Transforming growth factor-beta 1 levels in women with prior history of gestational diabetes mellitus.

It is known that women with prior history of gestational diabetes mellitus (pGDM) feature obesity, insulin resistance and endothelial dysfunction which cause premature atherosclerosis. Transforming growth factor-beta 1 (TGF-beta1) is a key cytokine in obesity and insulin resistance and also play important roles in the development of atherosclerosis. This study was conducted to demonstrate the serum TGF-beta1 levels of people with pGDM. Thirty women with pGDM, 20 women with type 2 diabetes mellitus (T2DM) and 20 healthy women were enrolled. Serum TGF-beta1 levels of people with pGDM were found to be significantly higher than healthy controls and significantly lower than women with T2DM. TGF-beta1 levels were found to be correlated with postprandial glucose and age and inversely correlated with body mass index (BMI) and waist circumference. On multiple regression analysis postprandial glucose level, age and BMI were determined as the most important factors affecting TGF-beta1 levels. This study demonstrates elevated TGF-beta1 levels in pGDM. The inflammatory response to hyperglycemia and insulin resistance could be the major factors for the increased expression of TGF-beta1.

Cushing's syndrome due to ectopic CRH secretion by adrenal pheochromocytoma accompanied by renal infarction.

Ectopic production of corticotropin-releasing hormone (CRH) by a pheochromocytoma is an infrequent cause of Cushing's syndrome. We report the case of a 43-year-old man with Cushing's syndrome due to a CRH-producing adrenal pheochromocytoma. The patient had clinical and biochemical evidence of hypercortisolism in conjunction with high ACTH levels and non-suppressible serum cortisol levels on low-dose and high-dose dexamethasone suppression testing. In addition to these clinical features of one month's duration, the patient developed symptoms of pheochromocytoma including headache, hypertension that was resistant to conventional therapy and excessive sweating. Biochemical testing confirmed elevated 24-hour urinary catecholamines and metabolites. Abdominal CT revealed a 4.5 x 4 x 3.5 cm mass in the left adrenal gland. He underwent elective left adrenalectomy. Light microscopic and immunochemical studies revealed a pheochromocytoma that contained immunoreactive CRH and was negative for ACTH. Plasma ACTH and dexamethasone supression tests normalized after surgery. This is an unusual case of a CRH-secreting pheochromocytoma. This was complicated by renal infarction, illustrating further the complexity of Cushing's syndrome in a patient with pheochromocytoma caused by CRH hypersecretion.

A case of recurrent pancreatitis due to hyperlipidemia misdiagnosed as familial Mediterranean fever.

Familial Mediterranean fever (FMF) is prevalent among Arabic, Turkish, Armenian, and Jewish people and it must always be considered in the differential diagnosis of patients from these ethnic groups presenting with recurrent abdominal pain with fever. In cases of fever and recurrent abdominal pain, acute pancreatitis is an important clinical condition, which should be considered in the differential diagnosis. Serum amylase concentration in acute pancreatitis is usually more than three times the upper limit of normal. However, in recurrent pancreatitis secondary to hypertriglyceridemia, serum amylase levels, for reasons that are not well understood, may be normal or mildly elevated. Recurrent pancreatitis secondary to hypertriglyceridemia may thus pose a problem in the differential diagnosis and may lead to an erroneous diagnosis of FMF. Measurement of serum triglyceride along with amylase levels should be required for a suspected diagnosis. Computerized examination of the abdomen may need to be undertaken to exclude acute pancreatitis in the presence of hypertriglyceridemia since serum amylase levels may be normal or slightly elevated.

Iron, nitric oxide, and myeloperoxidase in asthmatic patients.

