In 29 cases undergoing lung resection, effects of the surgery on left ventricular function were investigated indirectly with a Nuclear Stethoscope. Various parameters were measured following an exercise load before and after surgery. There were significant decreases in post-operative resting levels of stroke volume (SV) (p < 0.001), end-diastolic volume (EDV) (p < 0.001), ejection fraction (EF) (p < 0.05) and ejection rate (ER) (p < 0.001) and significant increase in heart rate (HR) (p < 0.001) when compared to pre-operative resting levels. Neither filling rate (FR) nor cardiac output (CO) showed significant difference. At maximum exercise load, there were significant decreases in post-operative EDV (p < 0.005), SV (p < 0.005), ER (p < 0.001) and FR (p < 0.005), but no significant differences were detected in HR and EF; consequently, there was a significant decrease in CO (p < 0.005). Ratio of the levels at maximum load to those at resting of each parameter did not show significant difference between before and after operation with regard to any parameters except CO and FR which showed significant decrease (p < 0.005 and p < 0.001, respectively). Effects of the surgery on left ventricular function were studied according to amount of lung resection. In 13 cases where more than two lobes were resected, similar significant differences to those mentioned above were found in all parameters except EF. In cases where a single lobe was resected, only ER and FR showed similar tendency to that described above. Effects of the surgery on left ventricular function were also studied according to age of patients.(ABSTRACT TRUNCATED AT 250 WORDS)
Differences in plasma thiobarbituric acid (TBA) levels were studied by sex and age in 1,178 adults (471 males and 707 females) participating in a mass health examination conducted in a town of Kumamoto Prefecture in 1989. The relationships between plasma TBA levels and obesity, the consumption of cigarettes, alcohol, and fish, and various indices for health status derived from the data of physiological examinations were analyzed in age-groups 40-59 and 60-79 for both sexes. The prevalence of abnormal physiological findings by plasma TBA levels were further investigated. The results were as follows; 1. The highest plasma TBA levels for males were observed in the group aged 40-49, with levels gradually decreasing with age thereafter. The plasma TBA levels for females increased with age and the highest levels were obtained at age 60-69. The plasma TBA levels for males were significantly higher than for females at aged 40-49. 2. Plasma TBA levels for both sexes increased with obesity and the amount of fish consumed. 3. Plasma TBA levels in all examined groups had significant and strong positive correlations with serum total cholesterol levels and atherogenic indices, and had significant positive correlations with serum calcium and gamma-GTP levels. 4. A stepwise multiple regression analysis showed that plasma TBA levels for males were associated with serum total cholesterol, gamma-GTP, fish consumption and serum HDL-cholesterol in that order. The levels for female were associated with serum total cholesterol, body mass index, serum HDL-cholesterol and serum calcium in that order. The relationship with serum HDL-cholesterol was a negative correlation. 5. Prevalence of abnormal values of serum total cholesterol, atherogenic index and body mass index were significantly higher in the high-plasma TBA level group than in the group with median-plasma TBA level. These results suggest that plasma TBA levels may be useful as an index of undesirable lipid metabolism. Further studies should be performed to establish the significance of the relation of plasma TBA levels to fish consumption.
A case of giant metastatic liver VIPoma manifesting WDHA syndrome is described. The patient was a 33-year-old Japanese male who was admitted because of persisting watery diarrhea and weight loss. Laboratory examinations revealed marked hypokalemia, mild hypercalcemia and highly elevated plasma vasoactive intestinal peptide (VIP) and glucagon levels. Intensive imaging examinations disclosed two large tumors in the liver but not in other organs, including the pancreas. Laparoscopy disclosed a slightly elevated purplish-colored large tumor on the surface of the apparently normal liver. At operation, however, a small hard nodule was observed in the tail of the pancreas. Histologically, it was revealed to be a VIPoma resembling a carcinoid or an islet cell tumor, and was identical to the liver tumors. Thus, this neoplasm was considered to be a VIPoma of pancreatic origin, with metastatic lesions in the liver.
