Hemodynamic effects of [Sar1, Ile8] AII, an angiotensin II analog, in the renin subgroups of essential hypertension.

Hemodynamic effects of [Sar1, Ile8] AII, an angiotensin II analog, were studied in 30 patients with essential hypertension, who were subdivided into 11 low renin, 10 normal renin and 9 high renin groups according to low, normal and high PRA values both before and after furosemide administration (80 mg, orally) plus 4 h of ambulation, respectively. [Sar1, Ile8] AII infusion (600 ng/kg/min) produced significant increases in mean blood pressure (MBP) and total peripheral resistance index (TPRI) in normal renin and low renin groups and significant decreases in MBP and TPRI in high renin group, while the cardiac index and heart rate remained unchanged during the infusion in these three groups. Change in MBP at 30 min of [Sar1, Ile8] AII infusion correlated significantly with alteration in TPRI in 23 patients with essential hypertension, who completed the 30-min infusion. The response of both MBP and TPRI to [SAR1, Ile8] AII also correlated significantly with basal PRA. These results suggest that blood pressure response to [SAR1, Ile8] AII in essential hypertension is primarily due to alteration in total peripheral resistance and that direction and amplitude of the response of both MBP and TPRI are practically dependent on basal PRA levels.

Cronkhite-Canada syndrome. A case report and analytical review of 23 other cases reported in Japan.

A case study is presented of a 57-year-old male who showed typical clinical features of Cronkhite-Canada syndrome. Numerous polypoid lesions were found in the stomach, duodenum, ileum, colon and rectum accompanied with characteristic ectodermal changes. Tests indicated a protein-losing gastroenteropathy. Intestinal lactase deficiency was demonstrated by the lactose tolerance test. Scanning electronmicroscopy of the gastric and colonic mucosa revealed prominent secretion of mucoid substances and distortion in the gastric pits and colonic crypts. These abnormal findings were interpreted as having a direct relationship to the loss of protein into the gastrointestinal tract.

Occurrence of renovascular hypertension in two sisters.

Renovascular hypertension in two sisters, aged 22 and 20, respectively, has been described. Renal artery stenoses were observed unilaterally in the elder patient and bilaterally in the younger one. In both patients, the functional significance of unilateral stenosis of the renal artery was documented by the renal vein renin ratio between the affected side and the contralateral or less affected side. High blood pressure and elevated plasma renin activity have been normalized with a unilateral revascularization in the elder patient, and with the treatment of propranolol in the younger one. The histological examination of the stenotic renal artery in the elder patient showed a finding comparable to the perimedial fibroplasia in Harrison and McCormack's classification of idiopathic fibromuscular stenosis.

Hemodynamics of hyperthyroidism. The effects of autonomic nervous blocking and anti-thyroid drug treatment.

This work was intended to analyze the sympathetic and parasympathetic factors affecting the hemodynamics of hyperthyroidism. Seven patients with hyperthyroidism, diagnosed based on the determinations of BMR, 131I-uptake, T3-resin sponge uptake (T3-RSU), and serum level of thyroxine (T4) were subjected to the study. The hemo-dynamic estimation was done (1) at rest, (2) after vagus blocking by the injection of 0.04 mg/Kg b.wt. atropine and 0.2 mg/Kg b.wt. propranolol before (hyperthyroid state=H) and after (euthyroid state=E) anti-thyroid drug therapy. Cardiac output was measured by the dye-dilution method, and the valve was calculated according to the standard Hamilton formula. The following results were obtained. (1) The vagal tone is lower in H than in E, on the other hand, sympathetic beta-receptor tone is higher in H than in E. (2) The sympathetic alpha-receptor tone in H may be equivalent to that in E. (3) There might exist inotropic factors which affect the hemo-dynamics, other than autonomic nervous system in H.
