Perinatal group B streptococcal colonization and infection.

Of 2,169 patients screened in labor for vaginal group B streptococcal colonization, 164 (7.6%) had positive results. Five hundred twenty-four of the patients were screened in the antenatal period, and 57 patients with positive tests for group B streptococcus were treated in labor with intravenous ampicillin. Four of the treatment group gave birth to colonized infants but none became infected. Of the 136 untreated mother-infant pairs with positive tests, 62 neonates were colonized, nine became infected, and three died. In the presence of an attack rate of 6.6% among infants born to colonized mothers, antenatal screening and treatment in labor of mothers colonized with group B streptococcus will reduce neonatal colonization and infection.

Cefotaxime, cefazolin, or ampicillin prophylaxis of febrile morbidity in emergency cesarean sections.

Cefotaxime, a new third-generation cephalosporin, was compared with ampicillin and cefazolin in a randomized double-blind trial to evaluate the efficacy of antibiotic prophylaxis of febrile morbidity associated with emergency cesarean sections. A 1-gm intravenous dose of one of the three antibiotics was given by bolus injection immediately after clamping of the umbilical cord and six and 12 hours later. All patients were in labor with membranes ruptured and had a temperature less than or equal to 37.8 C, and none had a history of penicillin or cephalosporin allergy. A total of 195 women were entered into the trial. Initially, the study included a placebo control group which was switched to ampicillin after 30 patients. Of the 188 evaluable patients, 51 of 59 (86.5%) ampicillin recipients, 59 of 67 (88.1%) cefazolin recipients, 48 of 55 (87.3%) cefotaxime recipients, and two of seven (28.5%) placebo recipients had uneventful postoperative courses. During the study, an additional 39 women who were in labor with ruptured membranes but who were allergic to penicillin or who declined antibiotic prophylaxis were classified as untreated patients and observed for postoperative complications. Standard febrile morbidity, primarily related to endometritis or wound infections, occurred in 6 of 59 (10.1%) ampicillin, 5 of 67 (7.5%) cefazolin, 5 of 55 (9.1%) cefotaxime, and 18 (40.0%) of placebo or untreated patients. Cefotaxime, cefazolin, and ampicillin were equally effective in reducing febrile morbidity in emergency cesarean sections.

Cesarean section: trends and morbidity.

A review of 1683 cesarean sections performed at one hospital in a 3-year period (1977-79) showed that the cesarean section rate had trebled since 1967-79, the rates being 16.9% and 5.8%. The main indications for cesarean section responsible for this rise were dystocia, breech presentation and a previous cesarean section. AFter the operation 23.3% of received antibiotics. If the cesarean section rate is to fall, the biggest impact can be made by planning vaginal delivery in selected patients with a previous cesarean section and by improving the management of nonprogressive labour.

Fetal biophysical profile scoring: a prospective study in 1,184 high-risk patients.

The results of a prospective clinical management based on fetal biophysical profile scoring method was evaluated in 1,184 referred high-risk patients. Six perinatal deaths occurred in the study group (perinatal mortality of 5.06 per 1,000); only one fetus suffered unpredictable and unpreventable death (true false negative rate 0.8 per 1,000). In addition, 13 fetuses with major congenital anomalies were detected as a result of ultrasound scanning for fetal biophysical activities. These data suggest that this method can be used effectively to screen and manage a high-risk population.

Simplified antepartum fetal heart assessment.

Antepartum fetal heart rate monitoring based on the association of fetal heart rate accelerations with fetal movements (non-stress test) is a standard method for fetal surveillance. Application of this method requires a continuous fetal heart rate recording monitor. Audible detection of heart rate accelerations was compared with recorded accelerations in 100 tests. A listening observer detected 77.7 per cent of all accelerations with an amplitude of at least 15 beats/minute. This allowed the detection of 94.2 per cent (82 out of 87) reactive non-stress tests. In no case was a non-reactive test called falsely reactive by the listening observer. This method may be useful for assessing fetal condition when continuous fetal heart rate monitors are not available.

A university department's involvement with medical care in the Canadian North.

