State-of-the-art patient care. The impact of doctorally prepared clinical nurses.

Delivery of state-of-the-art patient care requires new models to foster the development of professional nurses and integrate practice, research, and education. The Chief Nursing Officer of University Hospital/Associate Dean of the College of Nursing at a tertiary health science center positioned doctorally prepared nurses in clinical practice settings to assist in actualizing a vision of state-of-the-art patient care. Strategic targets for performance improvement included building collaboration between the college and hospital, supporting advanced education for nurses, moving the nursing culture from one of co-dependence to one of professionalism, fostering research at the unit level, and capitalizing on the strengths of nurse leaders. Creating an environment where staff nurses use critical thinking skills and access their advanced practice nurse (APN) resources as they do their work has been a win-win-win situation for the patients, the hospital, and the college at the health science center.

Developing patient and family education programs for a transplant center.

Patient and family education is an important component of the organ transplant programs at the University of Nebraska Medical Center. The Medical Center is in the process of planning a new transplant center which will employ the family-centered, educationally-intensive cooperative care concept. This approach was chosen as the model for the delivery of care at the Lied Transplant Center because it emphasizes efficient, effective clinical care by requiring active participation by the family or essential other, thereby better preparing both the patient and the family for the transition to home and to the community. This article presents the evolution of patient education in our transplant programs, discusses the educational needs of transplant patients across the continuum of care, provides insight into the process of planning educational programs for the new center and provides a sample module for teaching which is based on the Cooperative Care concept.

The effect of intravenous metoclopramide on blood pressure in normotensive and hypertensive subjects.

In the present study we evaluated the effect of intravenous metoclopramide on blood pressure in normotensive (untrained, football players, runners) and hypertensive subjects. There was a decrease in blood pressure only observed in untrained female subjects and this was greater in hypertensive subjects. In football players and runners the decrease in blood pressure was not statistically significant. There was no significant effect on heart rate. The probable mechanism of this new pharmacological effect of metoclopramide is unknown, however, research is now in progress to define its mechanism of action.

Allogeneic bone marrow transplantation.

From highly investigational to standardized therapy, allogeneic BMT has established its role in the treatment of selected diseases. Continued refinements in bone marrow transplantation will increase the number of potential candidates, reduce associated risks, and improve disease-free survival rates. With further exploration into its efficacy, allogeneic BMT will bring new challenges and opportunities for patients, families, and health care providers.

Prevention of CCL4-induced liver cirrhosis by silymarin.

The efficacy of silymarin treatment in preventing biochemical and histological alterations in CCL4-induced liver cirrhosis in rats was studied. Four groups of rats were treated with: (1) CCL4; (2) mineral oil; (3) CCL4 + silymarin; and (4) silymarin. All animals were sacrificed 72 h after the end of treatments. The activities of alkaline phosphatase (alk. phosp.), gamma-glutamyl transpeptidase (GGTP), glutamic pyruvic transaminase (GPT) and glucose-6-phosphatase (G6Pase), and bilirubin content were determined in serum. Na+, K+-ATPase and Ca++-ATPase activities were measured in isolated plasma membranes. Lipoperoxidation, triglycerides (TG), and glycogen contents were also measured in liver homogenates. Liver cirrhosis was evidenced by significant increases in liver collagen, lipoperoxidation, serum activities of alk. phosp., GGTP, GPT, G6Pase, bilirubin content, and liver TG. Activities of ATPases determined in plasma membranes were significantly reduced, as was liver glycogen content. Silymarin cotreatment (50 mg/kg b.wt) completely prevented all the changes observed in CCL4-cirrhotic rats, except for liver collagen content which was reduced only 30% as compared to CCL4-cirrhotic rats. Silymarin protection can be attributed to the agent's antioxidant and membrane-stabilizing actions.

Face-lift stigmas.

In this analysis of postoperative results in rhytidoplasties, the author points out some characteristics that announce the operated face. These characteristics include: retraction and elevation of the temporal hairline; straightening of the eyebrows; serious preauricular scars; deformation of the tragus; frequent hypertrophic scars in the mastoid region; and unevenness of the retroauricular hairline. These stigmas are sometimes unavoidable, depending on the patient's anatomy and the extent of the surgical procedure required to achieve the aimed-for young look. They can be improved, however, if one utilizes some alternative technical procedures and carefully observes each individual patient, avoiding the undiscriminating use of the same technique in different cases.

Axial flap from the inferior pole of the breast.

The dermal-adipose segment of the inferior pole of the breast is usually resected during mammaplasty. However, this segment, based only on a medial or lateral pedicle, may be used as an axial flap to provide a moderate amount of tissue in reconstruction of the anterior thoracic regions.
