Pointers to the diagnosis of gastric lymphoma on computed tomography.

The computed tomographic features of 12 patients with primary gastric non-Hodgkin's lymphoma and a further seven patients with gastric involvement by disseminated non-Hodgkin's lymphoma have been reviewed. In eight of these 19 patients there were clefts within the area of gastric wall involved by lymphoma where oral contrast medium passed to within 2 mm of the serosal surface in and amongst areas of gastric wall thickening (maximal thickness ranging from 18 to 40 mm). In three of these patients some clefts extended beyond the expected margin of the stomach to enter into, and be contained by, a composite soft tissue mass of stomach, spleen and splenic hilar nodes. Awareness of these features, which are in keeping with the known range of macroscopic pathological appearances in non-Hodgkin's lymphoma, may help the radiologist to recognize this potentially curable disease.

Expression of rat liver ketohexokinase in yeast results in fructose intolerance.

Rat liver ketohexokinase (ATP:D-fructose 1-phosphotransferase; EC 2.7.1.3) was purified to homogeneity and the molecular mass of the protein was found by mass spectrometry to be 32,800 Da. The enzyme was cleaved and the amino acid sequences of seven peptides, comprising 24% of the total sequence, were determined. This sequence information was used to design oligonucleotide primers for a PCR using rat liver single-stranded cDNA as a template. The 224 bp PCR product was used as a probe to screen a rat liver cDNA library. A cDNA sequence of 1342 bp was obtained from three positive clones. This contained the entire coding region for ketohexokinase, and all seven peptides were identified in the predicted amino acid sequence. When ketohexokinase was expressed in Saccharomyces cerevisiae using the yeast expression vector pMA91, the cells became intolerant of the presence of fructose in their growth media. The growth of an exponential-phase culture was completely arrested within 90 min by the addition of fructose to a final concentration as low as 0.1% (w/v). This response is associated with an accumulation of fructose 1-phosphate. The cDNA for ketohexokinase encodes a protein composed of 299 amino acids with a combined molecular mass of 32,728 Da. This is in close agreement with the value for the isolated protein determined by mass spectrometry. The primary structure does not show any significant homology with those of other eukaryotic hexokinases, but it contains a highly conserved region that is present in three prokaryotic phosphotransferases that have furanose substrates.

Metabolic studies of a fructose-intolerant yeast by in vivo 31P-nuclear magnetic resonance spectroscopy.

The metabolic effects of the administration of fructose to a yeast expressing the cDNA for rat liver ketohexokinase have been investigated by 31P-nuclear magnetic resonance spectroscopy. Cessation of growth suffered by the yeast on exposure to 5 and 25 mM-fructose was accompanied by a large accumulation of fructose 1-phosphate at the expense of cytoplasmic orthophosphate and nucleoside triphosphate. Shifts in resonances were consistent with a drop in cytoplasmic pH. Arresting growth with 1 mM-fructose, however, did not result in these changes, although a large accumulation of fructose 1-phosphate occurred which may have been supported by the mobilization of polyphosphates.

Pulmonary artery sarcoma mimicking pulmonary embolism.

Two cases of primary pulmonary sarcoma which were initially diagnosed as pulmonary thromboembolism are presented. The clinical and radiologic features of pulmonary sarcoma are reviewed, with special emphasis on the features which distinguish it from thromboembolism.

An unusual case of hemifacial atrophy.

A case of hemifacial atrophy in a 10-year-old boy is described. The presentation in this patient is unusual because the lesion is extremely localized, and the abnormalities of teeth in the affected region are more severe than previously recorded. Alternative diagnoses are considered, and the results of a computed tomographic scan with three-dimensional reformatting are presented.

Use of computed tomography in the percutaneous drainage of abdominal abscesses.

The technique of computed tomographic guided drainage of abdominal abscesses is simple, and is becoming increasingly available. Obvious advantages include the avoidance of general anaesthesia and major surgery. Three illustrative cases of the drainage of abscesses in different locations are presented together with a description of the technique.

Flattened fundus sign of the septate gallbladder.

The fundus of a normal gallbladder is rounded. Any deviation from this configuration should prompt a search for the nonfilled segment of a septate gallbladder. Three illustrative cases are presented in which the initial observation of a flat fundus led to further evaluation and visualization of the remaining portion of a septate gallbladder.

CT features of rounded atelectasis of the lung.

Rounded atelectasis is an uncommon but increasingly recognized benign form of peripheral lung collapse. The three patients described here had pleural-based lung masses considered initially to represent carcinoma. On computed tomography, a characteristic appearance of eight major and five minor signs of rounded atelectasis of the lung were found. In conjunction with other radiologic evaluations, these allowed the patients to be confidently managed without exploratory thoracotomy.

The use of mammography in diagnosis and screening.

Up to nine per cent of women are destined to have breast cancer. Of these 25% are likely to be occult lesions only detectable on mammography. Early detection in important. Ten year survivals of over 65% can be expected in patients without axillary node involvement, compared to 25% with nodes. Apart from biopsy, mammography is the most sensitive method of cancer detection and is therefore of considerable help to the surgeon in evaluation of breast disease.
