Efficacy of doramectin against intestinal nematodes and sarcoptic manage mites in naturally infected swine.

Endectocidal efficacy of doramectin administered intramuscularly at a dosage of 300 micrograms/kg was evaluated in 464 pigs naturally infected with intestinal nematodes or mange mites on 14 commercial farms in Japan. By doramectin treatment, fecal egg counts were reduced > 99% for Ascaris suum, Strongyloides ransomi, Oesophagostomum dentatum, and Trichuris suis; worm counts of T. suis and mite counts of Sarcoptes scabiei reduced 90.1% on Day 21 and 99.5% on Day 28 following treatment, respectively.

Lengthening of short great toes by callus distraction.

We lengthened seven first metatarsals in four patients with short great toes by callus distraction using an external fixator. Good clinical and cosmetic results were obtained. Bone lengthening is effective in patients with short great toes not only for cosmesis, but also to relieve pain and callosities on the plantar aspect of the second and third metatarsal heads. Excessive lengthening of the first metatarsal resulted in limitation of the range of movement of the metatarsophalangeal joint of the great toe. To prevent this the amount of lengthening should not exceed 40% of the preoperative length of the metatarsal.

Gamma-delta T cells in BAL fluid of chronic lower respiratory tract infection.

Gamma-delta (gamma/delta) T cells are thought to represent the first line of defense against various pathogenic microorganisms. The aim of the present study was to investigate whether gamma/delta T cells were increased in BAL fluid (BALF) of patients with diffuse panbronchiolitis (DPB), a model of chronic lower respiratory tract infection. The study population consisted of four groups, including patients with DPB, sarcoidosis, idiopathic pulmonary fibrosis, and normal subjects. Two-color direct immunofluorescence and flow cytometry were used for analysis of peripheral blood or BALF from these patients. The percentage of peripheral blood or BALF gamma/delta T cells relative to the total number of lymphocytes was similar in the four groups. Although the absolute number of gamma/delta T cells in BALF was significantly higher in DPB patients compared with the other three groups, the total lymphocyte number in BALF in DPB patients was increased and the number of BALF gamma/delta T cells correlated with the total lymphocyte number in BALF. Furthermore, the percentage and number of BALF gamma/delta T cells were not related to a certain group of pathogenic organisms or the number of colony-forming units. Our results suggest that gamma/delta T cells are unlikely to play a part in chronic lower respiratory tract infection.

Maxillary growth following atelocollagen implantation on mucoperiosteal denudation of the palatal process in young rabbits: implications for clinical cleft palate repair.

OBJECTIVE: The implantation of atelocollagen matrix on the denuded surface of palatal bone following cleft palate repair has been used because it enhances wound healing. This study was performed to determine whether the beneficial effect of atelocollagen matrix implantation on the prevention of scar tissue contraction also inhibits the scar's interference with the growth of maxillary bone. METHOD: Fifty New Zealand White rabbits (aged 4 weeks) underwent palatal mucoperiosteal denudation, and etelocollagen matrix was implanted on the left palatal process. The opposite side was left open as a control. RESULTS: Histopathologically, the implantation side exhibited early infiltration of mononuclear cells and fibroblasts, and better growth of connective tissue strands and epithelium. In addition, the formation of rate ridges were seen that were similar to the normal mucosa. The bone of the atelocollagen-implanted side was covered with regenerated periosteum-like layers, but that of the control side was lined by granulation tissue, suggesting the existence of continuous inflammation on the periosteal region. When the animals reached adulthood (aged 24 weeks), the areas of scars and palatal processes, palatal shelf width, molar teeth incline, and bone mineral contents were measured and compared between sides. The atelocollagen-applied scars showed less contraction, the area and width of atelocollagen-implanted palatal processes showed more satisfactory growth, and the dental arch deformity was suppressed in comparison with the control side. CONCLUSIONS: Our results suggest that the use of atelocollagen matrix on the denuded bone surface following cleft palate repair decreases the scar's effect on maxillary growth.

Toxicological correlation between changes in blood biochemical parameters and liver histopathological findings.

The toxicological correlation between blood biochemical parameters and liver histopathological findings was summarized mainly in rats and dogs on the basis of our experiments and published papers. In rats and dogs with hepatocytic necrosis, GPT and GOT increased with a good correlation to necrotic severity. In dogs with cholestasis, ALP, gamma-GTP, T.BIL and BSP retention rates increased. In mixed types of hepatitis or cholestasis and hepatic necrosis, GPT, GOT and ALP increased in rats and dogs and additionally gamma-GTP and BSP retention rates increased in dogs. In hepatic steatosis, CHOL and TRIG decreased in rats and dogs. In hepatic injury due to accumulation of foreign materials or cell components and sinusoidal cell injury, no specific correlation with biochemical parameters was noted.

Bilateral adrenal lymphoma with neoplastic angioendotheliosis.

