[Use of prazosin and capoten in the treatment of heart failure in patients with ischemic heart disease].

Eighty eight patients with ischemic heart disease were studied. There was a significant increase in pulmonary blood flow time (Tp), pulmonary blood volume (BVp). mean pulmonary pressure (MPP), and total pulmonary resistance (TPR). as regarded as initial signs of heart failure, in group 1 that comprised 35 patients out of 65 after a graded isometric loading test. After prazosin therapy, these patients showed a significant decrease in blood pressure, only moderate reduction in MPP and TPR, and a rise of the cardiac index (CI). There was a significant decrease in Tp, BVp, MPP, TPR during the loading test following the therapy. From group 2, 23 patients were treated with capoten. The monotherapy was beneficial in 9 patients, capoten was discontinued in 1 case; 13 patients received a combined therapy with glycosides and saluretics. In all the cases, a positive clinical and hemodynamic effect (lower blood pressure, total peripheral resistance, BVp, higher CI with virtually unchanged heart rate) was achieved.

[Changes in the basic central hemodynamic indices in healthy persons and hypertension patients under physical loading].

In moderate exercise the rate of cardiac contractions is clearly increased in healthy individuals as well as their systolic pressure and minute volume, and to a lesser extent, the stroke volume too; the diastolic arterial pressure hardly changes in diminution of the total peripheral resistance. The systolic and diastolic pressure increase during exercise in patients with hypertensive disease to a greater extent than in healthy individuals. The stroke volume at the early stage of the disease increases to a lesser extent than in healthy individuals, while in the late stage it decreases. Total peripheral resistance in the late stage of the diseases reduces less than in healthy individuals.

[Myocardial contractile function and systemic hemodynamics in hypertensive crises].

The main parameters of central hemodynamics and myocardial contractile function were studied according to the results of the phase analysis of left-ventricular systole in 74 patients with stable stage hypertensive disease during a crisis increase of pressure. It was established that during a crisis increase of pressure, structural changes occur in central hemodynamics, whereas the value of the cardiac output varies within wide limits and the peripheral resistance is obviously increased in all cases. The maximum mobilization of compensatory and reserve possibilities is manifested in intensified inotropic function of the myocardium which contributes to the maintenance of the pumping function and circulation at a level close to normal.
