Cimetidine vs a combination of antacid and anticholinergic for hospitalized patients with gastric ulcer.

One hundred and eighty-five inpatients with ulcer in the gastric corpus and angle were treated with either 1 g cimetidine per day or with a combination of antacid and anticholinergic. At 2, 4, 6 and 8 weeks after entry into the study, the ulcer had healed in 3.3, 30.4, 55.4 and 87.0 per cent of patients treated with cimetidine, as compared with 0, 12.9, 51.6 and 73.1 per cent of those treated with a combination of antacid and anticholinergic (NS., p less than 0.01, NS., p less than 0.05), respectively. Factors that had a delaying effect on ulcer healing were seen more frequently in the cimetidine group. Thus, cimetidine proved to be superior to a combination of antacid and anticholinergic, for the treatment of gastric ulcer.

Adrenocorticosteroid therapy and gastroduodenal lesions.

Gastroduodenoscopy was performed in 25 patients with various disorders, such as liver cirrhosis, nephrotic syndrome, and ulcerative colitis, to assess the effects of corticosteroids on the stomach and duodenum. The main criterion for entry into the trial was the absence of open ulcer, healed ulcer, erosion, or bleeding from the stomach or duodenum on pretreatment endoscopy performed within 48 hours before administration of corticosteroids. Endoscopy repeated at 2 to 4 weeks disclosed gastroduodenal lesions in 11 cases (44%)and no lesion in 14 cases (56%). The gastroduodenal lesions observed in 11 cases are as follows: one gastric ulcer (4.0%), six gastric erosions (24.0%), two gastroduodenal erosions (8.0%), and two duodenal erosions (8.0%). A lack of correlation between the patients' subjective complaints and endoscopic findings indicates the unreliability of patients' complaints and the importance of endoscopy in assessing gastroduodenal lesions. There were no differences in the total and average daily doses of corticosteroid between a group with gastric and/ or duodenal lesions and a group without such lesions. Corticosteroids may produce gastroduodenal lesions, regardless of the dose.

Radiographic and endoscopic findings in penicillin-related non-pseudomembranous colitis.

The characteristic findings of barium enema and colonofiberscopic studies and their changes over time were investigated in 10 patients with penicillin-related non-pseudomembranous colitis. Radiographic examination within 6 days of onset revealed abnormal findings such as narrowing of the lumen, loss of haustral markings, thumb-printing, transverse ridging, and saw-toothed irregularities. These findings tended to be more pronounced on the right than on the left side of the colon. Endoscopy revealed lesions characterized by mucosal reddening, edema, and hemorrhage, without definite ulceration or erosion. In 5 of the 10 patients, non-continuous distribution of the lesions was noted. In 4 patients, the rectum was unaffected. Reversion to normal occurred within an average of 15.7 days following onset of the symptoms. Thus, for an accurate diagnosis of this disease, barium enema and/or total colonoscopy should be performed within 6 days of onset.

Factors influencing the healing rate of gastric ulcer in hospitalised subjects.

A multiple linear regression analysis was carried out on 75 inpatients with gastric ulcer. In order to elucidate the effects of various factors - endoscopic and roentgenological findings, age, sex, medical history, and drugs such as antacids, anticholinergics or both - on the healing rate, these factors were compared between those with ulcer which healed within eight weeks after treatment and those which did not. In patients over 50 years of age, alcohol consumption of over 60 g per day until admission, duration of present ulcer pain for over three months, single ulcer, ulcer located in the lesser curvature and uneven elevation around the ulcer, there was significant delaying effect on ulcer healing. Drug ingestion, sex, smoking habits until admission, size, depth, and shape of ulcer, coexisting gastritis, and past and family history of ulcer disease had no significant effect on healing after eight weeks. The patients with less than two unfavourable factors (n = 46) had the best healing rate (100%) compared with those with three (n = 20) or four or more (n = 9) unfavourable factors. The healing rate of the latter two groups was 60% and 22%, respectively (p less than 0.001). A prognostic score based on these six factors represents the severity of gastric ulcer disease with regard to the healing rate in patients prescribed antacids, and/or anticholinergic drugs.

Hypophosphatemia induced by intravenous administration of Saccharated iron oxide.

