[Surface treatment of dental implants with high-energy laser beam] .

In the present study the biological value of the laser prepared titanium implants has been investigated. Screw-type implants with different surface modifications were placed into the femurs of rabbits. Removal torque measurements and histological examinations were prepared. The examinations were divided into two groups. In the first part of the examinations the laser surface treatment was compared to other type of surface modifications. In the examination groups the implants were prepared with the following surfaces: 1. machined, 2. sandblasted, 3. low intensity laser surface, 4. high intensity laser surface, 5. titanium oxide coated surface. In the second part of the examinations laser treatment with different physical variables were observed. It has been concluded, that in both cases the high energy laser treated implants needed higher removal torque and in some cases an osteogenic activity was observed around them in the medullary space as well. The authors think the advantage of laser surface treatment lies in special micromorphology and the increased cleanliness of the surface.

[The role of plastic shock absorbers in dental implantation].

The mechanical behaviour of different plastics (PE, PP, PI, PA, ABS, POM) was examined by static and dynamic loading. Detection of microdeformations and photoelastic stress analysis served as the examination method. According to the results, polyethylene is unsuitable, however the other plastics, with clauses, are suitable as shock absorbers. Apart from the mechanical investigation photoelastic stress analysis also revealed the benefit of osseointegration in force transmission to the bone.

Five-year 224-patient prospective histological study of clinical applications using a synthetic bone alloplast.

Clinical and histologic evaluations of a synthetic bone were performed in 224 patients in a well monitored environment. This material (Bioplant HTR, Bioplant Inc., South Norwalk, CT) was used to obliterate cysts, treat periodontal defects, correct jawbone contours and deficiencies, and gain sufficient support for the placement of dental implants in maxillary subantral augmentation. Implants were positioned either at the time of the synthetic bone graft or after the new supporting tissue was formed. Evidence of new bone formation between the synthetic granules and host tissue was observed during histological examinations. After the material was placed, tissue that could strengthen and augment the inferior wall of the maxillary sinus formed in 3 months. This was observed both clinically and radiographically. After 8 to 12 months, this tissue provided sufficient hard tissue support for the placement of dental implants. This clinical study reconfirmed the applicability of a synthetic bone for bone replacement and augmentation in oral and maxillofacial surgery. No complications caused by infection, inflammation, or rejection of the implanted graft material were observed.

HTR polymer and sinus elevation: a human histologic evaluation.

HTR (hard tissue replacement) polymer was used in a second session to perform 26 sinus floor elevations in 16 patients with dental implantation (Flexiroot, U.S.A.). A period of 6, 8, 10, or 12 months elapsed between the two operations, making it impossible to examine the tissular integration of HTR from both clinical and human histologic aspects. Following the sinus elevation, neither prolonged wound healing nor a rejection reaction was observed in any of the cases. During an 8 to 10 month period, sufficient new bone and fibrous connective tissue had grown between the HTR granules to ensure appropriate supporting tissue for the implantation. Subsequently (12 months), the HTR and the new bone became clinically increasingly harder, forming a union that was difficult to shape. HTR may be stated to be a material suitable for purposes of sinus elevation. In three cases in which the alveolar bone was originally very thin (2 to 3 mm), the resorption of this bone was observed, as a consequence of this the implantation was not performed. The resorption is explained by the inadequate blood supply that developed for surgical-technical reasons, and is not connected with the nature of the material used for the sinus elevation.

[Conditions of successful endosseous dental implantations].

The conditions of success of endosteal dental implants have been summarized on the base of the literature and own experiences. Three corresponding factors may determine the success: biocompatibility, biological seal, and the optimal force transfer. On the base of the data, the implantology is a biological way in the rehabilitation of edentulousness, if the conditions are kept in mind and are applied to the clinical practice.

[Biomechanical testing of DIAKOR implants by photoelastic stress analysis].

Diakor aluminium oxide implants of different forms and sizes have been investigated and compared by photoelastic stress analysis. On forming the conditions of investigations the authors aimed at the modelization based upon the physiological direct implant-bone contact, installing implants having resin that is optically double refractive. In the sample results show a move advantageous stress relation at multi grooved implants of extended dimensions.

Evaluation of alveolar ridge bend and length in upper and lower jaws in anthropological skulls for planning of a series of endosseous dental implants.

The alveolar ridge bend and length were measured in the interforaminal part of the upper and lower jaws in 338 anthropological skulls. Interforaminal sections were determined by projection of the most medial points of the two foramina infraorbitale and foramina mentale at right angles to the alveolar ridge bends. Interforaminal arch lengths and radii of the circles that best fit the arches were analyzed with the use of celluloid transparencies. The differences between female and male group samples and the left and right sides were established. The data were processed by computer and were used for the development of a series of endosseous dental implants to satisfy various anatomical conditions.

[Experience with the clinical use of HTR (hard tissue replacement) polymer. Sinus elevation, human histological studies].

The HTR polymer was employed under clinical circumstances. Results supported that the HTR polymer may well be employed for bone replacement. The forming of the support tissue between the plastic granulums has been accompanied with biopsies. Though in some cases giant cell reaction of alien body type have been observed, yet this has never been of such degree that it would have impeded the reception of the stuff. According to the authors opinion after reinforcement of the lower wall of the facial cavity with HTR polymert in 8 to 10 months an enduring suitable support tissue is formed.

[Questions of principle and practice concerning the use of Diakor aluminum oxide in bioceramical oral endosseous implantation. III. Surgical intervention].

The osseointegration desired in interest of lasting success may be obtained but by means of an aseptic atraumatic operational technique which ensures the primary stability of the implant. It is pointed out that the infection free wound healing plays a decisive role in the lasting stability of the implant also the surgical planning and the method of avoiding complications are described.

[Evaluation of the alveolar curvature and the length of the mandibular ridge on skulls several hundred years old].

The alveolar curvature and the length of the curvature of the upper and lower jaw bone ridge of 338 skulls from an anthropological collection were examined with special regard to sex and side differences for designing dental implantation and jaw bone replacements. The corresponding mean values of men and women deviate at a level of 5% significance. However, in view of practical viewpoints, the difference is not significant. Data revealed that deviations of the right and left side do not exceed 1 millimeter, the jaw bone halves may be considered symmetrical. The mean value of the alveolar curvature length is 58.5 millimeters on the upper jaw bone ridge while it is 48.1 millimeter on the lower one. The lower and the upper jaw bone ridge curvature may be characterized by a circle having a radius of average 24.3 millimeter. When making implantations it seems to be advantageous to make orders of magnitude of 6, 25, 38, 25 and 6% in relation to the total number of pieces from sizes corresponding to the five class moddles.

[Tooth reimplantation].

At the Clinics of Oral Surgery of the Semmelweis Medical University Budapest 16 teeth were reimplanted during the last four years. According to experiences improved by splinting the reimplanted tooth with passive wire arch fastened to brackets, and by filling the root channel with Ca(OH)2. The Favourable therapeutic effect of Ca(OH)2 means impediment of almost always developing resorption.
