Difference between continuous wave and superpulse carbon dioxide laser in bladder surgery.

Adult exteriorized rabbit bladders were treated with both continuous wave and rapid superpulsed laser energy. A carbon dioxide laser was utilized in these studies. Time and power density were varied, and the effect in both acute and delayed preparations was noted. An additional group of animals treated with the carcinogen dibutylnitrosamine was studied in a similar manner. The depth of tissue ablated was monitored by the operating microscope and varied from 1 to 2 mm., using the power-time grid we employed. Not only could the depth of destruction be controlled, but also re-epithelialization was apparent in one and one-half to three and one-half weeks after destruction. Differences between superpulsed and continuous wave carbon dioxide laser energy are compared.

Observations of carbon dioxide laser and bone bur in the osteotomy of the rabbit tibia.

The tissue responses of a rabbit tibia to superpulse laser osteotomy, continuous-wave laser osteotomy, and bur osteotomy were compared. Healing of the bone occurred after use of all three modalities, but there was a delay in healing after laser osteotomy when compared with bur osteotomy. Laser osteotomy is feasible, but improvements will be needed to make such bone cutting as efficient and controllable as the rotating bur.

IGA nephropathy in HLA-identical siblings.

This report describes a patient with end stage IgA nephropathy who received a renal transplant from his asymptomatic HLA-identical brother. A biopsy of the donor kidney performed at the time of transplantation showed evidence of widespread electron-dense mesangial deposits. On immunofluorescence these deposits stained with IgA, documenting clinically occult IgA nephropathy in this otherwise healthy donor. These findings are of particular interest in view of the association of IgA nephropathy with the HLA-Bw35 alloantigen, and raise the possibility that asymptomatic disease, already present in a donor kidney, may have accounted for what has previously been called "recurrence" of this disease in renal allograft recipients.

Healing of continuous-wave and rapid superpulsed, carbon dioxide, laser-induced bone defects.

With rabbits as the experimental model, a comparative study was done to assess the healing of bone in response to osteotomy by rapid superpulsed and continuous-wave carbon dioxide lasers. The laser was capable of easily incising bone; the rapid superpulsed mode required less energy to do so than did the continuous-wave mode. When the narrow zone of tissue necrosis adjacent to the laser osteotomy was debrided, it was found that healing was similar for each laser mode. Healing occurred in orderly progression from deposition of trabecular bone to formation of lamellar bone.

Chronic renal vein thrombosis.

Twenty-eight patients with demonstrated chronic renal vein thrombosis were studied. In seven, only small venous channels were involved; in 21, both small and large veins were thrombosed. A constellation of findings occurred with such frequency in these patients that we believe it virtually diagnostic of renal venous obstruction. These findings include the nephrotic syndrome, great variability in proteinuria and glomerular filtration rate, pulmonary embolization, sterile pyuria, hematuria, hyperchloremic acidosis, decreased renal tubular threshold for glucose and increased fibrin degradation products. These findings are an indication for definitive angiographic and biopsy procedures. Prolonged anticoagulant therapy was generally very effective.
