[Value of cancer-related oncoprotein expression as prognostic factors in breast-cancer with one to three axillary lymph nodes positive].

The patient group with breast carcinoma with one to three axillary lymph-node metastases (n1-3) shows a poorer prognosis than the node-negative one but a better prognosis than the group with 4 or more lymph node metastases. Univariate analysis shows that loss of bc1-2 expression, nuclear accumulation of p53, overexpression of c-erbB-2, age at diagnosis, menstrual status, tumor size, histologic grade, number of lymph nodes involved, and estrogen receptor were all significantly associated with the survival rates. Multivariate analysis demonstrated that menstrual status, number of involved lymph nodes, bc1-2, and p53 were significant predictors for overall survival, and that histological grade and number of nodes involved were significant predictors for disease-free survival. Combination of those factors would be useful to select highly aggressive n1-3 breast cancer group.

[A case report of surgical treatment for left ventricular papillary fibroelastoma].

A case of surgical treatment for left ventricular papillary fibroelastoma was reported. The patient was 75-year-old male who was admitted with complete A V block. Transesophageal echocardiogram revealed a mobile rounded mass (15 mm x 15 mm) in the LV attached to the IVS. On the basis of this echo finding, operation was performed. Under cardiopulmonary bypass, an incision was made in the ascending aorta and the tumor was successfully removed through the aortic valve. The postoperative course was uneventful. To our knowledge, our case is the 10th surgical case in the Japanese literatures.

Higher Level of P-glycoprotein Expression in Human Breast Cancer Cells after Radiation Therapy.

We investigated the effects of radiation on the expression of p53 protein and P-gp in breast cancer cells. Thirty-two preoperatively irradiated surgical specimens of stage I primary breast cancer were examined, and 39 surgical specimens of non-irradiated stage I breast cancer were selected as a control group. p53 and P-gp expression was detected by an immunohistochemical technique using rabbit polyclonal antibodies (RSP-53 and NCL-pGLYPp). The expression of p53 protein, defined as a nuclear immunoreaction in 20% or more of tumor cells, was detected in 4(13%) of the 32 cases in the irradiated group and 7 (18%) of the 39 cases in the non-irradiated group without any statistically significant difference. The expression of P-gp, defined as the presence of strong staining in more than 75% of the tumor cells, was detected in 19 (59%) of the cases in the irradiated group and 12(31%) of the cases in the non-irradiated group. P-gp expression was significantly higher in the former group than in the latter (P <0.05). There was no significant difference in P-gp expression between the stromal invasive component and the intraductal component in either the irradiated or the non-irradiated group. Radiotherapy possibly induces membranous P-gp expression in human breast cancer cells.

Germline mutation of BRCA1 in Japanese breast cancer families.

We analyzed germline mutations of the BRCA1 gene in 18 Japanese breast cancer families and two Japanese breast-ovarian cancer families. In two site-specific breast cancer families, the same mutation was detected; a nonsense mutation at codon 63 encoding a truncated small protein. It was demonstrated that the mutant allele cosegregated with breast cancer patients within a family and was absent in healthy Japanese, suggesting a breast cancer-predisposing allele. The average age at diagnosis was 44 and 55 years in each family with BRCA1 mutation. No bilateral breast cancer patients were present in the BRCA1 mutation-positive families, although five were present in the BRCA1-negative families. No germline mutations of BRCA1 were detected in the two breast-ovarian cancer families examined in this study, although BRCA1 mutation plays a major role in breast-ovarian cancer families in Western countries. Thus, the proportion of families who inherit the mutated BRCA1 allele seems to be small among Japanese breast cancer families and Japanese breast-ovarian cancer families.

Adjuvant six cycles of high-dose adriamycin, cyclophosphamide, methotrexate, 5-fluorouracil (ACMF) vs. 12 cycles of low-dose ACMF with tamoxifen for premenopausal, node-positive breast cancer patients: results of a prospective randomized study.

