[Results of a multicenter study of Rebif-22 mcg administration in Russia].

The results of multicenter study of Rebif 22 micrograms in Russia have been reported. 167 multiple sclerosis patients have received Rebif 22 micrograms three times a week for 1 year. This study provides evidence for reduction of the relapse rate in patients with relapsing/remitting and secondary progressive multiple sclerosis. The tendency to the decrease of the severity of relapses, less need for steroid use and the decrease of EDSS score in patients with relapsing/remitting multiple sclerosis have been shown. In general Rebif 22 micrograms was tolerated well.

[Glatiramer acetate (Copaxone) influence on different stages of multiple sclerosis pathogenesis].

Multiple sclerosis (MS)--autoimmune disease of CNS, characterized by myelin destruction and axonal damage. The action of drugs recommended for its treatment [symbol: see text] aimy[symbol: see text] at the reduction of the autoimmune responses. These responses lead to the neurological damage. One of immunoregulators is glatiramer acetate--Copaxone. Copaxone acts at different phases of MS pathogenesis and probably has a neuroprotective action. Copaxone reduces the rate of relapses and leads to clinical stabilisation in cases of relapsing-remmiting MS. Its efficacy and safety has been confirmed in long-term clinical and MRI-trials. Copaxone has few side-effects that are non-significant.

[Results of open post-registration clinical trials of copaxone in patients with multiple sclerosis].

Results of the 2-year open postregistration study of Copaxone (glatiramer acetate), conducted in 3 Moscow leading medical centres are given. 32 MS patients with remitting-relapsing MS were investigated in accordance with international requirements (neurological scales, MRI of the brain, multimodal evoked potential). Significant reduction of annual relapse rate for 70.3%, stabilization of neurological status and benign safety profile were demonstrated during Copaxone treatment. Practical recommendations for Copaxone use are given.

[Transcranial dopplerography in chronic disorders of cerebral circulation].

Forty one 41-82 year-old patients with chronic cerebrovascular diseases were examined neurologically, using ultrasonic dopplerography (USD) of extracranial cerebral vessels and transcranial dopplerography (TCD) of intracranial cerebral vessels. Changes of the blood flow through major cerebral arteries as well as peculiarities of the venous circulation were evaluated. A significant decrease of the cerebral blood flow velocity (BFV) was found by TCD on early stages of cerebrovascular diseases, while USD revealed it on the III stage of dyscirculatory encephalopathy. The changes in the regulation of the venous circulation increase from latent to obvious alterations of both BFV and the pulsatility index. The conclusion is made that manifestation of clinical symptomatology in the diseases studied is undoubtedly connected with cerebral ischemia and TCD plays an important role in its determination.

[The clinico-immunological characteristics of central nervous system involvement in systemic lupus erythematosus: the relationship with antibodies to cardiolipin].

As many as 30 patients suffering from systemic lupus erythematosus (SLE) with the clinical signs of central nervous system derangement were examined. The mean age of the patients was 31.1 years. Using EIA, antibodies against cardiolipin (a-CL) were detected in 21 patients (70%). A-CL were revealed in all the patients with cerebral circulation impairment (CCI), choreic hyperkinesis, and convulsive syndrome. A-CL were discovered in 12 out of 18 SLE patients suffering from migraine-like headaches and in 4 out of 5 patients with mental disorders. Antibodies reacting with cardiolipin were mostly represented by the IgM isotype (80%) whereas a-CL-IgG were only identified in 13% of the patients, being associated in all the cases with a-CL-IgM. The high level of a-CL-IgG in blood serum was recorded in patients with the gravest patterns of nervous system derangement: CCI, occlusion of the retinal artery, psycho-organic and convulsive syndromes. All these patients demonstrated generalized reticular livedo. The high levels of a-CL-IgM were observed in SLE patients with choreic hyperkinesis and migraine-like headaches. Thus, the studies made it possible to trace the relationship between the development of certain neurological disorders (CCI, chorea, convulsive syndrome) in SLE patients and a-CL.

[Results of clinical and immunoenzymatic studies of neurospecific leucine aminopeptidase in patients with neurologic disorders].

Leucine aminopeptidase of myelin was discovered to contain a protein subunit possessing, in contrast to the main enzymatic mass, antigenic properties and contained by different structures of the central nervous system. The protein was shown to be accumulated by neurons and astrocytes of embryonic tissue culture. The protein was absent from biological fluids of healthy persons and in schizophrenia but it was recordable in the blood and cerebrospinal fluid of patients with different forms of neurological pathology. In patients with neuronal degenerative diseases, the protein level in biological fluids in the terminal stages was significantly lower than in compensated patients.

[Cyclic nucleotides of human and animal glial tumors].

Studies on the level of cyclic nucleotides (cAMP and cGMP) in human and animal glial tumours showed that the content of both nucleotides, especially that of cAMP, decreases in all the tumours. The cAMP/cGMP ratio also drops down. Concurrently it appears to be the most consistent parameter of nucleotide metabolism both in brain tissue and in human or animal glial tumours. The growing tumour affects cAMP and cGMP metabolism not only in the involved but also in the other hemisphere. No principal differences between human and animal tumours have been revealed in the content of cyclic nucleotides and its variation in tumour tissue.

[Changes in the level of cyclic nucleotides in brain tumors].

Cyclic 3-5-adenosine-monophosphate and 3-5-guanosinemonophosphate concentrations were studied in the bioptate of brain tumors, examined during neurosurgical operation and in normal brain tissues. The correlations between cyclic nucleotides in the tumors appeared to be significantly decreased in comparison to the intact tissue, being a characteristic signs for all types of tumors. There were significant fluctuations in the level of cyclic nucleotides both in the tumours and nontumorous brain tissues. There were no correlations found between the level of cyclic adenosine-monophosphate and cyclic guanosine-monophosphate on the one hand and the duration of the disease, localization of the tumor, degree of the brain oedema and the type of narcosis on the other.

[Cyclic nucleotide metabolic disorder in brain tumors].

The content of cAMP and cGMP as well as their ratio in the tissues of different cerebral tumors in humans were studied. The concentration of both cyclic nucleotides in tumors of the brain was found to vary widely. The richest information on disorders of metabolism of these substances may be gained from the ratio of cAMP and cGMP concentrations, which statistically is significantly reduced in the neoplasm as compared to the ratio in the cerebral tissue. Bearing in mind the principal possibility of correcting this ratio, further study of the metabolism of cyclic nucleotides in cerebral tumors holds promise for their treatment.