Plasma nitric oxide (NO), myeloperoxidase (MPO), and iron (Fe) levels were determined in bronchial asthma. The relations among these parameters in different stages of asthma were interpreted. Their association with airway inflammation observed in patients with bronchial asthma as well as the roles and the contributions to the pathological processes were evaluated. A total of 62 individuals, 32 asthmatics and 30 controls, were included into the scope of this study. Plasma nitric oxide metabolites (NOx) and MPO and Fe levels were determined by the Griess reaction, ELISA, and the automated TPTZ (2,4,6-tri[2-pyridyl]-5-triazine) method, respectively. In the asthmatic individuals, plasma NOx, MPO, and Fe concentrations were 133 +/- 13 microM, 95 +/- 20 ng/ml, and 159 +/- 20 microg/dl, respectively; in the control group these values were 82 +/- 11 microM, 62 +/- 11 ng/ml, and 96 +/- 9 microg/dl. Increased values were detected for plasma MPO (p > 0.05), NOx (p < 0.01), and Fe (p < 0.01) concentrations in asthmatic individuals. Considering the facts that NO modulates the catalytic activity of MPO and induces the expression of heme oxygenase as important contributors to the mechanisms causing free Fe release, it is concluded that elevated NOx, MPO, and Fe levels observed in the asthmatic group act in a concerted manner and appear to be involved in the pathogenesis of asthma.

Comparison of the effects of intrathecal ropivacaine, levobupivacaine, and bupivacaine for Caesarean section.

BACKGROUND: This study aimed to detect if intrathecal (i.t.) ropivacaine and levobupivacaine provided anaesthesia (satisfactory analgesia and muscular relaxation) and postoperative analgesia of similar quality to bupivacaine in patients undergoing Caesarean section. METHODS: Ninety parturients were enrolled. A combined spinal-epidural technique was used. Patients were randomly assigned to receive one of the following isobaric i.t. solutions: bupivacaine 8 mg (n=30), levobupivacaine 8 mg (n=30), or ropivacaine 12 mg (n=30), all combined with sufentanil 2.5 microg. An i.t. solution was considered effective if an upper sensory level to pinprick of T4 or above was achieved and if intraoperative epidural supplementation was not required. Sensory changes and motor changes were recorded. RESULTS: Anaesthesia was effective in 97, 80, and 87% of patients in the bupivacaine 8 mg, levobupivacaine 8 mg, and ropivacaine 12 mg groups, respectively. Bupivacaine 8 mg was associated with a significantly superior success rate to that observed in the levobupivacaine group (P<0.05). It also provided a longer duration of analgesia and motor block (P<0.05 vs levobupivacaine and ropivacaine). CONCLUSIONS: The racemic mixture of bupivacaine combined with sufentanil remains an appropriate choice when performing Caesarean sections under spinal anaesthesia.

Octreotide reduces ischaemia-reperfusion injury in the retina.

PURPOSE: To investigate the role of octreotide on retinal lipid peroxidation and histopathological changes during ischaemia/reperfusion (I/R). METHODS: Three groups of seven pigmented guinea pigs each were formed. These represented a control group, an ischaemia group and an ischaemia/octreotide group. One eye of each animal was selected for histopathological evaluation and the other for biochemical assay. Bilateral pressure-induced retinal ischaemia was instigated for 90 min and was followed by 24 hours of reperfusion. Animals in the ischaemia/octreotide and ischaemia groups received either 10 micro g/kg of octreotide or saline, repeated five times at 6-hourly intervals, with the first dose administered 15 min prior to the ischaemic insult. Retinal malondialdehyde (MDA) levels and the thickness of the retinal layers were measured. These were compared with equivalent measurements of the control group. RESULTS: The mean MDA level increased in the ischaemia group (p < 0.01) but not in the octreotide group (p > 0.05). Significant increases in the thickness of the overall retina (p < 0.01), inner retina (p < 0.05) ganglion cell layer (p < 0.01) inner plexiform layer (p < 0.01) and inner nuclear layer (p < 0.01) were observed in the ischaemia group. No significant difference in thickness was found in any of the layers in the ischaemia/octreotide group. CONCLUSION: Octreotide reduces the increases in retinal MDA levels and retinal thickness observed during I/R.

sICAM-1 as a serum marker in the diagnosis and follow-up of treatment of pulmonary tuberculosis.

