[Studies of anti-cytomegalovirus IgG antibody positive rate and cytomegalovirus mononucleosis in adults].

We analyzed anti-Cytomegalovirus (CMV) IgG and IgM antibody (EIA) and anti-Epstein-Barr virus (EBV) viral capsid antigen (VCA) IgG and IgM antibody (FA) in adults during 1994-1999. We examined these IgM sero-positive patient's medical records, and diagnosed CMV mononucleosis and EBV mononucleosis. Anti-CMV antibody positive rates decreased from 87.6% in 1994 to 77.8% in 1999. Especially in twenties, anti-CMV antibody positive rates decreased from 65.2% in 1994 to 53.3% in 1999. On the other hand, anti-EBV VCA antibody positive rates were not changed (91-94%). Number of cases of CMV mononucleosis increased from 2 cases in 1994 to 16 cases in 1999, but EBV mononucleosis was not changed. These results suggested that increasing cases of CMV mononucleosis was influenced by decreasing anti-CMV antibody positive rate.

Aspergilloma: comparison of treatment methods and prognoses.

We report five cases of aspergilloma. Three patients had a previous history of tuberculosis, including one who fully recovered after resection of the right lower lobe. Four patients were treated mainly with oral itraconazole. Two of these four patients, died, one of massive hemosputa, and one of heart valve disease, while two had a good outcome, although one of them has since developed respiratory insufficiency and has received oxygen therapy. In itraconazole therapy, the daily dose may be 200 mg or more, and the duration of treatment may be 1 year or more. Percutaneous intracavitary instillation of amphotericin B, performed in one patient, showed no efficacy. The efficacy of this treatment may depend on the width and number of drainage bronchi, and on the mechanism of acceleration of degradation of the fungus ball. It is important to carefully choose the therapy for aspergilloma, with due consideration being given to the patient's pulmonary function and general status.

[Comparison between cytomegalovirus hepatitis and Epstein-Barr virus hepatitis in healthy adults].

In order to determine the factors responsible for the differentiation of cytomegalovirus (CMV) hepatitis and Epstein-Barr virus (EBV) hepatitis in previously healthy adults, the clinical features and laboratory data of both types of hepatitis were retrospectively analyzed. CMV hepatitis showed a tendency to increase in our department. In comparison with EBV hepatitis, CMV hepatitis occurred in significantly older hosts than EBV hepatitis. We found that lymphadenopathy, cough and sore throat was more common in EBV hepatitis than in CMV hepatitis. The number of peripheral white blood cell count and atypical lymphocytes, and serum GOT, GPT, LDH and CRP levels of CMV and EBV hepatitis showed no significant differences.

[A study on the effect of combined chemotherapy on Mycobacterium avium complex pulmonary disease].

Annual incidence of Mycobacterium avium complex (MAC) pulmonary disease has been gradually increasing in the last 10 years in Japan, however, the optimal therapeutic regimen for the disease has not yet established. We investigated the effect of our new regimen in twenty seven cases of pulmonary MAC infection without HIV infection, diagnosed according to the American Thoracic Society criteria during the period from January 1996 to October 1997 at our hospital. These patients were treated with rifampicin (RFP), ethambutol (EB) and clarithromycin (CAM) for more than 12 months, together with streptomycin (SM) initially (first 2-3 months), except one patient who was treated for 11 months only. Twenty-four months after the therapy, sputum cultures converted from positive to negative in 13 patients and the amount of bacilli in sputum reduced in two patients. The radiological findings improved in 10 patients, showed no significant changes in 11 patients, while worsened in the remaining 6 patients. As to adverse reactions 1 case of liver damage, 3 cases of skin disorders, 4 cases of gastrointestinal malfunctions, and 1 case of optic neuritis were observed. This regimen was safe and tolerable even in the elderly outpatients, but not so effective against MAC pulmonary disease compared with the results of recent reports from the U.S. and Europe. Size of pulmonary lesions was closely associated with the effectiveness in this study. However, five bacteriologically converted cases did not show radiological improvement, and the reasons behind this fact remain to be investigated.

[A case of endophthalmitis and abscesses in the liver and the lung caused by Klebsiella pneumoniae].

[Case report] A 65-year-old male war admitted to a local hospital because of fever. He was treated with piperacillin and clindamycin without noticeable effect. He began to complain of loss of vision on the third hospital day and culture of the blood specimen yielded Klebsiella pneumoniae. He was diagnosed as endophthalmitis and referred to our hospital for further examination. The hematological laboratory test showed leukocytosis (12,700/microliter) and increased CRP (20.4 mg/dl). A computed tomographic (CT) scan of the thorax revealed multiple lung abscesses. An abdominal ultrasonographic scan and a CT scan of the abdomen revealed multiple liver abscesses. We drained the abscess in the liver and Klebsiella pneumoniae was detected from the sample of aspirated fluid and his sputum. Meropenem was administered intravenously. Fever started to improve on the tenth hospital day and the size of both liver and lung abscesses were reduced. He has lost vision of his right eye. He was discharged after sixty days. He did not have any immunosuppressive underlying disease including HIV infection and diabetes mellitus which cause these lesions.