We report a case of bilateral lymphoma of the adrenal glands. A 61-year-old man was admitted to our hospital in clinical shock with anuria. Endocrine examination indicated a diagnosis of Addison's disease. Large tumors were found bilaterally in the suprarenal area, and a needle biopsy was performed, and the diagnosis was malignant lymphoma. Treatment with chemotherapy achieved a moderate response. He was discharged, but died 1 month later. On autopsy, microscopic examination showed extensive intravascular infiltration of lymphoma cells. We considered this to be a case of bilateral adrenal lymphoma with neoplastic angioendotheliosis.

[A clinical study of 1079 patients with laryngeal cancer].

A clinical study was made of 1079 patients with previously untreated laryngeal cancer registered in our department during the 15 years between 1979 and 1993. Their mean age was 64.2 years, ranging from 22 to 91. The male to female ratio was 13:1 and the proportion of non-smokers was 3.2%. These patients consisted of 687 with glottic carcinoma (64%), 373 with supraglottic carcinoma (35%) and 12 with subglottic carcinoma (1%). A sexual difference with regard to the proportion of glottic carcinoma was found; 66% in males and 39% in females. Therefore, the male to female ratios according to the primary sites were 22:1 for the glottis and 8:1 for the supraglottis. According to the TNM classification (UICC 1987), there was a marked difference in the distribution between glottic type and supraglottic type: of the glottic type 439 cases (64%) were stage I, 159 (23%) stage II, 73 (11%) stage III, 16 (2%) stage IV, whereas of the supraglottic type 22 (6%) were stage I, 99 (27%) stage II, 136 (36%) stage III, 116 (31%) stage IV. Our treatment policy for laryngeal cancer has depended not only on the T stage but also on the cancer characteristics including the growing type, anatomical location and extension and depth of invasion. Of the 1079 patients, 1050 were treated radically, 17 palliatively and 12 were not treated. According to the T stage, radical irradiation was chosen for 410 cases (93%) of glottic T1, for 75 cases (45%) of glottic T2 and for 21 cases (81%) of supraglottic T1. Partial laryngectomy was performed in 16 patients (4%) with glottic T1, in 15 patients (10%) with glottic T2 and in 8 patients (6%) with supraglottic T2. Total laryngectomy was performed in almost all patients with T3, T4. The overall 5-year relative survival rate, cumulative crude survival rate and cause-specific survival rate were 84%, 72% and 88%, respectively. The survival rates for the glottic type were significantly higher than those for the supraglottic type (p < 0.0001): the 5-year relative survival rate, cumulative crude survival rate and cause-specific survival rate for the glottic type were 93%, 80%, and 95% whereas for the supraglottic type they were 68%, 58% and 76%, respectively. The 5-year relative survival rates for stage I through stage IV were 96%, 87%, 72% and 53% overall and they were 98%, 91%, 74% and 52% for the glottic type, whereas they were 67%, 83%, 70% and 52% for the supraglottic type, respectively. The results suggested that the better survival rate for glottic T1, which accounted for 64% of all glottic cases, made a great difference in the survival rate between glottic and supraglottic carcinoma.

[Intraoperative continuous epidural lidocaine combined with preoperative administration of epidural morphine for post-hepatectomy pain relief].

In a randomized double-blind study, the use of continuous epidural lidocaine during surgery combined with preoperative epidural morphine was compared with that of preoperative epidural morphine alone for postoperative analgesia in 20 patients undergoing hepatectomy. Morphine 2 mg was administered through a catheter inserted epidurally at T10-11 before surgery, followed by continuous epidural administration of 1% lidocaine 5ml.h-1 in group Lid (n = 10) or normal saline 5ml.h-1 in group NS (n = 10) during surgery. Anesthesia was maintained with N2O-O2-isoflurane in both groups. On admission to the ICU, the visual analog scale score (VAS; mm) was 20 +/- 7 (mean +/- SE) in group Lid and 38 +/- 10 in group NS, and the number of patient with VAS < or = 30 was 9 in group Lid and 4 in group NS; these differences were significant (P < 0.05). Pain score during mobilization in group Lid was significantly lower than that in group NS (P < 0.05). All patients in both groups had adequate analgesia for the remainder of their stay in the ICU. No patient had any serious adverse effect. We conclude that continuous epidural administration of lidocaine during hepatectomy combined with administration of epidural morphine just before surgery results in better pain relief during the early postoperative period than that obtained with epidural morphine alone, and is without serious side effects.

[Antigenic epitopes recognized by autoantibodies to calpastatin in patients with rheumatoid arthritis and their clinical significance].