A prospective randomized study was conducted to compare the adjuvant efficacy of six cycles of high-dose ACMF (Adriamycin, ADM; cyclophosphamide, CPA; methotrexate, MTX; 5-fluorouracil, 5-FU) with that of 12 cycles of low-dose ACMF in premenopausal, node-positive breast cancer patients. The six-cycle ACMF group (93 patients) received, intravenously (i.v.), 130 mg/m2 CPA, 26 mg/m2 MTX, and 600 mg/m2 5FU on days 1 and 8, and 26 mg/m2 ADM on day 1 of each cycle. The 12-cycle ACMF group (97 patients) received i.v. 65 mg/m2 CPA, 13 mg/m2 MTX, and 300 mg/m2 5-FU on days 1 and 8, and 13 mg/m2 ADM on day 1 of each cycle. These treatments were repeated every 4 weeks, and all the patients took tamoxifen (30 mg/day) for 2 years. The background factors of the two groups were comparable. There were non-significant trends toward better disease-free and overall survival rates in the high-dose, six-cycle ACMF group. Both treatments were well tolerated, but more patients in the low-dose, 12 cycle group refused to continue to receive chemotherapy. These data suggest that escalating doses of ACMF over a shorter period, even with doses within the conventional range, are superior to low-dose prolonged therapy.

Isolation of a serotype G6P11 bovine rotavirus showing two-way cross-neutralization with the serotype G10P11 virus.

Four stains designated as OB94-1 to OB94-4 of group A bovine rotavirus (BRV) were isolated from 35 fecal samples of calves with diarrhea in sporadic outbreaks. In VP7 (G) and VP4 (P) serotyping of these isolates, OB94-1 to OB94-3 were determined as G6P5, G6P5 and G10P5, respectively, by cross neutralization (NT) test and the G- and P- serotyping polymerase chain reaction (PCR) analysis. OB94-4 showed a one-way antigenic relation with the Lincoln stain (G6P1) and a weak antigenic relationship with the KK3 strain (G10P11), and was determined as G6P11 by the PCR method. Thus, OB94-4 was shown to be a new G6 BRV with different antigenic properties from the others in the NT test.

Relation of family history of cancer and environmental factors to the risk of colorectal cancer: a case-control study.

The relation of a family history of cancer and environmental factors to colorectal cancer was investigated in a case-control study conducted from 1992 to 1994 at 10 medical institutions in Japan using a self-administered questionnaire, and 363 cases of colorectal cancer were compared with 363 controls matched for sex and age. A family history of colorectal cancer was positively associated with colon cancer (odds ratio (OR) = 2.0, 95% confidence interval (CI) 1.03-3.87) and rectal cancer (OR = 2.1 CI 0.94-4.48), but a family history of other cancers did not increase the risk. The proportion of patients with a family history of colorectal cancer within first-degree relatives was 12.4%--appreciably higher than figures previously reported in Japan. On the other hand, the incidence of hereditary non-polyposis colorectal cancer was 1.4%, and lower than previous estimates. Among dietary factors, a western-style diet significantly increased the risk of both colon and and rectal cancer (OR = 2.3 CI 1.30-3.88 and OR = 2.1 CI 1.26-3.63, respectively). Consumption of rice was protective against both colon and rectal cancer (OR = 0.5 CI 0.31-0.82 and OR = 0.3 CI 18-0.65, respectively). Animal meat, oily food, fish, vegetables and fruit were shown to affect the risk, but no statistically significant correlation was found. Among other factors, constipation increased the risk of colon cancer (OR = 2.0 CI 1.02-3.76) and consumption of coffee raised the risk of rectal cancer (OR = 1.7 CI 1.07-2.82). Our findings suggest that a family history of colorectal cancer is an important risk factor for this disease, and does not contradict the hypothesis that the risk of colorectal cancer in Japan may be influenced by westernization of lifestyle. However, we were unable to find conclusive evidence that familial clustering of this disease is strongly affected by environmental factors or genetic diseases such as hereditary non-polyposis colorectal cancer.

[A case report of an operation for graft stenosis with complete obstruction of the coronary artery].

A case report of an operation after CABG and AVR was presented. The patient was a 52-year-old female. She underwent CABG with saphenous veins at 43 years old and AVR at 45 years old. She was admitted to our hospital due to acute myocardial infarction. Coronary angiography revealed that all the native coronary arteries were occluded at the proximal side, two grafts to RCA and CX were occluded, and LAD graft had a 99% stenosis. She became critically ill due to low cardiac output and acute renal failure. Endoartrectomy of the LAD graft was performed under CPB. Early postoperative course was uneventful. Severe ST depression in the pre-operative ECG normalized in the postoperative ECG. But she had a chest pain again in the 4th postoperative week. She became critically ill and died on the 43rd postoperative day. It was thought that redo CABG should have been performed after her condition improved.