Clinical features of polymyositis/dermatomyositis with steroid-resistant interstitial lung disease.

Polymyositis and dermatomyositis (PM/DM) without creatine kinase (CK) elevation shows a poor prognosis. PM/DM is complicated with interstitial lung disease (ILD), some of which progress rapidly. To clarify the clinical features of PM/DM from the viewpoint of ILD progression, the clinical data of 25 PM/DM patients with ILD were reviewed. They were classified as responders or non-responders. The patients whose ILD responded to steroid therapy and elicited good clinical courses were termed as responders. On the other hand, the patients who had rapidly progressive ILD resistant to steroid therapy were considered as non-responders. The patients diagnosed to have DM were likely to be steroid-resistant. The non-responder group revealed significantly high aspartate aminotransferase (AST), low CK, low white blood cell (WBC), and low absolute lymphocyte counts in their peripheral blood. High CK/AST may be a favorable predictor of the disease. The percentages of lymphocytes in bronchoalveolar lavage fluid were increased in both groups. However, the percentages of two responders with low CK/AST were lower than those of three non-responders. A steroid-resistant ILD group with PM/DM may be clinically different from a steroid-responsive ILD group.

Asymptomatic hyponatremia due to inappropriate secretion of antidiuretic hormone as the first sign of a small cell lung cancer in an elderly man.

A 72-year-old man was hospitalized with asymptomatic hyponatremia. Despite hyponatremia, urinary sodium excretion with urine osmolality exceeding plasma osmolality persisted. Plasma vasopressin levels were high and independent of plasma osmolality during hypertonic saline infusion. Computed tomography of the chest showed enlarged mediastinal and right hilar lymph nodes. Microscopically, a specimen of lymph nodes obtained by biopsy represented vasopressin-producing small cell lung carcinoma. Chemotherapy plus irradiation improved the hyponatremia. Thus, careful evaluation is necessary to determine the cause of hyponatremia disorders in elderly patients.

[Nodular bronchioloalveolar carcinoma with multiple small air spaces caused by bronchiolectasis].

A 67-year-old man was admitted to our hospital with a faint abnormal shadow in the right S2 on January 1995. Chest CT showed a faint abnormal shadow about 3 cm in diameter with multiple small air spaces. He was admitted again 18 months later for surgical treatment because the shadow on chest X-ray had grown. Right upper lobectomy was performed in June 1996 and pathohistological examination revealed bronchioloalveolar carcinoma. Tumor cells along the walls of the alveolus were recognized only in the periphery of the lesion. The center of the lesion was fibrotic and multiple small air spaces ranging from 0.5 mm to 10 mm in diameter were present. One cause of these air spaces was considered to be the "check valve" mechanism, but the majority of the air spaces were derived from ectatic bronchioli. These results suggested that bronchiolectasis had developed with central fibrosis only in the tumor. Central fibrosis in this case had played a significant role in determining the radiographic appearance of the lesion.

[Bronchial lipoma with extrabronchial growth].

A 69-year-old woman was admitted to our hospital with a faint infiltrative shadow in the right middle lung field on chest X-ray in August 1996. In addition to inflammatory changes in right S2 and S3, an intrabronchial elliptical mass with a low CT number (mean number:-144 HU), was noted in the right upper bronchus on chest high resolution computed tomogram (HRCT). Fiberoptic bronchoscopy revealed a yellowish-orange polypoid lesion in the right upper bronchus, and bronchial biopsy demonstrated proliferation of fat tissue in the submucosa. Bronchial lipoma was subsequently diagnosed. Because HRCT findings indicating extrabronchial growth of the tumor, surgical resection was performed in October 1996, when extrabronchial growth of the tumor was confirmed. When extrabronchial growth of a bronchial tumor is suspected based on CT findings, surgical treatment should be considered even for a benign bronchial tumors.

[Suspected leiomyosarcoma of the trachea].

A 73-year-old women was admitted to our hospital because of dyspnea, bloody sputum, and an apparent intratracheal tumor. A chest X-ray film from August 1991 showed a sharply circumscribed soft-tissue density in the trachea at the level of the aortic arch. By March 1993 the tumor had grown from 15 mm to 20 mm in diameter. A chest CT scan and fiberoptic bronchoscopy showed an intratracheal tumor that occupied 80% to 90% of the tracheal lumen. The tumor was resected on March 26, 1993. Histopathological study revealed a suspected leiomyosarcoma of the trachea. Only 18 cases of leiomyosarcoma of the trachea have previously been reported worldwide. We know of only 3 previous reports of the case of suspected leiomyosarcoma of the trachea in Japan.

[Pneumonitis due to Hangeshashin-to].

