Musculoskeletal examination: a complete review.

The musculoskeletal system provides the most obvious visual clues to a person's health. Problems in the musculoskeletal system can be intrinsic muscle problems or referred from organs. The history and physical examination, which are important in assessing the health of this system, are reviewed. Dynamic examination for assessing the functioning of the system are needed and are outlined. The aging of muscles and how it affects muscle function is discussed.

A practical approach to fibromyalgia.

The term fibromyalgia refers to a collection of symptoms with no clear physiologic cause, but the symptoms together constitute a clearly recognizable and distinct pathologic entity. The diagnosis is made through the examiner's clinical observations. The differential diagnosis must include other somatic syndromes as well as disease entities, including hepatitis, hypothyroidism, diabetes mellitus, electrolyte imbalance, multiple sclerosis, and cancer. Diagnostic criteria serve as guidelines for diagnosis, not as absolute requirements. Treatment of fibromyalgia, which is an ongoing process, remains individualized, relying on a good physician-patient relationship. It is goal-oriented, directed at helping patients get restorative sleep, alleviating the somatic pains, keeping patients productive, and regulating schedules. It can be achieved through a goal-oriented agreement between patient and provider. Because fibromyalgia is chronic and may affect all areas of an individual's functioning, the physician needs to also evaluate the social support systems of patients with fibromyalgia. The approach to treatment should integrate patient education as well as non-pharmacologic and pharmacologic modalities. To keep patients well educated and involved in their healthcare, physicians should provide patients with adequate sources for reliable information.

Osteoporosis.

Human bone thickness is genetically determined and influenced by lifestyle. Deterioration of bones affects the entire body's ability to function. Initially, osteoporosis is asymptomatic but may lead to bone fractures, altered mobility, and decreased functional ability. Identifying individuals at high risk for problems caused by low bone mass and evaluating bone density are key factors in fighting this major health problem. This article reviews the tests used to diagnose and follow the progression of the disease. It also discusses the benefits of vitamin D and calcium supplementation in bone formation and preservation, as well as treatment and prevention of osteoporosis.

'Ad Diction Ary' of drug users' terms.

Physicians obtain information by verbally communicating with patients. But communication is frequently impaired for varied reasons. Often, physicians and patients speak different languages, even when both use English as their vernacular. Intravenous drug users have their own way of referring to their lifestyle and habits. Without understanding the terms they use, the physician will have difficulty appreciating the course of events leading up to a disease. Terms unique to the drug culture, or terms that have a different meaning to drug users than they do to the general public are defined herein. This special lexicon is the culmination of the author's 5-year file-card collection of terms.