[Problems of breast conservation therapy].

Breast conservation therapy (BCT) is now considered an appropriate method to treat stage I-II breast cancer. However, local recurrence after BCT is found in at least 5-10% of the patients. We discussed the problems of BCT, including prognostic factors for local recurrence, optimal surgical technique, management of ductal carcinoma in situ, the clinical usefulness of postsurgical radiotherapy, and the indications for induction chemotherapy for more advanced cases.

[Familial breast cancer].

Japanese breast cancer families were collected and classified into the following 7 types according to the onset age and the distribution of other cancers in the family lines; early-onset type, late-onset type, familial breast-ovarian cancer type, familial breast-prostate cancer type, familial breast-thyroid cancer type, familial male and female breast cancer type and multiple primary cancer type. We have detected no p53 germ line mutations in the patients from these families. Linkage with BRCA1 was not detected in any single families. These data indicate that neither BRCA1 or p53 is a major susceptible gene in Japanese familial breast cancer. However, in the two site-specific breast cancer families, the same nonsense mutation of the BRCA1 gene was detected.

Pattern of gene alterations in intraductal breast neoplasms associated with histological type and grade.

To reveal any association between the histological type and grade of intraductal breast neoplasms and the manner of accumulation of gene alterations, eight types of gene alterations, i.e., loss of heterozygosity (LOH) on chromosomal arms 16p, 16q, 17p, 17q, and 18q, amplification of the c-erbB-2 and hst-1/int-2 genes, and mutation of the p53 gene, were examined by Southern blot analysis or single-strand conformation polymorphism analysis in a total of 60 cases of intraductal breast cancer and 18 nonmalignant proliferative lesions. Among the histological types and three histological grade groups of intraductal carcinomas, the gene alterations which occurred most frequently were LOH on 16q alone in non-comedo type and Grade 1, alterations of c-erbB-2, 17p, and 16q in comedo type and Grade 2, and alterations of 17q and p53 as well as those of 16q, 17p, and c-erbB-2 in Grade 3. LOH on 16q and 18q was frequent in intraductal carcinoma of the intracystic papillary type, whereas LOH on 18q alone was detected in 27% of papillomas. Among intraductal carcinomas, the mean number of gene alterations was largest in comedo type and Grade 3, whereas it was smallest in non-comedo type and Grade 1. It was possible that LOH on 18q and 16q was involved frequently in papillary tumori-genesis and acquisition of malignant phenotype, respectively, whereas most of the other gene alterations were involved in acquisition of aggressive biological properties by intraductal carcinoma cells. It was also possible that the phenotype of breast neoplasms was determined by the combination of gene alterations at a relatively early developmental stage.

Bilateral Breast Cancer in a Patient with a Strong Family History of Cancer: Analysis of p53 Germ Line Mutations.

We report a case of bilateral breast carcinoma in a patient with a strong family history, including 4 cases of breast carcinoma, 1 case of prostate carcinoma (father), 1 case of hepatocellular carcinoma (mother), 2 cases of gastric carcinoma, 1 case of lung carcinoma, and 1 case of lingual carcinoma, in second degree relatives, together with analysis of germ line p53 mutations. The patient was a 51-year-old female who had undergone mastectomy 9 years previously for an invasive ductal carcinoma of the right breast. Lymph nodes were free of metastases and the tumor had negative estrogen receptor (ER) status. Bone and lung metastases developed 18 months after surgery, and had been well controlled with chemoendocrine therapy. She subsequently underwent a modified radical mastectomy for carcinoma in the contralateral breast. This was an invasive lobular carcinoma with negative lymph node metastasis, negative p53 immunoreaction, negative c-erbB-2 protein and positive ER status. In this breast-prostate carcinoma-type cancer family there was a high incidence of breast carcinoma; the father, who had prostate carcinoma, was possibly a carrier of a breast carcinoma susceptible gene. We have however detected to p53 germ line mutations in the lymphocytes DNA of the patient and her niece. The accumulation of cancers in this family line remains to be elucidated further using other genetic markers.

Allele loss on chromosome 16q24.2-qter occurs frequently in breast cancers irrespectively of differences in phenotype and extent of spread.

