[Symptomatic urinary tract infection of the female--diagnostics].

The basis for the diagnostic work-up of recurrent cystitis is formed by taking a precise medical history against the background of knowledge of the pathogenesis of urinary tract infections. The anamnesis should also focus on factors that influence the natural flora (sexual intercourse, hygiene) but additionally include preceding antibiotic treatment and diseases that affect the immune status (diabetes mellitus). Urinalysis is the principal examination among the laboratory diagnostic procedures. The diagnosis is promptly confirmed by immediate analysis of a clean catch midstream urine sample using a counting chamber or a test strip. As a matter of principle, microbiological diagnosis always ensues. Extended diagnostic work-up (urological staging) is aimed at detecting functional and anatomic abnormalities. While these factors only play a subordinate role during the premenopausal phase, they gain in importance during the postmenopausal phase. A key role is also attributed to local estrogen deficiency.

Quantitative analysis of the velopharyngeal sphincter function during speech.

OBJECTIVE: This study reports a new concept for quantitative analysis of velopharyngeal function using electromagnetic articulography and simultaneous transoral video endoscopy. PARTICIPANTS: The velopharyngeal closure mechanism was studied in 16 healthy German subjects during production of the vowel-consonant-vowel sequences /afa/ and /apa/. DESIGN: Velar movements in the sagittal and vertical direction were measured by electromagnetic articulography (EMA) while video endoscopy was used for recording in the sagittal and transverse directions. MAIN OUTCOME MEASURE: A transverse sagittal quotient (TSQ) was defined after the measurement of sagittal and transverse diameters of the velopharyngeal port (VPP) in the video-endoscopic pictures on the basis of a frame-by-frame analysis. RESULTS: The sphincter morphology was objectively determined with the TSQ base value. A terminal sphincter deformation (TSD) for closure phases of articulatory movement was revealed by a qualitative shape analysis of the TSQ curves. CONCLUSIONS: The assessment of the TSQ of velopharyngeal port diameters and the TSD in the terminal closure period lead to a new dynamic interpretation of velopharyngeal closure patterns.

Angioarchitecture of the human spermatic cord.

Human spermatic cords were investigated by means of cast preparations, light-microscopic examination and computer-aided 3-dimensional reconstructions from serial sections of paraffin-embedded material. After leaving the testis, the testicular veins formed two principal groups co-existing side by side. Numerous veno-venous anastomoses could be observed within each individual group, whereas only a few mutual intergroup anastomoses were found. The testicular artery ran within one group but showed no close topographical relationship to the other group. Light microscopy of the serial sections revealed that the group of veins with no close topographical relationship to the testicular artery ran at a distance of several centimeters embedded within fatty tissue. With the help of computer-aided 3-dimensional reconstructions, a spatial picture of the vascular organization was obtained. These results allowed the following classification of the veins of the pampiniform plexus. Group-I veins formed a tight plexus around the testicular artery by means of veno-venous anastomoses. Group-II veins formed veno-venous anastomoses between each other and ran over at a sizeable distance embedded in fatty tissue but showed no close topographical relationship to the testicular artery. Group-III vessels formed veno-venous anastomoses between group-I and group-II. Group IV veins formed arterio-venous anastomoses with the testicular artery. Based on the differences in wall structure and diameter, a subclassification in group-I and group-II was undertaken. This organization of the veins of the human pampiniform plexus should further the understanding of physiological processes, such as the transfer of hormones and other substances from the veins to the testicular artery and vice versa. It should also facilitate the tracing of the veins during antegrade sclerosing.

[Antegrade scrotal phlebography of the v. spermatica in left-sided testicular varicocele--are retrograde phlebographic classifications suitable for typing?].