We have previously described that novel autoantibodies to calpastatin (endogenous inhibitor for calcium-dependent neutral protease, calpain) were detected in patients with rheumatoid arthritis (RA) and other disorders. Since calpain is thought to mediate inflammatory process and cartilage destruction, autoantibodies to its inhibitor protein, calpastatin, may be involved in the pathogenic mechanism of rheumatoid arthritis. In the present study, we analyzed antigenic epitopes reactive with autoantibodies to calpastatin and their clinical correlation. cDNA encoding the C-terminal 178 amino acids of human calpastatin (RA-6) was digested by restriction enzymes and ligated in to pEX expression vectors. These recombinant plasmids were tranfected into E. coli POP2136 and screened by colony blots using RA sera containing anticalpastatin antibodies and a mouse monoclonal antibody. RA patient sera recognized the C-terminus of domain IV (epitope C1 ; aa. 647-673) and C-terminus of domain III (epitope C2 ; aa. 496-571), whereas the mouse monoclonal antibody recognized an entirely different region containing the calpain-binding site (epitope B2 ; aa. 572-625). To evaluate epitope reactivity of patient autoantibodies, 15 RA sera containing anti-calpastatin were reacted with epitope fusion proteins. In immunoblotting, most RA sera recognized either C1 or C2 epitopes (67% and 40%, respectively), and only one patient recognized both epitopes. B2 epitope a more progressed and sever state of arthritis than those not reacting with C1. These results suggests that anti-calpastatin antibodies may play a role in the pathogenic mechanisms of RA and their epitope reactivity may be important for disease progression.

[Jargon agraphia in a case with Broca aphasia: dissociation between language and writing dominancy].

A 69-year-old left-handed man developed Broca aphasia and jargon agraphia following a right cerebral infarction. He learned to write with his right hand. There is no family history of left-handedness. Neurological examination revealed hemiparesis, sensory impairment, and hyperreflexia with pathological reflexes on the left side. An MRI demonstrated lesions in the area including the pre- and post-central gyri, the posterior part of the middle and inferior frontal gyri, and inferior parietal lobule on the right side. Neuropsychologically he was alert and cooperative. He demonstrated severe Broca aphasia and his verbal output was limited to a few residual words. On the other hand, he could communicate through writing Kanji words. Writing words was relatively preserved with Kanji but not with Kana characters. Asked to write a Japanese folk tale, he showed jargon agraphia mixed with some correct Kanji words. His comprehension of spoken and written sentences was well preserved. He showed mild ideomotor and buccofacial apraxia, left unilateral spatial neglect, and constructional impairment. Dissociation between spoken and written language is a prominent feature of this patient's language output. In this patient, right hemisphere seems to be dominant for language and praxis, and left hemisphere for motor engram of writing. Learning to write with the right hand may have enhanced the establishment and maintenance of motor engram for writing in the left hemisphere. The intact motor engram of characters in the left hemisphere could be retrieved by the right hand without control from the language area in the diseased right hemisphere, resulting in jargon agraphia especially with Kana characters. In left handed people, the hand with which they learn to write may effect interhemisphere lateralization of language functions.

Studies on the first-pass metabolism of ebastine in rats.

The metabolic conversion of ebastine (CAS 90729-43-4, LAS-90), an antiallergic agent, to its active principle carebastine (CAS 90729-42-3) in the rat intestine and liver was investigated using intravenous-intraportal infusion techniques and jejunum loop preparations. The steady state blood concentrations of ebastine and carebastine were determined during continuous intravenous or intraportal infusion of ebastine to evaluate their respective activity to metabolize ebastine in the intestine and liver. Total body clearance of ebastine was calculated to be approximately 22-26 ml/ min. The intestinal and hepatic clearances were 6.7 ml/min and 15.4 ml/min, respectively, accounting for about 32% and 60% of the total clearance, respectively. The ratio of the concentrations of carebastine in portal blood to that in arterial blood was 1.41 during intravenous infusion, suggesting the single-pass metabolic conversion of ebastine to carebastine in the intestine. The ratio of the arterial blood concentration of carebastine during intraportal infusion to that during intravenous infusion was 1.88, suggesting the single-pass metabolic conversion in the liver. The contribution of the intestine to form carebastine from ebastine present in the systemic circulation was thus about 1/2 (0.41/0.88) of that of the liver under these conditions. When [14C]ebastine was administered in the jejunal loop, carebastine was detected in the mesenteric plasma circulated from the loop, as the major component accounting for approximately 56% of the plasma radioactivity, while the unchanged ebastine was only about 13%. Therefore, the jejunal tissue converted > 1/2 of the permeated fraction of ebastine to carebastine under these conditions. The results in the infusion studies suggested that metabolic potential to convert ebastine to carebastine was higher in the liver than in the intestine. However, since after oral administration all of the drug appeared in the systemic circulation firstly permeated the mucosa of small intestine and then passed through the liver, the contribution of the small intestine in the metabolic conversion of ebastine given orally would be greater than that of the liver, as suggested by the above in situ jejunum loop study.