OBJECTIVE: To detect the value of sICAM-1 in the diagnosis and follow-up of treatment of tuberculosis. DESIGN: sICAM-1 levels were evaluated before and after treatment in 30 patients with pulmonary tuberculosis, only before treatment in five patients with pneumonia, five with lung cancer, and five with bronchiectasis, and in 10 healthy volunteers. RESULTS: sICAM-1 levels were as follows: 436.2 +/- 194.4 ng/ml in patients with pulmonary tuberculosis, 274 +/- 32.1 ng/ml in lung cancer patients, 268 +/- 41.9 ng/ml in bronchiectasis patients, 199.6 +/- 43.1 ng/ml in pneumonia patients, and 146.5 +/- 20.2 ng/ml in healthy individuals. sICAM-1 levels of tuberculosis cases before treatment were higher than in both the healthy group and in all the other groups. The levels in the healthy group were lower than in all other groups. CONCLUSION: The cut-off point (298 ng/ml) obtained by adding the standard deviation to the mean sICAM-1 value of patients without tuberculosis had 83.3% sensitivity, 86.6% specificity and 84.4% accuracy in differentiating pulmonary tuberculosis from other pulmonary diseases. sICAM-1 can be used as an auxiliary marker in the diagnosis of pulmonary tuberculosis.

Clinical features and co-morbidity of social phobics in Turkey.

The aim of this study is to investigate the clinical features and frequency and importance of related co-morbid disorders of social phobia in a clinical sample. Eighty-seven patients meeting DSM-III-R diagnostic criteria for social phobia were studied. All patients were assessed by using a semi-structured socio-demographic form, the Structured Clinical Interview for DSM-III-R, Manual for the Structured Clinical Interview for DSM-III-R Personality Disorders, Liebowitz Social Anxiety Scale, Hamilton Rating Scale for Depression and Hamilton Rating Scale for Anxiety. Sixty-eight (78.2%) of the group were male, 19 (21.8%) were female. The ages varied between 16-58 years, with a mean of 26.2 years (SD = 8.5). Fifty-one point seven percent of the subjects were assessed as having a co-morbid axis I disorder, of which 12.6% consisted of panic disorder and 10.3% agoraphobia. An additional axis II disorder had been found in 67.8% of the subjects, and 54.0% of them had been diagnosed as having avoidant personality disorder. The frequency of co-morbid disorders in our social phobic sample is lower than most of the studies in the literature. The interface between social phobia and avoidant personality disorder needs to be studied and discussed further.

Effects of systemic octreotide, local mytomycine-C and local corticosteroids on wound-healing reaction after glaucoma surgery.

PURPOSE: To determine and compare the effectiveness of octreotide, mitomycine-C and corticosteroids on wound-healing reaction after glaucoma surgery. METHODS: A full thickness scleral trephination was carried out by the same surgeon on tour groups of six rabbits. A sponge soaked in mytomicine-C was applied subconjunctivally in group 1 before trephination. Group 2 received corticosteroid drops tid topically for 14 days. Group 3 received subcutaneous octreotide injections tid for 14 days. The control group (group 4) was not given any drug that may interfere with wound healing. All groups received gentamycine drops tid for seven days. The rabbits were Sacrificed on the fourteenth day and the trephination area with overlying conjunctiva was excised. The samples were prefixed with glutaraldehyde, dehydrated and embedded in Araldite Cy 212. Ten semithin sections stained with toluidin blue were analysed for each group. Fibroblast and macrophage counts were performed on the surgical site and subconjunctival area. RESULTS: Intensive fibroblastic activity, increased number of vessels and active macrophages were observed only in group 4. The fibroblast and macrophage densities in this group were significantly higher than the other three groups in which wound healing was modulated (p < 0.001). Mean number of fibroblasts in group 1 was also significantly less than the ones of groups 2 and 3 (p < 0.01). Macrophage densities were similar in groups 1, 2 and 3. No statistical significance was found between groups 2 and 3 by means of fibroblast and macrophage densities. CONCLUSION: Octreotide reduced wound-healing reaction in a similar fashion to corticosteroids or mitomycine-C. These initial results seem promising.