Both visiting consulting services and resident general practice services to the Churchill Health Centre in northern Manitoba are provided by the University of Manitoba through its northern medical unit. The roles of the university's department of obstetrics and gynecology with regard to visiting consulting services include patient care, education and medical audit. This paper reviews the several aspects of this involvement over the period 1971 through 1977.

Obstetric cholestasis. A 14 year review.

A 14 year review of obstetric cholestasis in the Women's Centre, Winnipeg, revealed 42 cases (incidence one in 1,293 deliveries). The perinatal outcome in these 42 patients was compared to that in a group of control subjects and showed a significantly higher incidence of premature labor, low-birth-weight babies, and postpartum hemorrhage. There was no particular association of intrapartum fetal distress or intrauterine growth retardation with the cases of obstetric cholestasis.

Obstetric care in the central Canadian Arctic.

All pregnancies that occurred during 1971-5 among 4000 Canadian Eskimos living in isolated settlements in a district of the North-west Territories were reviewed. Obstetric care was provided in settlement nursing stations, at a base hospital manned by general practitioners, and at a teaching hospital in Winnipeg. Of the 622 infants delivered in 1971-5 218 were delivered in nursing stations by midwives, 338 in the base hospital, and 54 in the teaching hospital. Caesarean sections were performed in 10 cases, and the perinatal mortality was 25.7 per 1000 births. Though it is hard to defend patients delivering their babies in remote areas with no medical help, the results seemed to be acceptable. The credit for this goes to experienced midwives, a liberal evacuation policy, close co-operation from general practitioners, and the specialist visiting and consulting service.

Fine structure of the human placenta in prolonged pregnancy. Preliminary report.

Placentas from 15 prolonged (is greater than 42 weeks), but otherwise normal pregnancies, and 8 normal-term placentas were studied. Placental villi from term placentas revealed intact syncytial and cytotrophoblast elements. All term placentas presented a minimal to mild degree of villous necrosis and deposition of fibrinoid material. Placental villi from cases of prolonged pregnancy presented a moderate to severe degree of necrosis in 11 of 15 placentas and a minimal to mild degree in 4 of 15 placentas. A moderate to severe degree of fibrinoid deposition was noted in 7 of 15 placentas, whilst 8 of 15 revealed a minimal to mild degree of deposition. The presence or absence of meconium was noted in the prolonged-pregnancy group and correlated with the above morphological features. Statistically, a correlation was found between prolonged pregnancy and necrosis (p is less than 0.001), prolonged pregnancy and fibrinoid deposition (p is less than 0.03) and between necrosis and deposition of fibrinoid (p is less than 0.05). There was no correlation between the incidence of meconium staining and the above parameters.

The oxytocin challenge test and antepartum fetal assessment.

The oxytocin challenge test (OCT) was performed 537 times on 364 high-risk patients over a three-year period. It was shown that a positive test had a highly significant correlation with a worsened perinatal outcome as measured by stillbirth rate, fetal distress in labour, intrauterine growth retardation, Apgar scores and the need for neonatal resuscitation and intensive care. However, one-third of positive tests were falsely positive when tested by labour. A negative test proved reassuring for a further week of intrauterine life in 98.2 per cent of cases. The role of the OCT in antepartum fetal monitoring is discussed.

The estimation of gestational age by amniotic fluid analysis and its relationship to fetal growth retardation.

The relationship between three amniotic fluid parameters (fat staining cells, anucleate cells and creatinine) and gestational age has been studied in 166 samples from 107 patients of known gestational age. Individually there is an unacceptable degree of inaccuracy. However, when the three parameters are combined in a multilinear regression, an equation for gestational age is produced that is reliable to with +/-14 days in over 90% of cases. This held true when the equation was applied to 60 samples from 45 patients with uncertain dates and related to the paediatric estimate of gestational age. The accuracy was unaltered by intrauterine growth retardation.

Paracervical block with bupivicaine.

Ninety-five cases in which paracervical block was administered in labour are reported. These permitted comparison of the duration of analgesia provided by 1% lidocaine and 0.25% bupivicaine, with and without adrenalin. The dangers of paracervical block are reviewed and its role in modern obstetrics is discussed.