A 72-year-old woman was admitted to our hospital because of dyspnea and abnormal shadows on a chest X-ray film. The X-ray film revealed diffuse reticular and patchy shadows in both lung fields. Drug-induced pneumonitis was suspected, and all drug treatment was stopped. The symptoms were relieved and the X-ray findings improved markedly. In bronchoalveolar lavage fluid, the lymphocyte percentage was high and the CD4/CD8 ratio was low. Microscopic examination of open-lung biopsy specimens showed pneumonitis, dominant lymphocyte invasion within peribronchiolar and alveolar interstitia. The result of leukocyte migration inhibition test was positive for Hangeshashin-to and Byakkokaninjin-to, and lymphocyte stimulation test was positive for Hange, an ingredient of Hangeshashin-to. Based on these findings, we diagnosed as pneumonitis induced by Hangeshashin-to. To our knowledge, there has been no previous report of pulmonary hypersensitivity induced by Hangeshashin-to in Japan.

[Mega-dose chemotherapy with peripheral blood stem cell transplantation (PBS-CT) for small cell lung cancer (SCLC)].

Feasibility and the efficacy of mega-dose chemotherapy with peripheral blood stem cell transplantation (PBSCT) for small cell lung cancer (SCLC) were evaluated. Autologous peripheral blood stem cell (APBSC) was collected after induction chemotherapy (Extensive disease: CDDP+etoposide+ADM, Limited disease: CDDP+etoposide) followed by the administration of granulocyte colony stimulating factor (G-CSF). Five cases of ED and 2 cases of LD have been entered in the protocol so far. PBSC was harvested after the second course of induction chemotherapy, and a sufficient number of PBSC (CFU-GM > or = 1 x 10(5)/kg) could be harvested and cryopreserved in all 7 cases. After completion of 4 courses of induction chemotherapy, three patients who achieved CR or good PR were treated by the combination of CBDCA (1,600 mg/m2) plus etoposide (1,600 mg/m2) followed by APBSCT and G-CSF. Hematologic recovery after APBSCT was rapid (absolute granulocyte count > 500/microliters: 9-11 day, Plt > 3 x 10(4)/microliters:10-14 day) and no infectious episode nor bleeding tendency occurred throughout the treatment period. Although grade 3 gastrointestinal toxicity was seen in one patient, the mega-dose chemotherapy with APBSCT in SCLC was considered safe and feasible. Further clinical trials are needed to establish the role of mega-dose chemotherapy for the treatment of SCLC.

Lung abscess: analysis of 66 cases from 1979 to 1991.

Sixty-six patients with bacterial lung abscess were treated between 1979 and 1991 in our hospital. Among these patients, death occurred in one of the 42 cases of community-acquired infection (mortality rate: 2.4%) and in 16 of the 24 cases of nosocomial infection (mortality rate: 66.7%). Of all 66 cases, 55 were culture-positive, and the etiologic agents isolated from 24 of the culture-positive cases were found to be anaerobic bacteria. The most common aerobes isolated from the foci were identified as Staphylococcus aureus, Klebsiella spp. and Pseudomonas aeruginosa, while the most common anaerobes were Bacteroides spp., Peptostreptococcus, Fusobacterium spp., microaerophilic Streptococcus and Veillonella. The mortality was higher in the cases with P. aeruginosa, Klebsiella spp. and Candida spp. than in those with other bacteria. The prognosis of lung abscess patients proved to depend on the presence of underlying diseases and on superinfection with aerobes.

Analysis of cases of central nervous system fungal infections reported in Japan between January 1979 and June 1989.

Data from 129 cases of central nervous systemic fungal infections reported in Japan between January 1979 and June 1989 were analyzed. Of 129 cases, 116 were cryptococcal meningitis, 6 candidal meningitis (including meningitis due to Trichosporon cutaneum) and 7 Aspergillus meningoencephalitis. Fifty-six of the patients with cryptococcal meningitis had an underlying systemic disease or condition. The overall survival rate was 72.4%, which was a markedly improved rate compared to earlier reports. Cryptococcal meningitis was treated most often with the combination of intravenous amphotericin B and 5-fluorocytosine and was associated with a survival rate of 81.8%. All 7 patients treated with fluconazole alone survived. Candidal meningitis occurred secondary to a shunt infection; some patients with candidemia did well when adequate therapy was instituted. Aspergillus meningoencephalitis is most often diagnosed at autopsy or in surgical specimens and the prognosis of Aspergillus meningoencephalitis is generally poor.

Periodic fever compatible with familial Mediterranean fever.

A 55-year-old male presented with a recurrent fever of over 38 degrees C, occurring at irregular intervals 1-6 times a month with chest, back or abdominal pain. After admission to our hospital, we found the following characteristics: 1) the febrile attacks were accompanied by obvious inflammatory findings and pleuritis or peritonitis; 2) the patient's elder sister had a similar periodic fever; and 3) there were no apparent causative factors responsible for his symptoms. Therefore, we diagnosed this as a case compatible with familial Mediterranean fever. The febrile attacks have been completely suppressed by daily colchicine. This is the seventh case of familial Mediterranean fever reported in Japan.