Loss of heterozygosity on chromosomal arm 16q has been shown to be a frequent event in sporadic breast cancer and is suggested to be involved in cancer development through inactivation of a tumor-suppressor gene. To specify the commonly deleted region in which the unknown tumor-suppressor gene is located, a deletion map of chromosome 16 was constructed for 78 breast cancers, using 27 polymorphic DNA markers. Loss of heterozygosity on chromosome 16q was detected in 38 of the tumors. From the deletion map, the incidence of the loss of heterozygosity was deduced to be > or = 36% in the region distal to 16q12 and was most frequent in the 16q24.2-qter region. Then, association of the loss of heterozygosity in the 16q24.2-qter region with clinicopathological parameters of the tumors was examined for a total of 234 tumors, to reveal its biological significance in breast cancer development. The total incidence of loss of heterozygosity in the 16q24.2-qter region was 52% (118 of 225), and loss of heterozygosity was frequent irrespectively of the presence of invasion and metastasis, differences in clinical stage, tumor size, histological grade, or type, or amounts of estrogen receptor. Inactivation of an unknown tumor-suppressor gene on 16q24.2-qter was thus suggested to be involved commonly in the genesis of sporadic breast cancer, irrespectively of the extent of tumor spread or grade of aggressiveness of the cancer cells. On the other hand, eight cases revealed loss of heterozygosity not at 16q24-qter but in more proximal regions. Therefore, it appears that multiple tumor-suppressor genes are located on chromosome 16q.

Bilateral malignant lymphoma of the breast in patients with a familial history of cancer: a report of three cases.

We report three cases of bilateral malignant lymphoma of the breast in patients with a familial history of cancers. All the patients had B-cell malignant lymphoma and were treated by a radical mastectomy and polychemotherapy. Bilateral tumors were detected synchronously in one case, whereas the second tumors in the other cases occurred 4 years and 8 years after the initial treatment. Although the patients had no relevant past history, these cases were notable for the fact that all were associated with a familial history of cancers. One patient had a definite familial history of breast cancer, that is to say, her mother, sister and brother all suffered from this disease. The brother of another patient died of gastric cancer while two of her uncles each suffered from sarcoma of the lower extremities and a brain tumor, respectively. As for the remaining patient, one of her maternal uncles and an aunt also died of prostate cancer and leukemia, respectively. However, only one out of seven patients with unilateral malignant lymphoma of the breast used for comparison had a familial history of cancer. A possible relationship between bilateral primary malignant lymphoma of the breast and a familial history of cancer is suggested.

Different incidence of loss of heterozygosity on chromosome 16q between intraductal papilloma and intracystic papillary carcinoma of the breast.

Loss of heterozygosity (LOH) on chromosome 16q was examined in DNA isolated from 11 intraductal papillomas and 12 intracystic papillary adenocarcinomas of the breast. Such LOH was detected in 8 (67%) out of 12 intracystic papillary adenocarcinomas, and in 7 (64%) of 11 of these adenocarcinomas of low grade atypia (Grade 1), whereas it was not detected in 11 intraductal papillomas. Therefore, inactivation of tumor-suppressor genes on chromosome 16q was suggested to be involved in acquisition of malignant phenotype rather than in tumorigenesis in mammary gland epithelial cell. Examination of LOH on 16q should be helpful for differential diagnosis of intracystic papillary tumors.

Linkage analysis of BRCA1 in Japanese breast cancer families.

We examined the involvement of BRCA1, which plays a major role in Western breast cancer families, in Japanese breast cancer families. Eleven families, in which at least three individuals within third degree relatives were affected by breast cancer, were collected. Five of them were early-onset breast cancer families, in which the average age at diagnosis was less than 45 years, and the other six were late-onset families. Ovarian cancer was observed in one patient in the early-onset families. Using seven polymorphic markers on chromosome 17q21, D17S250, ERBB2, THRA1, D17S579, D17S588, GIP and NME1, linkage to BRCA1 was analyzed. Linkage was not detected in any single family. Assuming homogeneity in an inherited component that confines the susceptibility to breast cancer in all families, we summed the LOD scores of all families. The cumulative LOD score obtained was -1.86 for D17S588 at theta = 0.001, indicating no linkage with BRCA1. Since the proportion of families linked to BRCA1 is larger in Western early-onset breast cancer families than in late-onset ones, we also summed the LOD scores of five early-onset families. However, again a negative LOD score was obtained. These results suggest that BRCA1 is not a major breast cancer susceptibility gene in Japanese familial breast cancer.
