Influence of mastectomy technique on sex steroid receptor analysis.

Accurate analysis of hormone receptors in breast carcinoma is critical from prognostic and therapeutic standpoints. Controversy exists over whether there is receptor decay when specimens are obtained upon completion of, rather than prior to, mastectomy. In addition, the effect of mastectomy technique on receptor concentration has not been addressed. Twenty patients with breast carcinoma had biopsy specimens taken prior to and upon completion of modified radical mastectomy. Ten had axillary dissection followed by mastectomy (Group A). The others had mobilization of the breast before axillary dissection (Group B). The estrogen receptor concentration was higher in 14 of 20 premastectomy specimens. All 10 patients in Group B had positive receptors before mastectomy; 5 were negative after mastectomy. All seven patients in Group A with positive premastectomy receptors remained so postmastectomy. We concluded that if a tumor specimen for receptor analysis is not obtained prior to modified radical mastectomy, axillary dissection should precede breast mobilization.

Preoperative barium contrast enema in patients with inguinal hernia.

Whether patients over age 40 should have a barium contrast enema (BCE) examination for possible identification of an unsuspected colon carcinoma before elective inguinal herniorrhaphy remains an unanswered question. We reviewed the medical records of all patients over age 40 who had inguinal herniorrhaphy at our institution between January 1980 and December 1984. Of 80 patients, 46 received a preoperative barium enema. One asymptomatic colon carcinoma (2.1%) was discovered. The reported incidence of asymptomatic colon cancer found by BCE in association with inguinal hernia ranges from 0 to 2.5%. We conclude that if there is a cause and effect relationship between asymptomatic colon cancer and inguinal hernia, the mechanism is as yet unknown.

The personality characteristics and family backgrounds of women alcoholics.

Alcoholic women were lower on self-esteem and higher on alienation, neuroticism, and anxiety than "normal" women. On one comparison, alcoholic women appeared somewhat higher on conscious femininity and lower on unconscious femininity, while in another analysis they did not differ from normal women. Women alcoholics were somewhat lower in self-esteem than male alcoholics, less internal in locus of control, and less sex-role traditional than men alcoholics. When respondents with high Lie scores were eliminated from the analyses, these results remained constant.

The association between diabetic retinopathy and skeletal muscle capillary basal lamina thickening corrected for the influence of age and duration of diabetes.

The association between an objective measure of diabetic retinopathy and skeletal muscle capillary basal lamina thickness was examined in a group of 30 male insulin-treated diabetic subjects, mean age (+/- SD) 44.6 +/- 13.2 yr, duration of diabetes 21.2 +/- 11.2 yr, % ideal body weight (% IBW) 106 +/- 11%. In addition, muscle capillary basal lamina width was measured in a group of 18 nondiabeitc men, mean age 40.7 +/- 16.3 yr and % IBW 118 +/- 23%. The muscle capillary width of the diabetic subjects was significantly greater than that of the nondiabetic group (P less than 0.01), but the values of the two overlapped considerably. In the diabetic group, there was a significant association of basal lamina width with age (P less than 0.01) but not with duration of diabetes. The association between extent of retinopathy and muscle capillary basal lamina width was not strong. The findings of the study do not therefore support the use of an estimate of muscle capillary basal lamina thickness as a single representative measure of diabetic microangiopathy.

Dose-related effects of central noradrenaline stimulation on behavioural arousal in rats.

The general activity of rats in an open field test situation was observed following the central administration of isotonic saline and 0.5 and 2.0 mug of noradrenaline into the locus coeruleus. A significant increase in activity was found following the 0.5 mug, but not the 2.0 mug dose. The results are consistent with other reports indicating that low doses of centrally administered noradrenaline produce behavioural arousal whereas higher doses may result in behavioural depression. In addition, these findings suggest that the noradrenergic neurons arising from the locus coeruleus may be important for the arousal status of rats.

Initial assessment and management of a patient with a gunshot wound to the femur.

As the number of violent crimes rise, the likelihood increases that the nurse will be faced with patients presenting with gunshot injuries. These wounds pose a significant challenge to the nurse working in the acute care setting. Unlike other penetrating trauma, gunshot wounds produce a myriad of associated injuries related to the mechanism of injury. Nurses must be aware of the specific trauma associated with these wounds to assess and manage the victim in an organized and systematic fashion.