PURPOSE: The study was conducted to clarify whether the use of classifications in base of retrograde phlebography would be sufficiently suitable for typifying the findings of antegrade phlebography examinations of the internal spermatic vein with regard to antegrade sclerotherapy. SUBJECTS AND METHODS: In course of antegrade sclerotherapy in 236 patients with previously untreated idiopathic left-sided varicocele, phlebography via scrotal approach was performed. The results were compared with the relevant findings of the pre-existing classifications of the retrograde exams. RESULTS: Comparison with the classifications of Bähren and Braedel showed distinct discrepancies. In 28.8% (Bähren) respectively 36.4% (Braedel) of our findings, typification was unsuccessful. Correspondence was found with the Riedl-classification. Collateral vessels however, are not sufficiently differentiated. CONCLUSION: Retrograde classifications proved insufficient in typifying the findings of antegrade phlebography of the internal spermatic vein. A more subtle classification is required. This has been worked out and presented.

[The physician as manager--medical department coordination as a reaction to changed responsibilities].

As a reaction to the changing situation in the public health service, we have changed a senior house officer post into a job for medical management out off our own motivation. In the sense of job sharing, the department has now got an expert with medical competence for the new business. He is the connecting link between the department and the administration of the hospital.

[Vascular organization of the pampiniformis plexus in the man and its significance for antegrade sclerosing of testicular varicocele].

The vascular organization of human spermatic cord was analyzed by means of injection and by casting preparation of the testicular artery and veins of the pampiniform plexus after orchiectomy. After leaving the human testis, the testicular veins are organized in two main groups and form two vein plexus. One of these vein groups forms a tight plexus around the testicular artery. Computer-aided 3-D reconstructions based on the paraffin serial sections of the human spermatic cord allowed us to demonstrate the relation between the blood vessels and the other tissue structures of the spermatic cord. The results obtained allowed us to subdivide the human spermatic cord into three compartments. The dorsomedial compartment contains the vas deferens and its blood vessels. The middle compartment contains the testicular artery and the vein group which forms a tight plexus around this artery. The ventrolateral compartment contains a vein group which shows no topographic relation to the testicular artery. This vein group is embedded in a large, macroscopically visible volume of fatty tissue. To be sure of sparing the testicular artery during antegrade sclerotherapy this vein group should be prepared.

Simultaneous evaluation of articulatory velopharyngeal function under different dynamic conditions with EMA and videoendoscopy.

Objective examination of velopharyngeal (VP) sphincter function has been described based on multiple methods. Videofluoroscopy and videoendoscopy represent today's clinical standard. A new concept of analysis of VP functioning is described using electromagnetic articulography (EMA) for midsagittal measurement of velar movement and videoendoscopy in a setting for simultaneous recording. Multidimensional analysis of VP functions using this setting is demonstrated for a normal subject. Timing aspects of VP closure and a transverse-sagittal ratio of VP port diameter are reported under different conditions of speech dynamics.

Midsagittal velar kinematics during production of VCV sequences.

We present data on velar speech movements examined by electromagnetic articulography (EMA), a tool for the study of velar kinematics. Based on investigations on 22 normal German subjects (15 male, 7 female), movement paths of the velum using a sensor positioned on the midsagittal anterior surface were recorded using a series of high and low vowel VCV utterances. Maximum velar elevation (mean = 13.5 +/- 0.6 mm [SD]) with the rest position as a reference was observed during the production /iti/. The average maximum articulatory velocity of velar movements during /iti/ production was 141.0 mm/sec. Maximum movement of the velum observed during production of /ana/ averaged 7.5 +/- 1.1 mm. The direction of velar movement ranged between 44.6 degrees and 50.4 degrees relative to the bite plane registered electromagnetically. The influence of loudness variation of sequence production is demonstrated and discussed with the literature.

["Contra laparoscopy" position in treatment of testicular varicocele].

