Radiation therapy for stage III non-small cell lung cancer: a curative treatment option.

This paper analyzes our treatment results for a selected group of Stage III non-small cell lung cancer (NSCLC) patients treated with irradiation alone. One, two and five-year survival rates were 42%, 20% and 6% respectively. Survival rates for patients with Stage IIIA and Stage IIIB disease were similar. Our results agree with the literature and confirm that 5% of selected patients with Stage III NSCLC will be disease free and potentially cured, five years after treatment with irradiation. Recurrence in the radiation field continues to be a major problem for patients with Stage III NSCLC, accounting for the initial site of failure in 40%-55% of patients. Improvements in local control will likely improve survival rates somewhat; but, because of the marked propensity for these cancers to ultimately metastasize, significant improvement in survival rates will only occur when effective systemic therapy becomes available.

Hydrogen sulfide poisoning treated with hyperbaric oxygen.

Hydrogen sulfide inhalation injury can be life threatening. The toxic gas is produced, sometimes unexpectedly, from a wide variety of sources. Because its mechanism of toxicity is similar to that of cyanide, hydrogen sulfide poisoning is commonly treated with the nitrite component of the cyanide antidote kit. In this case report, hyperbaric oxygen was successfully used to treat hydrogen sulfide intoxication. Further evaluation of hyperbaric oxygen therapy as adjunctive treatment of hydrogen sulfide poisoning is recommended.

Ophthalmoplegic migraine.

A 5-year-old girl developed severe periocular pain lasting 4 days. As the pain subsided, a right oculomotor palsy developed which slowly resolved without aberrant regeneration over a 2 1/2-month period. A classic history and normal high-resolution CT scan were essential in making the diagnosis of ophthalmoplegic migraine. This is a rare disorder classically requiring normal cerebral angiography for diagnosis. With the advent of high-resolution CT scanning, angiography may no longer be indicated in the typical case.

A new technique for securing a chest tube.

Most techniques described for securing a chest tube in position do not discuss closing the orifice in the chest wall after removal of the tube. An air tight closure of the chest wall and good approximation of the wound edges after removal of the chest tube must be done to prevent complications and obtain a cosmetically good scar. A technique for securing a chest tube is described that permits excellent approximation of the wound edges upon removal.