The relation between submaximal oxygen consumption (MVO2) before and after lung resection was studied, and the possibility to predict postoperative MVO2 was investigated. Thirty four patients (19 lobectomies, 9 bi-lobectomies, 6 pneumonectomies) performed exercise tests one week before operation and at the time when they were able to resume work daily after operation (in two to eleven months). Predicted postoperative MVO2 was calculated from preoperative MVO2 as follows: 0.8 x resection rate x age index x preoperative MVO2 + 183. In this formula the resection rate means [the number of total lung segments--the number of non-working segments]. The age index is 1.5 for patients under 50 years, 1.0 for those between 50 to 69, and 0.8 for those over 70. This predicted MVO2 had strong correlation with actual postoperative MVO2 (r = 0.94, t = 13.4, y = 1.08x - 83) in twenty five patients. Correlation was low in the remaining nine patients, however. The nine had old myocard infarction, postoperative cerebral infarction, marked wet lung, anticancerous chemotherapy within ten days or marked reduced exercise capacity (below 3 METS at maximal exercise). From these results, we concluded it is possible to predict the postoperative MVO2 using preoperative MVO2 except in cases which have some exercise limiting factors or whose exercise capacity is very small.
In surgical treatment for lung cancer, large amount of intrabronchial sputum and secretion was occasionally encountered in patients, especially underwent bronchoplastic surgery. It has been discussed as causes which was affected from transection of pulmonary branches of vagal nerve and bronchial arteries. So we have measured pre- and postoperative hemodynamics and extravascular lung water (EVLW) at the bedside, as to investigate the correlation with formyl sputum and increase of right ventricular afterload which was presumed by lung resection and bronchoplasty. Respiratory complication was classified by the incidence of forced bronchial toilet in acute phase. Seven patients were investigated. Three of seven showed white sputum which was difficult to excrete. EVLW was 9.91 +/- 2.12 ml/kg in average before operation and then showed a tendency to increase within 102 +/- 29% of percent exchange (delta %) between pre- and postoperation. As to comparison of delta % of EVLW in acute period, the patients underwent bronchoplastic surgery showed a tendency to increase rather than the patients underwent lobectomy. According to former studies, we found the uniform correlation R = 0.7075 between observed EVLW and predicted postoperative EVLW estimated by prediction of residual functioning lung volume before operation. And also, the patients showing postoperative EVLW more than predicted EVLW before operation had a tendency to increase amount of sputum. Furthermore, concerning hemodynamic changes, pulmonary arterial resistance index (PARI) elevated to 196 +/- 165% (83-515%) in average associated with increased right ventricular stroke work index (RVSWI) as a result of compensatory changes for increased right cardiac afterload.(ABSTRACT TRUNCATED AT 250 WORDS)
alpha-Rat atrial natriuretic peptide (alpha-rANP) was synthesized by assembling five peptide fragments in solution, followed by HF-dimethylselenide-m-cresol deprotection and subsequent air-oxidation. Synthetic alpha-rANP exhibited more potent diuretic and natriuretic activity in rats than synthetic alpha-hANP.
Alpha-human atrial natriuretic peptide (alpha-hANP) was synthesized by assembling six peptide fragments in solution followed by deprotection with HF and subsequent air-oxidation. The trimethylbenzyl group was employed as an S-protecting group of cysteine. The HF-dimethylselenide-m-cresol system was employed as a final deprotecting reagent and, at the same time, as a reducing reagent of Met(O). Synthetic alpha-hANP elicited potent diuretic and natriuretic activity in rats.
We studied the correlation between the complications after pulmonary resection and the results of exercise testing and others. The exercise testing was performed preoperatively on 64 patients; 44 lobectomies, 6 bilobectomies and 14 pneumonectomies. The results which had the correlation with postoperative complications were RV/BSA (p less than 0.001), RV/TLC (p less than 0.005), VE/VCO2 (p less than 0.001) and VO2/VE (p less than 0.001). Men had more complications than women (p less than 0.001). And the patients with the past histories concerning cardiovascular disease, pulmonary disease and/or diabetes had more chances of complications than the patients without those histories (p less than 0.05). Judging from these results we concluded that the patients with one or more of the following results had more chances of postoperative complications. 1. maximum VO2/VE was less than 40 ml/l. 2. VE/VCO2 was more than 4.0 l/ml. 3. RV/BSA was more than 1.5 l/m2. 4. RV/TLC was more than 45%.