Forty milligrams saccharated iron oxide was given intravenously, daily, to nine iron-deficient patients with moderate to severe anemia. During the 14-42 days of treatment, levels of serum inorganic phosphorus (Pi) decreased significantly and in a stepwise manner (before vs 1 week, P less than 0.005; 1 week vs 2 weeks, P less than 0.01). Seven of the nine patients became hypophosphatemic within 2 weeks and the other two within 4 weeks. In parallel with the decline in serum Pi, the phosphate clearance increased and tubular reabsorption of phosphate decreased. Reversion to normal levels was delayed in patients treated for the longer period.

The BTPABA pancreatic function test in giardiasis.

The test for exocrine pancreatic function using N-benzoyl-L-tyrosyl-p-aminobenzoic acid (BTPABA test) was assessed in 7 patients with giardiasis and 7 healthy controls. Cumulative percent p-aminobenzoic acid (PABA) recovery in 6 hr was significantly lower in patients with giardiasis, compared with the control group. When an equivalent dose of free PABA was given, there were no differences in PABA recovery between the groups. In patients with giardiasis, the post-treatment values of BTPABA test were significantly higher than the pretreatment values and no differences were found in PABA recovery between patients with giardiasis after eradication and healthy controls. These findings indicate that Giardia lamblia interferes with the action of pancreatic chymotrypsin. It is noteworthy that giardiasis could cause an abnormal BTPABA test.

Bleomycin in verrucous squamous cell carcinoma of the oesophagus.

A case of verrucous squamous cell carcinoma of the oesophagus treated with bleomycin is reported. Bleomycin brought prompt reduction of the tumour and relief of symptoms. There have been no previous reports on experience with anti-cancer agents for verrucous carcinoma of the oesophagus. Administration of bleomycin might be an effective treatment for verrucous carcinoma of the oesophagus.

Human gastric carcinoma: prognosis in relation to macroscopic and microscopic features of the primary tumor.

The correlation of prognosis with various clinical and pathologic factors was studied by use of multiple stepwise linear regression analysis among 77 patients undergoing resections for gastric cancer. Factors such as depth of penetration, invasion of the duodenum, macroscopic appearance, intramural and extramural growth pattern of the cancer, lymphatic or vascular invasion, and cancer cell-free dense submucosal fibrosis around the cancerous area were significant, and independent factors correlated with the prognosis, in that order. When these factors were taken into account, other factors such as sex and age of the patient, location, size, and histologic classification of the tumor, and folds' convergence were not significant.

[Evaluation of fosfomycin in Campylobacter jejuni enteritis].

Out of 1,219 pediatric patients who were brought to our hospital with chief complaint of diarrhea and abdominal pain and in whom stool cultures were obtained for bacteriological studies in the 17 month period from May 1 in 1981 to September 30 in 1982, Campylobacter jejuni was isolated in 203 patients (16.7%). In this study we assessed the efficacy of fosfomycin (FOM) for C. jejuni enteritis. Results of antibiotic susceptibility tests revealed that C. jejuni is highly sensitive to FOM. The efficacy of FOM was confirmed both clinically and bacteriologically. Moreover, improvement in diarrheal symptoms and the duration required for cultural conversion under FOM treatment were assessed. From the results obtained, it was considered that FOM is an antibiotic with high efficacy for the treatment of C. jejuni enteritis.

Effects of total parenteral nutrition on colonic lesions in Crohn's disease: radiographic and endoscopic study.

Changes in the colonic lesions were studied by means of radiographic and colonofiberscopic examination in 7 cases with Crohn's disease treated with total parenteral nutrition (TPN) for an average of 28.6 days. During the treatment, radiographic examination showed that narrowing, irregular contour, and various ulcers of the colon were reversed, at least in part, in all cases, although there was no improvement in the strictured lesions of 2 cases, who finally underwent surgical intervention. The colonic changes were further improved in 4 of the 5 cases who received radiographic examinations of the colon from 18 to 58 months (average 32.2 months) after TPN was discontinued. The present study suggests that treatment with TPN for Crohn's disease is not only beneficial for clinical symptoms but also improves radiographic and endoscopic changes of the colonic lesions except for stricture.
