Surgical strategy for combined wounds of vessels and internal organs.

Surgical strategy and outcomes were analysed for 51 patients with combined wounds of internal organs and major blood vessels, with total 87 organ and 65 vessel injures. In 29 cases intracavity vessels were injured, in 22 - peripheral ones. In 47 cases (92.1%) traumas were gunshot. On admission 43 patients were in poor clinical condition with III-IV grade shock. The delay of surgical intervention did not exceed 30-40 min, all patients were operated in emergency, regardless of complete preoperative preparation. Blood reinfusion was used for all intracavity bleedings. The consequence of surgical steps was determined by localization of vascular trauma, bleeding intensity and severity of organ injuries. In order to decrease invasiveness and duration of surgery, conventional wound suturing was used for 53 organs, vascular plasty and sutures - in 35 cases, ligation of vessels - in 27. Lobectomy was executed in 1 case, intestinal resection and anastomosis - in 8, ileo-and colostomy - in 7 patients. Total 73 internal organs and 62 vessels were reconstructed. Postoperative complications have developed in 30 patients (58.8%), lethal outcome occurred in 3 patients during operation and in 10 (25.4%) - postoperatively. The majority of complications and deaths were observed for thoracoabdominal traumas. High postoperative mortality was also seen for patients with combined injuries of abdominal organs and vessels (13.7%).

[Functional state of the myocardium in patients with renovascular hypertension].

The authors studied the changes in the mass and contractile function of the myocardium in 24 patients with vasorenal hypertension before operation and in the late periods after surgical correction of the renal blood flow. It was found that in vasorenal hypertension the mass of the myocardium doubled while its contractile and pumping functions diminished. In the late periods after adequate surgical correction of renal blood flow with a good hypotensive effect hypertrophy of the myocardium regresses and its contractile function improves.

[Advantages of endarterectomy of the carotid arteries under local anesthesia].

Analysis of the postoperative results of carotid endarterectomy under local anesthesia in 40 patients showed the possibilities of this type of anesthesia in evaluation of cerebral collateral circulation. The method allowed the use of temporary intraluminal shunt to be rejected in all patients and intraoperative ischemic complications on the part of the brain were not encountered in any of them. A high correlation between the results of EEG and the intraoperative evaluation of tolerance of the brain to ischemia was registered.

[Evaluation of reserve potentials of the heart in diseases of the abdominal aorta in middle aged and elderly patients].

By the method of radionuclide ventriculography on the nuclear stethoscope, 106 patients at the age of 60-84 years with occlusive lesions (64) and abdominal aortic aneurysm (42) were examined. After preoperative therapy, 48 patients underwent resection of the abdominal aortic aneurysm with grafting, 41--aortofemoral shunting. Three (7.5%) patients developed myocardial infarction after the operation, one patient died.

[Surgical treatment of sequelae in injuries of main peripheral blood vessels].

The authors examined 46 patients with sequelae of injuries to the peripheral vessels: pulsatile hematoma (in 12), traumatic aneurysm (in 21), arteriovenous aneurysm (in 5), and disease of a ligated vessel (in 8). Operations were carried out on 45 patients. Favourable results were produced in 92.7% of 41 patients who were treated by restorative operations. Replacement of the involved vascular segment by an autologous vein was the operation of choice in this case.

[Evaluation of pump function of the heart in patients with occlusive diseases of the abdominal aorta and ischemic heart disease based on radionuclide ventriculography].

Altogether 56 patients with occlusive abdominal aortic lesions and concomitant CHD were examined by radionuclide ventriculography on a nuclear stethoscope. Suppression of left ventricular systolic and diastolic function was noted in the patients with CHD and arterial hypertension. Investigations of cardiohemodynamics using the above method made it possible to identify patients with limited functional cardiac reserve, to predict possible myocardial complications, and to take preventive measures before, during and after reconstructive interventions on the abdominal aorta.

[Indications for cerebral revascularization in patients with combined occlusive diseases of the renal and extracranial arteries].

The authors have performed a complex ultrasonic angiographic and nephrological examination of 32 patients with associated occluding injuries of branches of the arch and renal arteries. The indications and tactics of reconstructive interventions on renal arteries in patients with pathology in question have been established which were not followed by neurological complications. All the patients who survived have shown substantial improvement of neurological symptoms.

[Volume load in the evaluation of functional reserves of the heart in reconstructive surgery of the abdominal aorta and its branches].

A review of the results of volume load tests, carried out in the presence of intracardiac hemodynamic monitoring in 19 patients with abdominal aortic diseases and attendant coronary heart disease, has demonstrated that the productivity curve slope and left-heart pumping coefficient are the most valuable criteria for the assessment of cardiac functional reserve. Volume load tests identified 12 patients with limited functional reserve of the heart as being in need of continuous intra- and postoperative inotropic support and pre- and poststress control in the presence of central hemodynamic monitoring.

[External respiratory function in echinococcosis].

The paper presents the results of a study of the parameters of external respiratory function in 33 patients with echinococcosis. Restrictive respiratory dysfunction was detected in 85.71% of the examinees. After inhalation tests, better parameters of external respiratory function were documented.