Laparoscopic varicocelectomy does not offer any significant advantage in comparison to sclerotherapy or open surgical procedures. Invasiveness (transperitoneal approach, intraoperative complications, general anesthesia, duration of operation, possible ligation of the spermatic artery) is not less, but the technical expenditure and costs are higher than those in competing procedures. Laparoscopic treatment is not the procedure of first choice and even in recurrent varicocele it should only be performed if prior treatment by-repeated antegrade sclerotherapy is not successful and distal reflux has been ruled out. Therefore, the laparoscopic approach is of only minor value in the treatment of varicocele testis.

Extracorporeal piezoelectric shock wave lithotripsy as mono and multiple therapy of large renal calculi including staghorn stones in unanaesthetized patients under semi-ambulant conditions.

OBJECTIVE: To examine the effectiveness and complication rate of extracorporeal piezoelectric shock wave lithotripsy (EPL) as monotherapy for the treatment of large renal calculi. PATIENTS AND METHODS: Treatment consisted of semiambulant EPL as monotherapy combined with protective ureteric stenting in 84 patients. Three groups were established according to stone volume (borderline-stone, partial and complete staghorn). An average of 18,230 shock waves was applied in 3.76 sessions to each patient. RESULTS: No major complications were observed but there were minor complications in 33% of patients. Invasive auxiliary methods were necessary in only 12%. After a mean follow-up of 9 months (n = 72), 54% of patients were stone-free and in 22% the fragments passed spontaneously. Thus the success rate was 76%. Analysis of the three groups showed no difference in stone status during follow-up. There was no relation between the outcome of treatment and stone volume. Analysis of the patients who failed to respond to treatment showed that they were treated for longer than the stone-free group, with significantly fewer sessions per week (0.71 vs 1.28 sessions/week). It appears that higher rates of stone removal can be achieved with more frequent treatment sessions at shorter intervals. CONCLUSION: We consider semi-ambulant EPL monotherapy to be a minimally invasive alternative treatment in patients with large renal calculi, including staghorn stones.

Monoclonal antibody to the prostate specific antigen.

Somatic cell hybrids were made from mouse myeloma cells and spleen cells derived from BALB/c mice immunized with homogenized epithelial fractions of BPH. The screening by immunoperoxidase staining on human prostate and non-prostate tissue resulted in one monoclonal antibody identifying a prostate specific antigen. Upon SDS-PAGE and Western blot this antigen exhibited a single band at the position of 34 kDa molecular weight. The immunoreactivity of the prostate antigen was found to be localized exclusively in the epithelial lining of ducts and secretions of normal prostate, BPH and prostate cancer. Anti-p34 antibody reacted with an antigenic determinant on the PSA molecule cancer. Anti-p34 antibody reacted with an antigenic determinant on the PSA molecule and inhibited the binding of Anti-PSA antibody to PSA by about 80 to 90% in the RIA.

Neonatal screening for cystic fibrosis in Wisconsin.

Primary care physicians have been very cooperative in referring screened patients to the two designated CF centers in Wisconsin--the University of Wisconsin Cystic Fibrosis Center, and the center at the Medical College of Wisconsin in Milwaukee--and their help has made this study possible. By 1990, we anticipate that meaningful clinical comparisons between the screened and control groups will be possible, and at that time we can begin to obtain some definitive answers concerning the benefits and potential risks of neonatal screening for cystic fibrosis. At this time, it would be premature to make a decision concerning the efficacy of screening for cystic fibrosis for the State of Wisconsin. It is very important that the study go to completion before making conclusive recommendations. We are eager to meticulously document the natural history of CF by following study patients for a long time. Answers to questions concerning rate of decline of the IRT value in true positives, psychosocial risks of screening to true positives, effect on future reproductive plans, and the cost effectiveness of the screening program will not be available for at least two more years. False positive IRT results seem to be related to perinatal asphyxia. We postulate the mechanism is ischemia in the pancreas related to hypoxia during the perinatal period leading to transient release of trypsin from the pancreas into the bloodstream. Decline of the IRT result over time is of great interest because a repeat blood sampling approach would hopefully eliminate several false positives.(ABSTRACT TRUNCATED AT 250 WORDS)