[Thermoradiotherapy combined with daily administration of low dose cisplatin for locally advanced laryngeal carcinoma].

We applied thermoradiotherapy combined with daily administration of low dose cisplatin (CDDP) to five patients with locally advanced laryngeal carcinoma. The total response rate (CR + PR) was 100% after irradiation of 30 or 40 Gy. One case showed CR, and the larynx could be preserved by adding a full dose of irradiation. CDDP administration was discontinued in two cases due to renal dysfunction or thrombopenia, but other cases tolerated the therapy without severe side effects. Total laryngectomy and bilateral neck dissection was carried out in four cases after irradiation of 30 or 40 Gy. Removed larynxes were prepared for light microscopic observations of the serial sections. One of the four specimens revealed a CR histologically. The combination of thermoradiotherapy with low dose CDDP showed a remarkable effect on reduction of tumor size and cytotoxicity of tumor cells. This might contribute to saving the larynx of patients with locally advanced laryngeal carcinoma.

[Blood biochemical properties in male workers analysed according to body type].

The characteristics of body type, serum lipids and blood pressure in male blue workers were evaluated. The subjects were 1,705 healthy men aged 40.3 +/- 4.33 years, and were classified into 4 groups according to body mass index: BMI; lean (group L: BMI < 20), normal (group N: 20 < or = BMI < 24), mildly obese (group MO: BMI 24 to 26.5) and obese group (group O: BMI > 26.5). The physical characteristics, blood pressure and 14 blood constituent items were measured. Groups L, N, MO and O accounted for 8.5, 53.4, 28.6, and 9.4% of all subjects, that is the obese body type (MO and O groups) occupied 38%. Rest systolic and diastolic blood pressure were positively related to BMI. Most blood constituent items biochemically analyzed increased in proportion to BMI values. The Trigriceride level was 88.5, 112.2, 152.8 and 171.9 mg/dl in groups L, N, MO and O, respectively. The Trigriceride level in groups MO and O were about the upper limit of the normal range. The total cholesterol level was in the normal range in 4 groups, but was higher in the MO and O groups than in groups L and N. The HDL-cholesterol level was low in inverse proportion to BMI, that is 58.2, 51.3, 47.6, and 42.6 mg/dl in groups L, N, MO, and O, respectively. Especially the values in group O were similar to the values in the aged and adult disease patients. A greater percentage of Japanese male workers were obese men and they risked developing atherosclerosis and adult diseases. the present study suggests that health education regarding exercise prescription and nutrition intake in daily life should be provided.

[Relationship between exercise-induced hypoxemia and long-term survival in patients with chronic obstructive pulmonary disease].

In patients with chronic obstructive pulmonary disease (COPD), exercise-induced hypoxemia (EIH) is an important limiting factor of exercise performance, but there are very few reports of the prognostic value of EIH. We therefore examined whether EIH is related to long-term outcome in patients with COPD. We gathered data on survival of 77 patients with COPD who had undergone three-minute incremental treadmill exercise tests between 1979 and 1988. A plastic catheter was placed percutaneously into a brachial artery to measure arterial blood gases at each stage of exercise. Expired gas was analyzed continuously during exercise with a Respiromonitor RM-300 (Minato Medical Science). As an index of the severity of EIH we used the delta PaO2/delta VO2 (mmHg/L/min), PaO2-slope. Patients were assigned to two groups according to PaO2-slope, and actuarial survival curves were plotted for the two groups. The survival data were analyzed by generalized Wilcoxon analysis. Survival was significantly better in the group with low PaO2-slopes (< 20 mmHg/L/min) than in the group with high PaO2-slopes (> or = 20 mmHg/ L/min) (p = 0.0031). Also, among the patients in whom PaO2 during exercise was less than 60 mmHg, there was a significant difference in survival between those who received home oxygen therapy and those who did not. We conclude that the degree of EIH can be used to predict survival in patients with COPD, and that the degree of EIH should be considered when prescribing continuous home oxygen therapy for these patients.

[Histopathological study in models of chronic pancreatitis].

UNLABELLED: Histopathological findings were examined in the models of chronic pancreatitis. Using mongrel dogs, we prepared a control group (group C), a chronic ischemic group (group I), an alcohol administration group (group A), a duct obstruction group (group O), and an alcohol + obstruction group (group AO). Group I showed severe inflammatory cell infiltration, fibrosis, fat replacement and loss of acinar cells. Group A showed no change. In group O, mild periductal fibrosis was recognized. Group AO showed moderate inter-lobular fibrosis and inflammatory cell infiltration, resembling those of human chronic alcoholic pancreatitis. CONCLUSION: 1) Histological findings of chronic ischemic group is severer than that of group O and AO. 2) The model of alcohol administration with incomplete duct obstruction is a useful model of human chronic alcoholic pancreatitis.
