Sixty-three elderly patients with gastrointestinal cancer were treated with 5-fluorouracil (5-FU) or tegafur alone, and a combination of mitomycin C (MMC) and 5-FU. Judged by Koyama-Saito's criteria, the combination treatment was superior to single drug therapy. The response was somewhat correlated with survival in the combination regimen although it produced more gastrointestinal and myelotoxicities than the single drug. Our results suggest that the combination of MMC and 5-FU is useful in the treatment of gastrointestinal cancers in the elderly.

Determinations were made of the Zn and Mg contents of pancreatic juice in 12 cases undergoing external pancreatic duct drainage following pancreaticoduodenectomy in order to investigate the output and excretion pattern of trace elements in pancreatic juice and also to study their relations to pancreatic enzyme secretion and fibrosis of the pancreas. THe results indicate that the output of Zn in pancreatic juice ran parallel with amylase activity and was closely related to secretion of pancreatic enzymes. Relatively smaller variations were noted of the Mg content in pancreatic juice during the postoperative course, with the pattern of Mg excretion in pancreatic juice being dependent upon the volume of pancreatic juice secreted. The output of both Zn and Mg in pancreatic juice correlated with the volume of the residual pancreatic parenchyma. The amylase/Zn concentration ratio of pancreatic juice remained stationary postoperatively in all cases and was shown to have a negative correlation with the pancreatic parenchymal ratio. The findings suggest that the amylase/Zn concentration ratio can be a reliable indication of the degree of pancreatic fibrosis or of pancreatic exocrine function.
Twenty elderly patients with hepatocellular carcinoma were treated with 5-fluorouracil (5-FU) at a daily dose of 200-300 mg per os, and with a combination of 0.3-0.35 mg/kg of mitomycin C (MMC) iv on day 1 and 8-10 mg/kg of 5-FU iv daily starting on day 1 for 5 consecutive days, repeating at 4 week intervals. According to the Koyama-Saito's criteria, there was no response in the 5-FU treatment, while a 20% (2/10) of response rate (2 partial responses) was obtained in the combination regimen. The durations of response in 2 partial responses were 7.3 and 4.0 months, respectively. The median survival with MMC and 5-FU was 3.4 months compared with 1.5 months with 5-FU alone, and the survival was correlated with tumor response. The combination produced more gastrointestinal toxicities and myelotoxicities compared to a single drug, but they were well tolerated. These results support the superiority of the combination of MMC and 5-FU over 5-FU alone in the treatment of elderly hepatocellular carcinoma.
For the purpose of examining the transfer of antitumor drug into the blood, pancreatic juice, bile and pancreatic tissue, Futraful suppository (FT-sup) 1500 mg were given to 10 patients with pancreatic or bile duct malignancies, measuring Futraful and 5-FU concentrations in the pancreatic juice, bile and pancreatic tissue with lapse of time. Futraful and 5-FU concentrations of the pancreatic juice and bile reached to the maximum 4 hours after administration. The peaks of Futraful and 5-FU concentrations in the pancreatic juice and bile about 2 hours after the maximum values in the blood. 5-FU concentration in the pancreatic tissue was higher than that in the blood in every case. Transfer rate of Futraful to the pancreatic tissue was higher especially in the cases with higher pancreatic parenchymal ratio.
