[Evaluation of cross circulation in conjoined twins].

This report describes the evaluation of cross circulation in conjoined twins. Female thoracoomphalopagus conjoined twins were delivered by cesarean section after 37 week's gestation. CT, MRI and echography were performed. A partial communication of pericardium and sternum, and a union of the liver were found. For the preoperative evaluation of cross circulation, a bolus of indigo carmine was injected, and the pigment appeared in the urine of the other twin. RI angiography showed that radionuclides in one twin were similar to those in the other after 5-10 minutes. During the operation, an injected bolus of SCC to one twin was not effective for the other twin. Inhaled isoflurane in one twin did not appear in the expired gas of the other twin. After the intentional hemorrhage for 3 minutes from one twin, the hemoglobin concentration of one of the twin was the same as that of the other. This demonstrates that acute hemorrhage from one twin seems to result in a significant and rapid decrease of hemoglobin in both babies equally. However, intravenous infusion of drugs to one of the twins requires a relatively long time to take effect in the other baby. We must be careful in anesthetic management of the surgical separation of conjoined twins.

[A case of T-cell type malignant lymphoma of the lung].

A case of T-cell type malignant lymphoma of the lung was reported. A 69-year-old woman was referred to our institute because of an abnormal shadow on the chest roentgenogram. She was symptom-free, and physical examination showed no abnormalities. Her laboratory tests were within normal limits. The chest roentgenogram showed a well-defined, nodular density in the left lower lung field without air bronchogram. She underwent a left lower lobectomy and a mediastinal lymphnodes dissection. This disclosed an elastic hard nodule, 2 cm in diameter, without pleural invasion. Pathologically the tumor was diagnosed as malignant lymphoma of T-cell type without regional lymphnode involvement. She is doing well without any sings of recurrence for one year after the operation.

The "recall effect" in radiotherapy: is subeffective, reparable damage involved?

It has been proposed that lethal mutations among the progeny of a surviving cell could be the basis for the recall effect when chemotherapy is applied subsequent to the repair of normal-tissue injury resulting from a course of radiation therapy. Because radiotherapy is usually multifractionated, the possibility exists that repair of heritable injury of this type could occur between fractions as is the case for sublethal damage. To examine this possibility, the endpoint small-colony formation was used--an endpoint which integrates the effects of a number of radiation-induced aberrancies including lethal mutations--and low-dose-rate irradiation. It was found that, even after net surviving fractions comparable to those sought in radiotherapy were reached, little damage remained expressible as a deficiency in the size of the colony generated from a surviving cell. We conclude that the damage expressible as a lethal mutation is reparable and therefore the recall effect must be attributed to some other cellular mechanism.

[Effect of induction methods of pediatric anesthesia on serum myoglobin].

Two hundred and twenty nine children from infancy to middle teens divided into six groups were studied to determine the effect of induction methods of pediatric general anesthesia on serum myoglobin. After the slow induction with nitrous oxide-oxygen-halothane (GOF), the serum myoglobin increased significantly to 1774.7 +/- 4285.8 ng.ml-1 (mean +/- SD) by iv administration of succinylcholine 1 mg.kg-1. This significant increase of serum myoglobin was reduced by pretreatment with d-tubocurarine (dTc) 0.05 mg.kg-1 before administration of succinylcholine. However, it was not reduced by pretreatment with thiopental 3 mg.kg-1. After the rapid induction with thiopental 5 mg.kg-1 and succinylcholine 1 mg.kg-1, the serum myoglobin increased slightly to 387.6 +/- 596.5 ng.ml-1. But this increase in serum myoglobin was stopped by pretreatment with dTc 0.05 mg.kg-1 before the rapid induction. Serum myoglobin was not changed without succinylcholine after the slow induction with GOF. In each group, there was no relation with serum myoglobin and ages or fasciculation. Serum myoglobin was more influenced by the induction methods of pediatric general anesthesia than by ages.

[Study of hypothermic anesthesia--cardiac contractility during hypothermia].

We studied cardiac function during hypothermia, and effects of various methods of anesthesia on hearts taken from guinea pigs which received extracorporeal circulation by Langendorff method. Morphine, fentanyl citrate and halothane were used as anesthetics. Morphine and fentanyl citrate were added to the infusate at the dose of 3 mg.kg-1 or 50 micrograms.kg-1. Halothane 1% was introduced into the infusate by bubbling. Left ventricular peak dp/dt was measured as an index of cardiac contractility. Halothane depressed cardiac contractility most at normal temperature, and this was changed by cooling. Cardiac contractility with both morphine and fentanyl citrate was similar, and showed significant advantage over halothane at 30 degrees C, 25 degrees C and 20 degrees C. Coronary arterial blood flows at 30 degrees C, 25 degrees C and 20 degrees C were in the order of halothane greater than fentanyl citrate greater than morphine. The result suggests that oxygen consumption of myocardium under halothane anesthesia, even during hypothermia is high.

[An autopsy case of ovarian clear cell carcinoma 14 years after irradiation for uterine cervical carcinoma].

An autopsy case of a 46-year-old woman who developed right ovarian clear cell carcinoma 14 years after radiation treatment for uterine cervical cancer, is reported. The pathological characteristics of the clear cell carcinoma were solid, tubulo-cystic and papillary structures of clear, granular eosinophilic, hobnail , and vacuolated cells. A Review of the literature on ovarian clear cell carcinoma and multiple cancers is also presented.

Suppression of 7,12-dimethylbenz[a]anthracene-induced chromosome aberrations in rat bone marrow cells after treatment with Sudan III and related azo dyes.

Noninbred Long-Evans rats fed Sudan III at 24 hours before they were given an injection of 7,12-dimethylbenz[a]anthracene (DMBA) displayed prominently suppressed DMBA-induced chromosome aberrations (CA) in their bone marrow cells. Rats fed Sudan III simultaneously with the DMBA injection showed no suppressed CA effect. The suppressive effect of Sudan III on DMBA-induced CA paralleled the dose rate of Sudan III when given in the range between 1 and 10 mg Sudan III/kg body weight; higher doses produced no additional suppression. The capacity of various Sudan III-related chemicals to prevent DMBA-induced CA paralleled their capacity to prevent DMBA-induced adrenal apoplexy and mammary cancer. Among the azo dyes investigated, Sudan III was most efficient in protecting against DMBA-induced CA. Polychlorinated biphenyl and phenobarbital, inducers of drug-metabolizing enzymes, also suppressed DMBA-induced CA.
