated with an erythematous rash with full thickness epidermal loss and characteristic histology. A 35-year-old woman underwent allogeneic hematopoietic stem cell transplantation (HSCT) for severe aplastic anemia (SAA). She developed an acute epidermolytic rash and TEN was diagnosed on the basis of skin biopsy. In the HSCT setting, TEN should be thought of as an important differential diagnosis of epidermolytic dermopathies. The most distinctive diagnostic test in the differential diagnosis of these disorders is skin biopsy

The thermodynamic control of protein binding to lipid bilayers for protein chromatography.

A new concept of charge-selective bioseparation with certain advantages over the established ion-exchange technique is reported. The procedure is based on the temperature-controlled creation and dispersion of domain structures in single phospholipid bilayers by means of the phase transition between the gel phase and the fluid phase of the bilayer. The bilayers are presented on a solid support of silica gel. Rather than altering the ionic strength of the buffer, protein elution is accomplished by a change of the chromatography column temperature. The application of temperature gradients improves the protein selectivity of phase transition chromatography.

The influence of Microgynon and Diane-35, two sub-fifty ovulation inhibitors, on voice function in women.

The early days of oral contraceptives showed some evidence that these medications may have caused undesirable side effects on the voice, mainly in terms of virilization. In a random study carried out at the university hospitals of Jena and Berlin (Charité), two more recent drugs were tested in this regard, one containing cyproterone acetate (Diane-35) and the other one levonorgestrel (Microgynon), both from Schering. Ninety-one patients took part in extensive clinical and instrumental phoniatric investigations of voice function over a period of one year. No significant side effects on the voice could be proven for the two preparations. Gynecological and clinical effects of both Diane-35 and Microgynon were similar during the one-year study period. Women taking Diane-35 had less intracyclic bleedings and amenorrhea. Also, acne was more favorably influenced by Diane-35.