Evaluation of gingival recession in left- and right-handed adults.

Recession is the exposure of the root surface by an apical shift in the position of the gingiva. Several factors have been implicated in the etiology of gingival recession. The aim of this study was to assess the relationship between gingival recession and the hand, right or left, duration, frequency, and technique of tooth brushing in left- and right-handed adults in Erzurum, Turkey. Fifty five left-handed (35 female and 20 male) and 55 right-handed (35 female and 25 male) subjects were included in the study. The left and right hand positions of the subjects were determined by using Oldfield's Edinburg Inventory Index, and they were divided into two groups as left-handed and right-handed. The amount of gingival recession and localization, oral hygiene situations, gingival bleeding, toothbrushing duration, frequency, and techniques of subjects were evaluated. The relationship between the amount of gingival recession and localization and the subjects' hand, duration, frequency and technique of tooth brushing were examined. The data were evaluated with variance analysis and student's t test. The oral hygiene situations in right- and left-handed subjects were determined. On comparing the left-handed subjects with the right-handed ones, it was observed that the left-handed subjects had better oral hygiene than the right-handed. But, this case was not statistically significant (p > .05). In both right-handed and left-handed subjects, women had better oral hygiene than men (p < .01). The rate of gingival recession was found more in the left-handed than in the right-handed (p < .05). In the right-handed subjects, gingival recession was found in the premolar and canine regions of upper right and lower right jaw. A similar result was also observed in the left-handed subjects, because gingival recession was seen on their upper left and lower left jaw. Incisive teeth were affected at the same rate in the left-handed and right-handed. The gingival recession was seen more in maxillary jaw than in mandibular jaw in both groups. A statistically significant relationship between gingival recession and frequency duration, and technique of tooth brushing was found. While the greatest amount of gingival recession was found in horizontal scrub technique, gingival recession increased with increasing tooth brushing duration and frequency. The relationship between gingival recession and hand using in tooth brushing was determined.

Comparison of postoperative analgesic effects of preemptively used epidural ketamine and neostigmine.

STUDY OBJECTIVE: To compare the analgesic and side effects of preemptively used epidural ketamine +bupivacaine, neostigmine +bupivacaine, and bupivacaine alone on postoperative analgesia after major abdominal surgery. DESIGN: Randomized, controlled study. SETTING: Inpatient anesthesia at the department of surgery of a metropolitan hospital. PATIENTS: 30 ASA physical status I, II, and III patients scheduled for abdominal surgery. INTERVENTIONS: Group K received 1 mL (50 mg) ketamine and 5 mL (25 mg) bupivacaine epidurally, Group N received 1 mL (0.5 mg) neostigmine and 5 mL (25 mg) bupivacaine epidurally, and Group B received 1 mL saline and 5 mL (25 mg) bupivacaine epidurally 30 minutes before operation. All patients underwent anesthesia induction with thiopental and vecuronium; anesthesia was maintained with isoflorane and vecuronium. For postoperative analgesia, all patients received epidural morphine for 48 hours postoperatively. MEASUREMENTS AND MAIN RESULTS: Standard monitoring included: 48 hours of analgesic requirement, visual analog scale (VAS), mean arterial pressure (MAP), and heart rate (HR) in the 1st, 2nd, 6th, 12th, 24th, and 48th hours. Data were analyzed using Kruskall-Wallis and Mann Whitney U tests, with a p < 0.05 considered statistically significant. No significant differences were observed regarding MAP and HR among the groups during the study period. In Group N, VAS was significantly lower than Group K and Group B. The total opioid consumption in Group N was significantly lower than in Groups K and B in the first 48 hours after the operation. CONCLUSIONS: Preemptive neostigmine can be a good choice for postoperative analgesia.

Analgesic activity of Nepeta italica L.