Since 1971, we have treated 76 patients with metastatic bone tumor. Forty-eight of 76 cases were treated surgically. In 34 cases, removal of metastatic foci of the spine was attempted from the anterior and/or posterior approach. In the remaining 14 cases, resection of the tumor with or without prosthetic replacement, osteosynthesis, or amputation was performed according to the site of bone metastasis. In cases of spinal metastasis, if foci were located anteriorly, resection and replacement of vertebral body by ceramic spacer and/or bone cement was effective for both relief of pain and reduction of paralysis. If foci were located posteriorly, however, the results of simple laminectomy were poor, and those of spinal instrumentation in addition to laminectomy were satisfactory. In the treatment of metastatic foci of pelvis, scapula and long bone, the results of orthopedic surgery were satisfactory, because relief of pain and maintenance of function were achieved in almost all cases. From the standpoint of prolongation of life, 15 out of 76 cases (20%) were alive with or without disease; 13 of them were treated not only surgically but also by radiation and/or chemotherapy.
Seventy-two strains of Hg-resistant bacteria (Pseudomonas) were isolated on agar plates containing 40 micrograms/ml of HgCl2 from Minamata Bay sediment, which was heavily polluted with mercury (45.8 micrograms/g). The minimal inhibitory concentrations (MICs) of mercurial compounds were determined for the Hg-resistant pseudomonads and 65 strains (Pseudomonas sp., Bacillus sp., Vibrio sp., and Corynebacterium sp.) isolated from Sendai Bay sediment (1 microgram/g of mercury) as control. The MICs to HgCl2, CH3HgCl, C2H5HgCl, C3H7HgCl, and C6H5HgOCOCH3 for the Hg-resistant pseudomonads from Minamata Bay were significantly higher than those of strains from Sendai Bay. The volatilization from liquid culture containing 20 micrograms/ml of HgCl2 was observed in all of the Hg-resistant pseudomonads from Minamata Bay (70 strains). The mean loss of mercury from liquid culture was 60.4 +/- 17.3%. Further study is warranted to determine what role the Hg-resistant bacteria, particularly the Pseudomonas species, play in the mercury cycle in Minamata Bay.

Methylmercury chloride was administered PO to male Kud: ddY mice at a dose of 5 mg/kg/day for 20 days. The contents of taurine, aspartate, glutamate, glycine, and gamma-aminobutyric acid were determined in tissue and crude synaptosomal (P2) fraction of cerebellum, cerebral cortex, and spinal cord of methylmercury-treated mice with or without ataxia. In the cerebellum of ataxic mice, increased levels of taurine and glycine were found in the tissue and P2 fraction, and increased levels of glutamate were found in the P2 fraction. In the cerebral cortex, the levels of gamma-aminobutylic acid decreased in the tissue and in the P2 fraction of ataxic mice, but increased levels were found in the tissue of non-ataxic mice. A decreased aspartate level in the cerebral cortex of ataxic mice and an increased taurine level in the cerebral cortex of non-ataxic mice were also found. In the spinal cord of ataxic mice, taurine increased in the tissue and in the P2 fraction. Glutamate level decreased in the spinal cord of ataxic mice, but increased in the P2 fraction of non-ataxic mice. Increased glycine levels in the P2 fraction of the spinal cord were also found in non-ataxic mice. Histologically, some degenerative changes were demonstrated in the cerebral and cerebellar cortices of ataxic mice. Such changes were also present to a mild degree in non-ataxic mice. In conclusion, methylmercury treatment altered the levels of putative neurotransmitter amino acids in neural tissue of mice. These alterations might be caused by specific neural cell dysfunction and could be related to the appearance of ataxia.
