[Characteristics of the distribution of the markers of hepatitis B vir us infection among the healthy population of the Tadzhik SSR and Azerbaijan SSR].

A total of 708 healthy persons in Tajikistan and 576 healthy persons in Azerbaijan, these groups comprising persons of both sexes and different age groups, were examined by the method of double gel immunodiffusion (the gel precipitation test) and by the passive hemagglutination test for the presence of the markers of hepatitis B virus (HBV) infection (HBsAg and HBeAg) and antibodies to them. This investigation showed that, in accordance with the level of hepatitis B morbidity, HBsAg was significantly more often detected among the population in Tajikistan (7.2%) than in Azerbaijan (2.8%). In both republics HBV carriers occurred most frequently among children aged 1-4 years (4.0% in Azerbaijan and 13.9% in Tajikistan), and among men more frequently than among women. In accordance with different intensity of the spread of HBV infection in the territories under comparison, differences in the age structure of the immune population were noted: in Tajikistan the formation of the immune layer occurred most frequently among younger age groups and in Azerbaijan, among senior adult age groups. The presence of a considerable percentage of persons with HBe-antigenemia (14.3-14.9% as determined by the gel precipitation test) among HBV carriers, observed in Tajikistan and in Azerbaijan, indicates that some of them have undetected chronic hepatitis B.

[HBe antigen and its antibodies in HBsAg carriers in various regions of the USSR].

A total of 300 blood serum samples, containing HBsAg and obtained from donors in three regions of the USSR (the RSFSR, the Uzbek SSR and the Moldavian SSR) differing in the level of HBsAg carriership, were studied for the presence of HBeAg and antibodies to this antigen in the passive hemagglutination (PHA) test and in the enzyme immunoassay (EIA). The occurrence of HBeAg was found to depend on the level of HBsAg carriership in the region. Thus, according to the EIA results, in Gorky, Kishinev and Tashkent HBeAg was detected, respectively, in 5.5%, 12.3% and 13.3% of serum samples, the level of HBsAg carriership in these cities being, according to the results of the PHA test, 1.4%, 5.0% and 9.0%. As shown by the results of EIA, the occurrence of HBeAg increased with the rise of the titer of HBsAg, while regarding the occurrence of antibodies to HBeAg the reverse relationship was observed.

[Characteristics of the transmission of hepatitis B virus infection to infants from their mothers--chronic HBsAg carriers with anti-HBe present].

A series of long-term prospective studies was carried out to investigate the possibilities and manifestations of perinatal infection in Moscow. The observations included 45 mothers, chronic carriers of HBsAg, and their 48 babies. In most of the mothers (31 out of 41), HBs antigenemia was accompanied by the presence of anti-HBe. No persisting HBsAg carrier state was ever detected in the newborns, although in 9 out of 48 babies transitory ephemeral infection was recorded with minimal clinico-biochemical disorders in 6 and complete lack of any such disorders in 3. The above results are compared with the observations in the countries of South-East Asia, West Africa, Europe and the USA. The causes of significant differences in the epidemiological role of perinatal infection in different world regions are discussed. The primary role of anti-HBe formation, ecological, and genetic factors is emphasized.

[Markers of hepatitis B virus infection in infants born of mothers who are HBs-antigen carriers with anti-HBe present and the problems of passive immunoprophylaxis].

In December, 1983 to December, 1985, blood sera of 14,603 pregnant women were tested for the detection of HBsAg carriers among them. A low level (1.1%) of HBsAg carrier state was demonstrated. HBeAg was detected in 5.2% of women-carriers of HBsAg, anti-HBe in 68.8%. The follow-up observations involved 59 babies born to the mothers HBsAg-carriers. The babies were divided into an experimental and a control group. The experimental group consisted of 22 babies who were given specific anti-HBs immunoglobulin, 37 babies of the control group did not receive this preparation. In the experimental group, 3 babies were found to be infected, all of them at the age of 9-12 months. In the control group the infection was detected in 9 babies, among them in 5 the markers of hepatitis B virus were detected at the age of 1-2 months. Based on these data, the efficacy of using anti-HBs immunoglobulin for prevention of neonatal hepatitis B is discussed.

[Significance of hepatitis B virus markers in children with acute and chronic hepatitis B in the familial spread of the infection].

Clinical, laboratory, and epidemiological examinations were carried out in 331 family members of 93 children with acute hepatitis B and 85 with chronic hepatitis B. Sensitive methods (passive hemagglutination test, radioimmunoassay, enzyme-immunoassay) were used for the detection of markers of hepatitis B virus: HBsAg, anti-HBs, HBeAg, and anti-HBe. Patients with chronic hepatitis B with prolonged HBs- and HBe-antigenemia were shown to play the main role in formation of the familial foci of HB virus infection, whereas the children with acute hepatitis B who, as a rule, had a short-time HBe-antigenemia, were of no great epidemiological importance for their family members. In the families of children suffering from chronic hepatitis B, infection with hepatitis B virus occurred most frequently in their mothers.

[Delta infection in patients with chronic viral hepatitis B].

The results of examination for anti-delta antibodies in the blood of 86 patients (33 children and 53 adults) with chronic persisting hepatitis (CPH) and chronic active hepatitis (CAH) verified by morphological examinations of liver biopsy specimens are presented. Delta infection was found in 43 (31.5%) of patients with CAH, only anti-HBe being demonstrable in the blood. In this group of patients, marked enzymatic disorders and more intensive changes in the morphological picture of the liver were demonstrated.