A screening study was performed on/by essential oils of Nepeta viscida Boiss and Nepeta italica L. using tail-flick and tail immersion (52.5 degrees C) methods. N. italica samples were collected from three different localities of Turkey. Surprisingly, only one of the essential oils showed significant activity, which was blocked by naloxone, indicating the involvement of opioid receptors. This was seen only with the mechanical but not the thermal algesic stimulus, suggesting a specific activity on opioid receptors, excluding mu receptors. The same, active essential oil also exhibited a non-competitive inhibition of acetylcholine contractions of isolated rat ileum but it was inactive on the isolated rat aorta. Furthermore, a correlation between the analgesic activity and the amount of 1,8-cineole was noticed.

Effect of Octreotide on experimental corneal neovascularization.

PURPOSE: To examine the ability of subcutaneously administered Octreotide ( a long acting somatostatin analoque) to serve as an inhibitory agent for corneal neovascularization in eyes of Wistar Albino rats. METHODS: Neovascular growth into the corneas of all the animals was induced by silver nitrate cauterization. Half of the animals which were randomly selected for the Octreotide group received 30 micrograms systemic Octreotide for 7 days. The treatment was initiated on the same day as chemical cauterization. The rest of the animals (control group) received no treatment. Slit lamp and histopathologic examination of the corneas of both groups were performed at the end of the study period. RESULTS: It was observed that the corneal neovascularization and histopatologic scores of the Octreotide group were significantly lower than those of the control group (p < 0.001, p < 0.01). CONCLUSION: Systemic administration of Octreotide inhibits the corneal neovascular response in a rat model.

[Hennekam syndrome].

UNLABELLED: Hennekam syndrome is a disorder comprising intestinal lymphangiectasia, facial anomalies and moderate mental retardation. Eight cases have been previously reported. CASE REPORT: A 17-month-old girl was admitted to hospital for peripheral edema. On physical examination, she presented with a normal mental development. Facial anomalies were noted including a flat face, depressed and broad nasal bridge, puffy eye lids, mild down-slanting palpebral fissures, hypertelorism, epicanthal folds, bulbous nasal tip, small mouth, and low set ears. A simian line and haemangiomas on the arms, trunk and left limb were also noted. There was no organomegaly. Laboratory investigations showed iron deficiency anemia, hypoproteinemia, hypogammaglobulinemia and an elevated level of alpha-1 antitrypsin excreted in the feces. Endoscopic investigation and the small bowel biopsy showed findings consistent with lymphangiectasia. The patient did well on 24 hour enteral nutrition including medium-chain triglyceride rich diet and infusion of human albumin. CONCLUSION: We have aimed to remind that Hennekam syndrome should be included in differential diagnosis when intestinal lymphangiectasia are associated with facial anomalies.

Penetration of ciprofloxacin into pleural fluid.

The penetration of oral 1000 mg/day ciprofloxacin into pleural fluid is investigated in 15 patients with exudative pleural effusion. After 4 days of ciprofloxacin therapy ciprofloxacin concentrations were measured in plasma and pleural exudate simultaneously by high-performance liquid chromatography (HPLC). Mean serum ciprofloxacin level was 1.58 +/- 0.91 mg/L and mean pleural exudate concentration was 1.00 +/- 0.59 mg/L. The concentrations achieved were all above the MIC90 of the majority of Gram-positive and Gram-negative pathogens. It is concluded that ciprofloxacin penetrates well into the pleural fluid.

The influence of smoking and non-insulin-dependent diabetes mellitus on periodontal disease.

The periodontal health of smokers and non-smokers with non-insulin-dependent diabetes mellitus (NIDDM) and non-smokers with periodontitis who were not suffering from a systemic disease was assessed. The investigation was carried out on 60 adult subjects. Levels of blood glucose, glycosylated haemoglobin and immunoglobulins G, A and M were determined, together with the plaque index, gingival index, probing pocket depths and clinical attachment level. Periodontitis was more severe in smokers and non-smokers with NIDDM than non-smokers without NIDDM, and the periodontal condition (clinical attachment level, probing pocket depth and gingival bleeding) was better in non-smokers with NIDDM than smokers with NIDDM. The results suggest that diabetes and smoking are high-risk factors for periodontal disease.
