[Four cases of hangman's fracture].

Four cases of hangman's fracture were reported. The cases were disturbed between 67 and 85 years of age and all were males. The exact mechanisms of injuries of these cases were not precisely documented, but all struck the head by falling for some distance. Three of them showed cerebral concussion immediately after injuries. One of four patients had complete transverse cord lesion at C5 level and the other showed right Brown-Séquard syndrome at C4 level, while another two showed no neurological deficits except for neck pain. Radiological findings disclosed fracture of the pedicles or the lamina of the axis and anterior spondylolisthesis of the axis upon the third cervical vertebra. In one patient, fracture of the body of third cervical vertebra was combined. All cases were initially treated by skeletal traction, in order for reduction of dislocation, then fixation of the injured spine. Steroid and mannitol were administered to patients with complete cord section and Brown-Séquard type of lesion. Two out of four patients showed uneventful clinical course. A patient shown Brown-Séquard syndrome revealed fairly good recovery, and another with showing transverse cord lesion died from fatal gastrointestinal bleeding 15 days after injury. Analysis of C2 fractures experienced in these four cases suggested that direct downward forces along the longitudinal axis when the head remained in extension or flexion caused these fractures, whereas pure hyperextension injures of the upper cervical spine, which has been considered as main mechanism, had only a minor role.

[Treatment and prevention of infections with cefoxitin sodium in patients with biliary tract infections and obstructive jaundice].

UNLABELLED: A total of 24 patients who was hospitalized in the Internal Medicine Wards of Yamaguchi University attached Hospital and the university's 3 related hospitals were administered with cefoxitin. The breakdown of the patients treated with cefoxitin was 7 with cholecystitis, 7 with choledochitis and the remaining 10 for the prevention of infections with obstructive jaundice. Daily doses of 2-6 g of cefoxitin were administered for 6-40 days by intermittent intravenous drip infusion in divided doses. RESULTS: 1. Of 14 patients with biliary tract infections, 10 (71.4%) responded favorably with cefoxitin. 2. Of 10 patients with obstructive jaundice used for the prevention of infections, 8 (80%) responded favorably with cefoxitin. 3. No untoward side effects were observed. 4. Cefoxitin proved to be a safe and effective antibiotic in the treatment of biliary tract infections and for the prevention of infections in patients with obstructive jaundice.

Type IV glycogenosis - a study of two cases.

Liver biopsy materials of two siblings with type IV glycogenosis were studied by light and electron microscopy. Biochemical analysis was added using autopsy material in one of the two cases. Two kinds of polysaccharides were noted not only in the cardiac muscle, skeletal muscles, smooth muscles and reticuloendothelial cells, but also in the neutrophils and platelets. One was glycogen and the other was similar to amylopectin. Ultrastructurally, a large amount of fibrils, 60 A in width, glycogen rosettes and glycogen granules were detected in those cells. Branching glycosyltransferase deficiency was biochemically confirmed in one case examined.

Mouse strain difference in the expression of antitumor activity of PS-K.

Effect of PS-K on tumor growth and antibody production was studied in inbred C57BL/6, SL, C3H/He, and AKR mice, and in colony-bred ICR mice. (1) PS-K exhibited antitumor activity on sarcoma-180 in ICR mice, but not in AKR mice. Growth of sarcoma-180 was suppressed to the intermediate extent in other strains. (2) In ICR strain, antibody production against trinitrophenyl was depressed in mice bearing sarcoma-180 and restored by PS-K. In AKR mice, on the other hand, antibody production was not depressed in tumor-bearing state and was not augmented by PS-K. Other strains showed intermediate degrees of suppression of antibody-producing capacity by sarcoma-180 and its restoration by PS-K.CR strain, the growth of Ehrlich tumor as the challenge tumor was enhanced in mice bearing sarcoma-180 as compared to that in controls. After the treatment with PS-K in this strain, however, not only the growth of sarcoma-180 but also of Ehrlich tumor was inhibited completely. On the other hand, the growth of Ehrlich tumor in AKR strain was neither enhanced in mice bearing sarcoma-180 nor inhibited by PS-K.
